Valutazione di un chatbot per massimizzare la valutazione del rischio genetico del cancro ereditario in una popolazione ginecologica scarsamente servita
In questo studio, i ricercatori mirano a confrontare una piattaforma sanitaria mobile, nota come "chatbot", che sfrutta l'intelligenza artificiale e l'elaborazione del linguaggio naturale per ridimensionare la comunicazione, con le "cure abituali" che i pazienti riceverebbero.
Questo confronto consentirà ai ricercatori di determinare se il sistema di chatbot può migliorare i tassi di raccomandazione per i test genetici tra i pazienti a rischio elevato di ospitare una sindrome da cancro familiare in una clinica ginecologica interamente Medicaid.

Criterio di inclusione:
- 8 anni o più.
- Programmato per un nuovo appuntamento paziente nella clinica ginecologica. Parla e legge in inglese.
- Accesso a un telefono con capacità di messaggistica.
- Non ha avuto precedenti test genetici per sindromi tumorali ereditarie.
Criteri di esclusione:
- Minori di 18 anni
- Ha avuto precedenti test genetici per sindromi tumorali ereditarie
- Non legge/parla in inglese
- Non ha accesso a un telefono con funzionalità di messaggistica

Sperimentale: Chatbot
I soggetti riceveranno un messaggio di testo che avvia una conversazione chatbot che si basa sull'elaborazione del linguaggio naturale per raccogliere il cancro personale e familiare.
I soggetti sono identificati dal chatbot come conformi ai criteri di alto rischio del National Comprehensive Cancer Network (NCCN).
Successivamente, i soggetti vengono sottoposti a consulenza genetica pre-test tramite il chatbot e quindi i medici vengono informati (tramite il portale chatbot) che il soggetto soddisfa i criteri ad alto rischio.
Per i soggetti che soddisfano i criteri ad alto rischio (basati sulla valutazione del chatbot), il medico completerà la consulenza genetica e raccomanderà i test genetici durante la visita.
Per i soggetti interessati ai test genetici, il medico ordinerà il test genetico.

Un chatbot è un programma software progettato per simulare una conversazione umana, in genere tramite testo.
I chatbot utilizzano l'elaborazione del linguaggio naturale per raccogliere i dati dei pazienti, anticipare le domande e prevedere le risposte.
In questo studio, il Chatbot simulerà una conversazione simile a un testo con i pazienti tramite uno smartphone e utilizzerà questa piattaforma per affrontare la natura dispendiosa in termini di tempo della raccolta della storia familiare.
I chatbot possono quindi valutare la storia familiare raccolta rispetto alle linee guida mediche per determinare quali pazienti richiedono test genetici.
Per quei pazienti che soddisfano i criteri per i test genetici, il chatbot può offrire formazione pre-test e assistere i medici nell'ordinare test genetici per i pazienti interessati che soddisfano i criteri ad alto rischio.

La storia personale e familiare del cancro sarà raccolta dal medico durante la visita del soggetto.
I medici valuteranno la storia personale/familiare del paziente secondo i criteri ad alto rischio del National Comprehensive Cancer Network (NCCN).
Per i soggetti riconosciuti dal medico come conformi ai criteri NCCN, il medico completerà la consulenza genetica e raccomanderà i test genetici.
Per i soggetti interessati ai test genetici, il medico ordinerà il test genetico.

The validated survey known as the Hospital Anxiety and Depression Scale (HADS) will enable these patients to identify various facilitators that influenced their decision not to complete genetic testing.
Specifically, the HADS survey has 7 questions each to assess depression and anxiety subscales.
Scoring for each item ranges from 0 to 3, with 3 denoting highest anxiety or depression level, and 0 denoting the lowest.
A total subscale score of >8 points out of a possible 21 denotes considerable symptoms of anxiety or depression.

Specifically, the NCCN Distress Thermometer allows patients to self report how much distress they have been experiencing in the past week on a scale from 0 (no distress) to 10 (extreme distress).

Specifically, the Satisfaction with genetic assessment includes the review of a patient's personal and family history with regards to cancer that was performed by the medical provider during the patient's Gynecology New Patient Visit.

Specifically, the Feelings about Genomic Testing Results (FACToR) survey asks 12 questions regarding how the patient felt after they received their genetic results, indicating within the past week if they had the specific feeling: not at all, a little, somewhat, a good deal, or a great deal.

Specifically, the Perceptions of Uncertainties in Genomic Sequencing (PUGS) survey assesses patient's certainty about 8 aspects of their genetic testing, from 1 (very uncertain) to 5 (very certain).
