慢性閉塞性肺疾患(COPD)は予防可能で治療可能な状態であり、人的・経済的に多大なコストを伴う主要な医療問題です。 英国では300万人が影響を受け、140万件の相談があり、3万人が死亡し、年間8億ポンドの費用がかかっている。 新たな高価な介入を見つけるために、多額の研究費が費やされています。 しかし、呼吸リハビリテーション(PR)は、生活の質と日常生活機能の面で患者にとって有益であるという十分な証拠が得られる利用可能な治療選択肢です。
この研究の主な目的は、COPD患者における呼吸リハビリテーション後の維持プログラムの追加の有効性と費用対効果を標準治療と比較して評価することです。
第 2 の目的は、呼吸リハビリテーション プログラムの改善と維持戦略の順守を予測するベースラインの特性を特定することです。 研究者らは、一連の人口統計学的、臨床的、生理学的、心理的、生化学的パラメーターを測定することで、呼吸リハビリテーションから最大の恩恵を受ける可能性が高い患者を予測できることを期待している。

詳細な説明
これは、慢性閉塞性肺疾患(COPD)患者を対象とした維持肺リハビリテーション(PR)プログラムのランダム化対照並行研究となる。 ノリッジでの PR プログラム (下記参照) が正常に完了した後、患者は維持 PR または標準医療を受けるように無作為に割り付けられます。
PR プログラムに登録する前に、患者はインフォームドコンセントを提供した後、標準的なベースライン評価を受けます。 これらには、健康診断、人口統計の詳細、過去の病歴、肺活量測定、予測最大酸素消費量を決定する増分シャトル歩行テスト (ISWT)、予測最大酸素消費量 (V02) の 85% での持久シャトル歩行テスト (ESWT) が含まれます。 )、慢性呼吸器アンケート(CRQ)、EuroQol(EQ5D)、病院不安およびうつ病スコア(HADS)、血清インターロイキン(IL)-6およびC反応性タンパク質(CRP)、肥満指数(BMI)、皮下脂肪の厚さおよび筋肉強さ。
PR の終了時と PR から 12 か月後に、患者は健康診断を受け、予測最大 VO2 の 85% の ESWT、CRQ、EQ5D、HADS、血清インターロイキン (IL)-6 と CRP、BMI、皮下脂肪の厚さと筋力、および前月の活動の評価。
PR 後 3 か月、6 か月、9 か月後に、患者は CRQ と前月の活動を評価するためのアンケートに回答します。 郵送によるアンケートにより実施させていただきます。
ベースラインの社会経済およびコストのアンケートは PR の開始時に記入され、フォローアップのコストのアンケートは PR 後、3、6、9、12 か月後に記入されます。 患者には、すべての来院時にNHSの連絡先、処方箋などを記録する日記カードが渡されます。
3、6、9 か月後のアンケートは郵送で回収されます。

包含基準:
- 35歳以上の男性または女性
- COPD、肺気腫または慢性気管支炎の医師による診断
- 20パック年以上の元または現在の喫煙者
- FEV1 が予測の 80% 未満
- 患者は長時間作用型または短時間作用型の気管支拡張薬および/または吸入または経口コルチコステロイドおよび/またはテオフィリンを服用している可能性があります。
- 初期 PR で運動セッションの少なくとも 60% に参加した患者 (22)* * これは、維持 PR または標準医療を受けるための無作為化の包含基準ですが、研究には参加しません。
除外基準:
- COPD以外の重大な心臓疾患または肺疾患(COPDが患者の症状に与える影響は小さい)。
- 過去6か月以内の心筋梗塞または不安定狭心症
- 呼吸器感染症は、無作為化前4週間以内の咳、抗生物質の使用、または化膿性痰として定義される。
- 研究の結果に影響を与える可能性がある、重度または制御されていない併存疾患。
- 研究で必要な処置を行う患者の能力を制限する認知機能の異常。

運動管理への介入

3 か月ごとに 2 時間のメンテナンス プログラム。
最初の 1 時間で、患者は自宅でのエクササイズをどの程度継続できたかを個別に説明し、このトレーニングの遵守を強化する方法について話し合います。
積極的な再強化が提供されます。
うつ病や社会的孤立が疑われる患者は、追加の管理のためにかかりつけ医に紹介されます。
患者には、呼吸困難の管理戦略、特に日常生活に関連する活動に対する監視下での活動の取り組みと組み合わせた呼吸の制御についてアドバイスされます。
その後、ウォーキング、サイクリング、座った状態からの立ち上がり、ダンベルを使った腕のエクササイズ、ステップアップなど、監督付きの1時間の筋力トレーニングと持久力トレーニングが行われます。
患者は医師から進行状況に関する書面による報告書を受け取り、そのコピーを主治医に受け取ります。
これは、対照群に与えられる標準的なアドバイスに追加されるものになります。
