バックグラウンド:
毎年、米国では 400,000 人以上が冠状動脈性心臓病で突然死亡しています。 大多数は冠状動脈性心臓病を知っています。 これらのうち、心筋梗塞後の集団が大きな割合を占めています。 急性心筋梗塞から回復した患者の約 8 ~ 15% が翌年に死亡します。 これらの死の半分は通常、おそらく不整脈による突然死です。 高齢、心室機能の低下、および心室性不整脈の存在により、心臓突然死および全死因死亡のリスクが高い心筋梗塞後の患者を特定できます。
多くの臨床試験で、急性または慢性の抗不整脈薬療法が心筋梗塞後の患者の死亡率を低下させることができるかどうかが評価されています。 これらのうち、急性静脈内および長期ベータ遮断薬の使用のみが、単独または併用で死亡率を低下させることが示されていました. しかし、ベータブロッカーは抗不整脈剤であることに加えて多くの作用があり、これらの他の効果が寿命を延ばす上で特に重要であった可能性があります.
他の抗不整脈薬の臨床試験では、治療による有意な効果は示されておらず、肯定的な傾向を示すことができなかったものも多数ありました. 心室性期外脱分極の治療自体が、死亡率の低下や突然死にさえつながらない可能性は確かにありました。 しかし、行われた研究は、この問題に十分に対処していませんでした。 ほとんどの研究は小規模であり、頻繁な異所性拍動に基づいて患者を選択していませんでした。 さらに、最適な用量の適切な薬が使用されていない可能性があり、副作用が潜在的な利益をはるかに上回っている可能性があります. おそらく悪影響による不十分なコンプライアンスも、有益な結果の機会を制限している可能性があります.
これらの点のいくつかに対処するために、国立心肺血液研究所は 1982 年に心不整脈パイロット研究を開始しました。 目的は次のことを評価することでした。 1年以上にわたって心室の早期脱分極を効果的かつ安全に減少させる1つまたは複数の薬物が見つかるかどうか。外来心電図を使用して線量調整を行うことができるかどうか。そして、良好な患者コンプライアンスを維持できるかどうか。 500 人の患者を登録した心不整脈パイロット研究では、プラセボに対して 4 つの有効な薬剤 (エンカイニド、エトモジン、フレカイニド、イミプラミン) を評価しました。 この研究は小さすぎて、これらの薬物のいずれかが死亡率または主要な罹患率に影響を与えたかどうかを判断できませんでした. この調査は 1986 年 7 月に完了しました。

デザインの物語:
無作為化、二重盲検。 登録は 1987 年 6 月に始まり、27 の臨床センターが 4,400 人の心筋梗塞後の患者をプラセボまたはエンカイニド、フレカイニド、またはモリシジンによる治療に無作為に割り付け始めました。 実際の無作為化の前に、治療に反応した患者を特定するための非盲検滴定期間がありました。 反応した合計 1,727 人の患者が無作為化されました。1,455 人がエンカイニド、フレカイニド、またはプラセボに、272 人がモリシジンまたはプラセボに割り当てられました。 1989 年 4 月、エンカイニドとフレカイニドは総死亡率の増加と突然の不整脈死のために中止されました。 CAST は 1,300 人の患者を対象にモリシジンとプラセボの比較を 1991 年 8 月まで 18 か月間続けましたが、死亡者数が多かったために試験のモリシジン部分が早期に中止されました。 フルスケール試験の主要アウトカム変数は心臓突然死であり、総死亡率が副次的エンドポイントでした。 データ分析は 1998 年 3 月まで続けられました。
