Diagnose van zure refluxziekte met behulp van nieuwe beeldvorming: een prospectieve studie

Gastro-oesofageale refluxziekte (GERD), een veel voorkomende chronische aandoening bij de veteranenpopulatie, gaat gepaard met medicijnkosten van meer dan $ 10 miljard/jaar. Slechts 30-40% van de patiënten met refluxsymptomen heeft erosieve oesofagitis. De overgrote meerderheid lijdt aan NERD; een aandoening waarbij standaardendoscopie geen mucosale breuken kan identificeren en de diagnose niet kan bevestigen. Helaas bestaat er geen gouden standaard voor de diagnose van NERD. 24-uurs slokdarm-pH-monitoring en histologische slokdarmslijmvliesveranderingen bij NERD-patiënten hebben een beperkte nauwkeurigheid om routinematig in de klinische praktijk te worden gebruikt.
Narrow band imaging (NBI) maakt gebruik van spectrale smalbandfilters (geïntegreerd in standaard endoscopen) en maakt beeldvorming mogelijk van functies zoals intrapapillaire capillaire lussen (IPCL's); functies niet geïdentificeerd tijdens standaard endoscopie. Voorlopige resultaten hebben aangetoond dat NBI-endoscopie een significante verbetering kan betekenen ten opzichte van standaard endoscopie voor de diagnose van NERD. Onze hypothese is dat NBI veranderingen in de distale slokdarm identificeert die specifiek zijn voor het diagnosticeren van patiënten met NERD. Specifiek doel #1: NBI-kenmerken in de distale slokdarm vergelijken bij patiënten met NERD (gevallen) en controles. Specifiek doel #2: bepalen of de NBI-kenmerken bij NERD-patiënten verdwijnen na PPI-therapie. Specifiek doel #3: NBI-bevindingen correleren met slokdarmhistologie. Specifiek doel #4: Het beoordelen van de intra- en interobserver-overeenkomst voor erkenning van de voorgestelde criteria voor het diagnosticeren van NERD. Gevallen zullen worden gedefinieerd als patiënten met refluxsymptomen (beoordeeld door twee gevalideerde vragenlijsten) met afwezige macroscopische erosies en abnormale slokdarm-pH-resultaten (NERD-groep). Controlepersonen omvatten patiënten zonder refluxsymptomen, afwezige macroscopische erosies en een normaal pH-resultaat van de slokdarm. Om NBI-bevindingen te identificeren als voorspellers van respons, zal respons op therapie in gevallen die zijn gerandomiseerd naar de PPI-arm worden beoordeeld met behulp van een gevalideerde GERD-vragenlijst en gecorreleerd met het IPCL-nummer en de aanwezigheid van micro-erosie. Bovendien zullen NBI-bevindingen bij patiënten met refluxsymptomen, geen macroscopische erosies en een normaal slokdarm-pH-resultaat ook worden vergeleken met controles. Er worden twee biopsieën van de distale slokdarm genomen, samen met digitale beelden en video-opnamen.
Statistische analyse wordt als volgt uitgevoerd: Doel 1 - chikwadraat en t-toets; met logistische regressie en berekening van odds ratio's, Doel 2 - McNemar's test en kappa-statistiek, Doel 3 - Spearman's correlatiecoëfficiënt en Doel 4 - intraclass correlatiecoëfficiënt. Potentiële impact op de gezondheidszorg: GORZ komt veel voor bij patiënten en door de noodzaak van aanvullend onderzoek weg te nemen en onnodige medicijnkosten te verminderen, kan NBI-endoscopie een aanzienlijk positief effect hebben op patiënten met NERD.

Beschrijving
Inclusiecriteria:
- 18 jaar oud
- In staat om geïnformeerde toestemmingen te geven.
- Gevallen van NERD zullen worden gerekruteerd op basis van de aanwezigheid van brandend maagzuur en/of regurgitatie met behulp van twee gevalideerde GERD-vragenlijsten in combinatie met een abnormaal slokdarm-pH-resultaat en afwezigheid van erosies bij standaardendoscopie.
- Tot de controlepersonen behoren patiënten die zijn doorverwezen voor een bovenste endoscopie voor evaluatie van niet-refluxsymptomen zoals onder andere bloedarmoede door ijzertekort, heem-positieve ontlasting, screening van slokdarmvarices. Een negatief pH-resultaat in de slokdarm en afwezigheid van erosies zullen inclusiecriteria zijn voor deze patiënten.
Uitsluitingscriteria:
- Aanwezigheid van macroscopische erosieve oesofagitis
- Zwangerschap/borstvoeding
- Chronische antistolling
- Patiënten met significante medische comorbiditeiten (zuurstofafhankelijke chronische obstructieve longziekte, NYHA klasse III of IV congestief hartfalen, recente diagnose van kanker met een levensverwachting < 5 jaar)
- Geschiedenis van de slokdarm van Barrett
- Aanwezigheid van zuilvormig gevoerde distale slokdarm op endoscopie met intestinale metaplasie
- Aanwezigheid van kanker of massalaesie in de slokdarm of maag
- Slokdarmstricturen
- Maagzweerziekte en Helicobacter pylori-infectie
- Voorgeschiedenis van slokdarmchirurgie
- Allergisch voor PPI's
- Patiënten die medicijnen gebruiken waarvan bekend is dat ze pilgerelateerde oesofagitis veroorzaken (bijv. kaliumsupplementen)
- Patiënten met HIV of andere immuungecompromitteerde aandoeningen die mogelijk infectieuze oesofagitis hebben
- Eosinofiele oesofagitis
