胃食道逆流症 (GERD) は、退役軍人によく見られる慢性疾患であり、年間 100 億ドルを超える薬剤費に関連しています。 逆流症状のある患者の 30 ~ 40% だけがびらん性食道炎を患っています。 大多数はNERDに苦しんでいます。標準的な内視鏡検査では粘膜の損傷を特定できず、診断を確定できない状態。 残念ながら、NERD の診断のゴールド スタンダードは存在しません。 24 時間の食道 pH モニタリングと NERD 患者の組織学的食道粘膜の変化は、臨床現場で日常的に使用するには精度が限られています。
狭帯域イメージング (NBI) は、スペクトル狭帯域フィルター (標準の内視鏡に組み込まれています) を利用し、乳頭内毛細血管ループ (IPCL) などの機能のイメージングを可能にします。通常の内視鏡検査では確認できない特徴。 予備的な結果は、NBI 内視鏡検査が NERD の診断において標準的な内視鏡検査よりも大幅に改善される可能性があることを示しています。 私たちの仮説は、NBI が NERD 患者の診断に固有の遠位食道の変化を特定するというものです。 特定の目的 #1: NERD 患者 (ケース) とコントロールの遠位食道における NBI 機能を比較します。 特定の目的 #2: NERD 患者の NBI 機能が PPI 療法後に解消するかどうかを判断すること。 特定の目的 #3: NBI の所見を食道の組織学と相関させること。 特定の目的 #4: NERD を診断するための提案された基準を認識するための観察者内および観察者間の合意を評価すること。 症例は、逆流症状のある患者(2つの検証済みアンケートによって評価)と定義され、巨視的なびらんがなく、異常な食道pHの結果が得られます(NERDグループ)。 対照被験者には、逆流症状がなく、肉眼で見えるびらんがなく、食道pHが正常な患者が含まれます。 反応の予測因子として NBI 所見を特定するために、PPI アームに無作為化された症例の治療に対する反応は、検証済みの GERD アンケートを使用して評価され、IPCL 番号および微小びらんの存在と相関します。 さらに、逆流症状があり、巨視的なびらんがなく、正常な食道pH結果を有する患者におけるNBI所見も、対照と比較される。 デジタル画像とビデオ録画とともに、遠位食道から 2 つの生検が得られます。
統計分析は次のように行われます。目的 1 - カイ 2 乗と t 検定。ロジスティック回帰とオッズ比の計算、目的 2 - マクネマーの検定とカッパ統計、目的 3 - スピアマンの相関係数、目的 4 - クラス内相関係数。 ヘルスケアへの潜在的な影響: GERD は患者の間で一般的であり、追加の調査の必要性を取り除き、不必要な薬剤費を削減することにより、NBI 内視鏡検査は NERD 患者にかなりのプラスの影響を与える可能性があります。

- 18歳
- -インフォームドコンセントを与えることができます。
- NERD の症例は、異常な食道 pH の結果および標準的な内視鏡検査でびらんがないことと組み合わせて、2 つの検証済みの GERD アンケートを使用して、胸やけおよび/または逆流の存在に基づいて募集されます。
- 対照被験者には、鉄欠乏性貧血、ヘム陽性便、とりわけ食道静脈瘤のスクリーニングなどの非逆流症状の評価のために上部内視鏡検査を依頼された患者が含まれます。 食道pHが陰性でびらんがないことが、これらの患者の選択基準になります。
除外基準:
- 肉眼的びらん性食道炎の存在
- 妊娠・授乳期
- 慢性抗凝固療法
- -重大な医学的併存疾患(酸素依存性慢性閉塞性肺疾患、NYHAクラスIIIまたはIVのうっ血性心不全、平均余命が5年未満の癌の最近の診断)を有する患者
- バレット食道の歴史
- 腸上皮化生を伴う内視鏡検査で円柱状に裏打ちされた遠位食道の存在
- 食道または胃のがんまたは腫瘤病変の存在
- 食道狭窄
- 消化性潰瘍およびヘリコバクター・ピロリ感染症
- 食道手術の既往歴
- PPIに対するアレルギー
- ピル関連食道炎を引き起こすことが知られている薬を服用している患者(例: カリウムサプリ)
- 感染性食道炎の可能性があるHIVまたはその他の免疫不全状態の患者
- 好酸球性食道炎
