Descrizione
Criterio di inclusione:
- identificarsi come immigrato cinese;
- avere tra i 18 e i 70 anni;
- avere in cartella clinica una diagnosi di diabete di tipo 2 (T2D);
- hanno avuto un appuntamento con un medico per la cura di routine del T2D negli ultimi 12 mesi;
- avere un HbA1c più recente pari ad almeno l'8%;
- essere disponibile a ricevere brevi video riguardanti la gestione del T2D; E
- possedere uno smartphone o, se sprovvisti, essere disposti e capaci di utilizzare uno smartphone di studio.
Criteri di esclusione:
- incapace o non disposto a fornire il consenso informato;
- incapace di partecipare in modo significativo all'intervento (ad esempio, disturbi della vista e dell'udito non corretti);
- non disposto ad accettare l'assegnazione di randomizzazione;
- è incinta, pianifica una gravidanza nei prossimi 6 mesi o rimane incinta durante lo studio; O
- sta allattando al seno (ad esempio, potrebbero avere potenziali restrizioni dietetiche).

Sperimentale: VIDEO
I partecipanti assegnati al braccio VIDEO riceveranno un breve video di formazione e supporto all'autogestione del diabete (DSMES) a settimana per 24 settimane.
I video verranno consegnati tramite SMS.

I video DSMES forniscono ai partecipanti un’importante formazione sul diabete e consentono loro di diventare un paziente attivo a livello individuale.

Sperimentale: VIDEO+CHW
I partecipanti assegnati al braccio VIDEO+CHW riceveranno un breve video DSMES a settimana, oltre alle chiamate di supporto bisettimanali da parte di un operatore sanitario di comunità (CHW), per 24 settimane.
I video DSMES verranno consegnati tramite SMS.

I video DSMES forniscono ai partecipanti un’importante formazione sul diabete e consentono loro di diventare un paziente attivo a livello individuale.
Gli operatori sanitari valuteranno gli ostacoli dei determinanti sociali della salute (SDOH) dei partecipanti alla cura del diabete di tipo 2 e li collegheranno alle risorse disponibili nella comunità.

Participants assigned to the CONTROL group will continue to receive usual care.
At the end of the study, they will receive DSMES videos.
