ARRIÈRE-PLAN:
Chaque année, plus de 400 000 personnes aux États-Unis meurent subitement d'une maladie coronarienne. La majorité a connu une maladie coronarienne. Parmi ceux-ci, la population post-infarctus du myocarde constitue une grande proportion. Environ 8 à 15 % des patients qui se rétablissent d'un infarctus aigu du myocarde meurent l'année suivante. La moitié de ces décès sont généralement soudains, vraisemblablement dus à une arythmie. Un âge avancé, une mauvaise fonction ventriculaire et la présence d'arythmies ventriculaires peuvent identifier les patients post-IM à haut risque de mort cardiaque subite et de mortalité toutes causes confondues.
Un certain nombre d'essais cliniques ont évalué si un traitement médicamenteux anti-arythmique aigu ou chronique peut réduire la mortalité chez les patients post-IM. Parmi ceux-ci, seule l'utilisation de bêta-bloquants intraveineux aigus et à long terme, indépendamment et en combinaison, s'était avérée efficace pour réduire la mortalité. Cependant, les bêta-bloquants ont de nombreuses actions en plus d'être des agents anti-arythmiques et il est possible que ces autres effets aient été particulièrement importants pour prolonger la vie.
Aucun des essais cliniques portant sur d'autres médicaments antiarythmiques n'avait montré d'avantages significatifs du traitement, et un certain nombre n'avaient même pas montré de tendance positive. Il était certainement possible que le traitement des dépolarisations ventriculaires prématurées, en lui-même, n'entraîne pas une réduction de la mortalité, voire une mort subite. Cependant, les études qui avaient été faites n'avaient pas suffisamment abordé la question. La plupart des études étaient de petite taille et ne sélectionnaient pas les patients sur la base de battements ectopiques fréquents. De plus, des médicaments appropriés à des doses optimales peuvent ne pas avoir été utilisés, et les effets indésirables peuvent bien avoir dépassé tout avantage potentiel. Une mauvaise observance, peut-être due à des effets indésirables, peut également avoir limité la possibilité d'obtenir un résultat bénéfique.
Dans un effort pour aborder certains de ces points, le National Heart, Lung, and Blood Institute a lancé l'étude pilote sur l'arythmie cardiaque en 1982. Les objectifs étaient d'évaluer : si les patients post-IM présentant une arythmie ventriculaire documentée pouvaient être identifiés et enrôlés dans un essai clinique en double aveugle ; si un ou plusieurs médicaments pouvaient être trouvés pour réduire efficacement et en toute sécurité les dépolarisations ventriculaires prématurées sur une période d'un an ; si un ajustement de la dose pouvait être effectué à l'aide d'ECG ambulatoires ; et si une bonne observance du patient pouvait être maintenue. L'étude pilote sur l'arythmie cardiaque, qui a recruté 500 patients, a évalué quatre médicaments actifs (encaïnide, ethmozine, flécaïnide, imipramine) par rapport à un placebo. L'étude était trop petite pour déterminer si l'un de ces médicaments avait un effet sur la mortalité ou la morbidité majeure. L'étude a été achevée en juillet 1986.
L'étude pilote a démontré que le recrutement des patients était faisable, qu'un ajustement de la dose pouvait être réalisé et qu'une bonne conformité au protocole pouvait être obtenue. En raison des résultats encourageants de l'étude pilote, le NHLBI a mené un essai à grande échelle.
NARRATIF DE CONCEPTION :
Randomisé, en double aveugle. L'inscription a commencé en juin 1987 lorsque vingt-sept centres cliniques ont commencé à randomiser 4 400 patients post-infarctus du myocarde pour recevoir un placebo ou un traitement par encaïnide, flécaïnide ou moricizine. Avant la randomisation proprement dite, il y avait une période de titration en ouvert pour identifier les patients qui répondaient au traitement. Un total de 1 727 patients qui ont répondu ont été randomisés : 1 455 pour l'encaïnide, la flécaïnide ou un placebo, et 272 pour la moricizine ou un placebo. En avril 1989, l'encaïnide et la flécaïnide ont été arrêtées en raison de l'augmentation de la mortalité totale et de la mort subite par arythmie. CAST a continué à comparer la moricizine à un placebo chez 1 300 patients pendant 18 mois jusqu'en août 1991, date à laquelle la partie moricizine de l'essai a été arrêtée prématurément en raison de décès excessifs. La principale variable de résultat dans l'essai à grande échelle était la mort cardiaque subite, la mortalité totale étant un critère d'évaluation secondaire. L'analyse des données s'est poursuivie jusqu'en mars 1998.
