Cada año, más de 400,000 personas en los Estados Unidos mueren repentinamente de enfermedad coronaria. La mayoría tiene enfermedad coronaria conocida. De estos, la población posinfarto de miocardio constituye una gran proporción. Alrededor del 8 al 15 por ciento de los pacientes que se recuperan de un infarto agudo de miocardio mueren en el año siguiente. La mitad de estas muertes suelen ser repentinas, presumiblemente debido a la arritmia. La edad avanzada, la mala función ventricular y la presencia de arritmias ventriculares pueden identificar a los pacientes después de un IM con alto riesgo de muerte súbita cardíaca y mortalidad por todas las causas.
Varios ensayos clínicos han evaluado si la terapia con fármacos antiarrítmicos agudos o crónicos puede reducir la mortalidad en pacientes que han sufrido un IM. De estos, solo el uso de betabloqueantes intravenosos agudos y a largo plazo, de forma independiente y en combinación, ha demostrado reducir la mortalidad. Sin embargo, los bloqueadores beta tienen muchas acciones además de ser agentes antiarrítmicos y es posible que estos otros efectos hayan sido particularmente importantes para prolongar la vida.
Ninguno de los ensayos clínicos de otros fármacos antiarrítmicos mostró beneficios significativos del tratamiento, y algunos incluso no lograron mostrar una tendencia positiva. Ciertamente, era posible que el tratamiento de las despolarizaciones ventriculares prematuras, en sí mismo, no condujera a una reducción de la mortalidad, o incluso a la muerte súbita. Sin embargo, los estudios que se habían realizado no habían abordado adecuadamente la cuestión. La mayoría de los estudios fueron pequeños y no seleccionaron pacientes sobre la base de latidos ectópicos frecuentes. Además, es posible que no se hayan utilizado los medicamentos apropiados en dosis óptimas y que los efectos adversos hayan superado cualquier beneficio potencial. El cumplimiento deficiente, quizás debido a los efectos adversos, también puede haber limitado la oportunidad de un resultado beneficioso.
En un esfuerzo por abordar algunos de estos puntos, el Instituto Nacional del Corazón, los Pulmones y la Sangre inició el Estudio piloto de arritmia cardíaca en 1982. Los objetivos fueron evaluar: si los pacientes después de un IM con arritmia ventricular documentada podrían identificarse e inscribirse en un ensayo clínico doble ciego; si se podrían encontrar uno o más fármacos para reducir de manera efectiva y segura las despolarizaciones prematuras ventriculares durante un período de un año; si se puede realizar un ajuste de dosis mediante ECG ambulatorios; y si se podría mantener un buen cumplimiento del paciente. El estudio piloto de arritmia cardíaca, en el que participaron 500 pacientes, evaluó cuatro fármacos activos (encainida, etmozina, flecainida, imipramina) frente a un placebo. El estudio fue demasiado pequeño para determinar si alguno de estos medicamentos tuvo un efecto sobre la mortalidad o la morbilidad importante. El estudio se completó en julio de 1986.
El estudio piloto demostró que el reclutamiento de pacientes era factible, que se podía lograr el ajuste de dosis y que se podía lograr un buen cumplimiento del protocolo. Debido a los alentadores resultados del estudio piloto, el NHLBI realizó una prueba a gran escala.
NARRATIVA DE DISEÑO:
Aleatorizado, doble ciego. La inscripción comenzó en junio de 1987 cuando veintisiete centros clínicos comenzaron a asignar al azar a 4400 pacientes que habían sufrido un infarto de miocardio para recibir placebo o tratamiento con encainida, flecainida o moricizina. Antes de la aleatorización real, hubo un período de titulación abierto para identificar a los pacientes que respondieron al tratamiento. Un total de 1727 pacientes que respondieron fueron aleatorizados: 1455 a encainida, flecainida o placebo y 272 a moricizina o placebo. En abril de 1989, se suspendieron la encainida y la flecainida debido al aumento de la mortalidad total y la muerte súbita por arritmia. CAST continuó comparando la moricizina con el placebo en 1300 pacientes durante 18 meses hasta agosto de 1991, cuando la porción de moricizina del ensayo se detuvo antes de tiempo debido al exceso de muertes. La variable de resultado principal en el ensayo a gran escala fue la muerte cardíaca súbita, con la mortalidad total como criterio de valoración secundario. El análisis de datos continuó hasta marzo de 1998.
