これは、デノボ腎臓移植レシピエントにおける迅速なステロイドおよびカルシニューリン阻害剤の離脱を伴うAlemtuzumab導入およびMyforticと組み合わせたAlefaceptの安全性と有効性を評価するための単一施設、研究者主導のパイロット研究です。
導入療法では、周術期にアレムツズマブとステロイドを単回投与します。
タクロリムスは、移植後最初の 30 日間投与されます。アレファセプトは、最初の2回の用量でIV投与され、その後、移植後12週間まで毎週皮下注射され、その後、研究の残りの期間は毎月注射されます。
主な結果は、生検で証明された急性拒絶反応エピソード、感染性合併症、またはその他の深刻な有害事象を含む、安全性と有効性の結果です。
副次的結果には、免疫モニタリングアッセイによって評価される T ヘルパー分化、サイトカイン産生、および T 制御細胞生成が含まれます。

除外基準:
- 以前に腎臓以外の臓器移植を受けた、または受けている
- ヨウ素に対する感受性
- 非心拍ドナーから移植を受けます (心臓死後の寄付 - DCD)
- HLA同一血縁生体ドナーから移植を受ける
- -5歳未満の死亡したドナーから孤立した腎臓を受け取ります
- -予想される冷虚血時間(CIT)が30時間以上の腎臓を受け取ります
- ABO不適合のドナー腎臓を受け取ります
- -レシピエントまたはドナーは、ヒト免疫不全ウイルス(HIV)に対して血清陽性であることが知られています
- -受信者は、治験施設の標準的な決定方法によるT細胞またはB細胞のクロスマッチが陽性です。 フローサイトメトリー クロスマッチが実行され、T または B 細胞検査のいずれかで陽性である受信者の場合、受信者は、ドナー特異的な抗 HLA 抗体がフローサイトメトリー ベースの特異的な抗 HLA 抗体検査によって検出された場合にのみ除外されます。
- -現在の悪性腫瘍または悪性腫瘍の病歴(過去5年以内)、正常に治療された皮膚の非転移性基底または扁平上皮癌を除く
- 重大な肝疾患
- -サイトメガロウイルス(CMV)が血清学的に陰性で、血清学的に陽性のCMVドナー
- エプスタイン・バーウイルスに対して血清学的に陰性
- -治験薬の初回投与の3か月前に静脈内免疫グロブリン(IVIG)療法を受けました

アレムツズマブ誘導およびカルシニューリン阻害剤(CNI)およびコルチコステロイド離脱と組み合わせたアレファセプトの安全性と有効性。

移植後30日目にカルシニューリン阻害剤を中止。
アレファセプト 7.5 mg を術後 0 日目に投与し、術後 2 日目に IV を投与します。アレファセプト 15 mg SQ X 12 週間、その後は月 12 まで毎月。
