这将是一项针对慢性阻塞性肺疾病 (COPD) 患者的维持性肺康复 (PR) 计划的随机、对照、平行研究。 在诺里奇成功完成 PR 计划后(见下文),患者将随机接受维持性 PR 或标准医疗护理。
在参加 PR 计划之前,患者将在提供知情同意后接受标准基线评估。 这些将包括体格检查、人口统计详细信息、既往病史、肺活量测定、增量穿梭步行测试 (ISWT) 以确定预测的最大耗氧量、耐力穿梭步行测试 (ESWT) 在预测最大耗氧量 (V02) 的 85% )、慢性呼吸问卷(CRQ)、欧洲生活质量(EQ5D)、医院焦虑抑郁评分(HADS)、血清白细胞介素(IL)-6和C反应蛋白(CRP)、体重指数(BMI)、皮褶厚度和肌肉力量。

描述
纳入标准:
- 男性或女性,年龄超过35岁
- COPD、肺气肿或慢性支气管炎的医师标记诊断
- 超过 20 包年的前吸烟者或现在吸烟者
- FEV1 低于预计值的 80%
- 患者可能正在服用长效或短效支气管扩张剂和/或吸入或口服皮质类固醇和/或茶碱。
- 患者在初始 PR(22) 期间参加了至少 60% 的锻炼课程* * 这是随机分配接受维持性 PR 或标准医疗但不进入研究的纳入标准
排除标准:
- 除 COPD 以外的严重心脏或肺部疾病,因此 COPD 对患者症状的影响很小。

运动管理干预

每 3 个月一次,时长为 2 小时的维护计划。
在第一个小时内,患者将根据个人情况描述他们能够在家中继续锻炼的程度,并讨论加强对这种训练的依从性的方法。
将提供积极的强化。
怀疑患有抑郁症或社会孤立的患者将被转介给他们的全科医生进行额外管理。
将向患者建议呼吸困难管理策略,尤其是控制呼吸并结合对其日常生活相关活动的监督活动。
随后将进行 1 小时的有监督的力量和耐力训练,包括步行、骑自行车、坐姿站立、使用哑铃和台阶进行手臂锻炼。
