A cada ano, mais de 400.000 pessoas nos Estados Unidos morrem repentinamente de doenças coronárias. A maioria tem doença cardíaca coronária conhecida. Destes, a população pós-infarto do miocárdio constitui uma grande proporção. Cerca de 8 a 15 por cento dos pacientes que se recuperam de um infarto agudo do miocárdio morrem no ano seguinte. Metade dessas mortes são geralmente súbitas, presumivelmente devido a arritmia. Idade avançada, função ventricular deficiente e presença de arritmias ventriculares podem identificar pacientes pós-IAM com alto risco de morte súbita cardíaca e mortalidade por todas as causas.
Vários ensaios clínicos avaliaram se a terapia medicamentosa antiarrítmica aguda ou crônica pode reduzir a mortalidade em pacientes pós-IM. Destes, apenas o uso de betabloqueadores intravenosos agudos e de longo prazo, de forma independente e em combinação, demonstrou reduzir a mortalidade. No entanto, os betabloqueadores têm muitas ações além de serem agentes antiarrítmicos e é possível que esses outros efeitos tenham sido particularmente importantes no prolongamento da vida.
Nenhum dos ensaios clínicos de outras drogas antiarrítmicas mostrou benefícios significativos do tratamento, e alguns até falharam em mostrar uma tendência positiva. Certamente era possível que o tratamento das despolarizações ventriculares prematuras, por si só, não levasse à redução da mortalidade, ou mesmo à morte súbita. Os estudos que haviam sido feitos, no entanto, não abordaram adequadamente a questão. A maioria dos estudos era pequena e não selecionava pacientes com base em batimentos ectópicos frequentes. Além disso, medicamentos apropriados em doses ideais podem não ter sido usados e os efeitos adversos podem ter superado qualquer benefício potencial. A baixa adesão, talvez devido a efeitos adversos, também pode ter limitado a oportunidade de um resultado benéfico.
Em um esforço para abordar alguns desses pontos, o National Heart, Lung, and Blood Institute iniciou o Estudo Piloto de Arritmia Cardíaca em 1982. Os objetivos foram avaliar: se os pacientes pós-IAM com arritmia ventricular documentada poderiam ser identificados e inscritos em um ensaio clínico duplo-cego; se um ou mais medicamentos poderiam ser encontrados para reduzir de forma eficaz e segura as despolarizações ventriculares prematuras no período de um ano; se o ajuste de dose pode ser realizado, usando ECG ambulatorial; e se a boa adesão do paciente poderia ser mantida. O Cardiac Arrhythmia Pilot Study, que envolveu 500 pacientes, avaliou quatro drogas ativas (encainida, etmozina, flecainida, imipramina) contra placebo. O estudo foi muito pequeno para determinar se algum desses medicamentos teve efeito sobre a mortalidade ou morbidade maior. O estudo foi concluído em julho de 1986.
O estudo piloto demonstrou que o recrutamento de pacientes era viável, que o ajuste de dose poderia ser realizado e que uma boa adesão ao protocolo poderia ser alcançada. Devido aos resultados encorajadores do estudo piloto, o NHLBI realizou um estudo em grande escala.
NARRATIVA DO DESENHO:
Randomizado, duplo-cego. A inscrição começou em junho de 1987, quando vinte e sete centros clínicos começaram a randomizar 4.400 pacientes pós-infarto do miocárdio para placebo ou tratamento com encainida, flecainida ou moricizina. Antes da randomização real, houve um período de titulação aberta para identificar os pacientes que responderam ao tratamento. Um total de 1.727 pacientes que responderam foram randomizados: 1.455 para encainida, flecainida ou placebo e 272 para moricizina ou placebo. Em abril de 1989, a encainida e a flecainida foram descontinuadas devido ao aumento da mortalidade total e morte súbita por arritmia. O CAST continuou a comparar a moricizina com o placebo em 1.300 pacientes por 18 meses até agosto de 1991, quando a porção moricizina do estudo foi interrompida precocemente devido ao excesso de mortes. A variável de desfecho primário no estudo em grande escala foi a morte súbita cardíaca, com a mortalidade total como desfecho secundário. A análise dos dados continuou até março de 1998.
