ACHTERGROND:
Elk jaar sterven meer dan 400.000 mensen in de Verenigde Staten plotseling aan coronaire hartziekten. De meerderheid heeft een bekende coronaire hartziekte. Hiervan vormt de post-myocardinfarctpopulatie een groot deel. Ongeveer 8 tot 15 procent van de patiënten die herstellen van een acuut myocardinfarct sterft in het volgende jaar. De helft van deze sterfgevallen is meestal plotseling, vermoedelijk als gevolg van aritmie. Hoge leeftijd, slechte ventriculaire functie en aanwezigheid van ventriculaire aritmieën kunnen post-MI-patiënten identificeren met een hoog risico op plotselinge hartdood en mortaliteit door alle oorzaken.
Een aantal klinische onderzoeken hebben geëvalueerd of acute of chronische anti-aritmische medicamenteuze therapie de mortaliteit bij post-MI-patiënten kan verminderen. Hiervan bleek alleen het gebruik van acute intraveneuze en langdurige bètablokkers, onafhankelijk en in combinatie, de mortaliteit te verminderen. Bètablokkers hebben echter veel andere werkingen dan dat ze anti-aritmica zijn en het is mogelijk dat deze andere effecten bijzonder belangrijk zijn geweest bij het verlengen van het leven.
Geen van de klinische onderzoeken met andere antiaritmica had significante voordelen van de behandeling aangetoond, en een aantal had zelfs geen positieve trend laten zien. Het was zeker mogelijk dat de behandeling van ventriculaire premature depolarisaties op zichzelf niet leidde tot een vermindering van de mortaliteit of zelfs tot een plotselinge dood. De onderzoeken die waren gedaan, hadden dit probleem echter onvoldoende aangepakt. De meeste onderzoeken waren klein en er werden geen patiënten geselecteerd op basis van frequente ectopische slagen. Bovendien is het mogelijk dat de juiste medicijnen in optimale doseringen niet zijn gebruikt en dat de nadelige effecten wellicht groter waren dan de mogelijke voordelen. Slechte naleving, misschien als gevolg van nadelige effecten, kan ook de kans op een gunstig resultaat hebben beperkt.
In een poging om enkele van deze punten aan te pakken, startte het National Heart, Lung, and Blood Institute in 1982 de Cardiac Arrhythmia Pilot Study. De doelstellingen waren om te beoordelen: of post-MI-patiënten met gedocumenteerde ventriculaire aritmie konden worden geïdentificeerd en opgenomen in een dubbelblind klinisch onderzoek; of er een of meer geneesmiddelen gevonden konden worden die ventriculaire premature depolarisaties over een periode van een jaar effectief en veilig verminderen; of dosisaanpassing mogelijk was met behulp van ambulante ECG's; en of een goede therapietrouw door de patiënt kon worden gehandhaafd. De Cardiac Arrhythmia Pilot Study, waaraan 500 patiënten deelnamen, evalueerde vier actieve geneesmiddelen (encaïnide, ethmozine, flecaïnide, imipramine) ten opzichte van placebo. De studie was te klein om te bepalen of een van deze medicijnen een effect had op mortaliteit of ernstige morbiditeit. De studie werd in juli 1986 afgerond.
De pilotstudie toonde aan dat rekrutering van patiënten haalbaar was, dat dosisaanpassing kon worden bereikt en dat een goede naleving van het protocol kon worden bereikt. Vanwege de bemoedigende resultaten van de pilotstudie heeft de NHLBI een volledige proef uitgevoerd.
ONTWERP VERHAAL:
Gerandomiseerd, dubbelblind. De inschrijving begon in juni 1987 toen zevenentwintig klinische centra 4.400 post-myocardinfarctpatiënten randomiseerden voor placebo of behandeling met encaïnide, flecaïnide of moricizine. Voorafgaand aan de daadwerkelijke randomisatie was er een open-label titratieperiode om patiënten te identificeren die op de behandeling reageerden. In totaal werden 1.727 patiënten die reageerden gerandomiseerd: 1.455 naar encaïnide, flecaïnide of placebo en 272 naar moricizine of placebo. In april 1989 werden encaïnide en flecaïnide stopgezet vanwege een verhoogde totale mortaliteit en plotselinge aritmische dood. CAST ging door met het vergelijken van moricizine met placebo bij 1.300 patiënten gedurende 18 maanden tot augustus 1991, toen het moricizine-gedeelte van de studie voortijdig werd stopgezet vanwege te veel sterfgevallen. De primaire uitkomstvariabele in de grootschalige studie was plotselinge hartdood, met totale mortaliteit als secundair eindpunt. De gegevensanalyse ging door tot en met maart 1998.
