2009 年の慢性閉塞性肺疾患に関する世界イニシアチブ (GOLD) ガイドラインで定義されている、安定した中等度から重度の COPD の診断。気管支拡張薬後の FEV1/FVC < 70%、および気管支拡張薬後の FEV1 ≧ 30% かつ < 80% の予測値
現在または元喫煙者

除外基準:

初診前3ヶ月以内にCOPDの急性増悪で入院した患者
訪問1の前の6週間に気道感染症またはCOPDの増悪
COPD以外の臨床的に重大な呼吸器疾患、心血管疾患、または精神疾患を患っている患者
医療記録から確認された喘息の病歴または存在
1日あたり15時間以上の酸素療法の慢性的な使用
HIVおよび/または活動性肝炎による制御不能な感染症を患っている患者
吸入抗コリン薬に対する過敏反応の既往歴のある患者
過去6か月以内の心筋梗塞、過去3か月以内に新たに診断された不整脈、不安定狭心症、薬物療法やその他の介入の変更が必要な不安定不整脈など、臨床的に重大な心血管疾患を患っている患者。
