冠状動脈バイパス移植術後の初期移行期間は、患者が持続可能な長期的なライフスタイルの変更を行う必要がある、激しい生理学的および心理社会的調整に関連する期間です。 心臓リハビリテーション (CR) は、冠状動脈疾患患者の罹患率と死亡率を低下させるだけでなく、全体的なフィットネスと生活の質を向上させることが証明されている、科学的根拠に基づいたリスク低減プログラムです。 したがって、心臓手術患者に対する早期介入を通じて急性期治療から CR までのギャップを埋める支援戦略の開発が不可欠です。 MyCardiacRecovery (MyCaRe) は、標準化された教育形式と、退院後最初の 6 ~ 8 週間のインタラクティブな追跡 (写真機能と Fitbit Inspire 2 加速度計を使用した創傷治癒と活動の進行) サポートを含むインタラクティブなプラットフォームです。 このアプリケーションは、以下の機能を提供することで、患者と家族が一連の治療を進めるのに役立ちます。 a) 急性期治療と外来心臓リハビリテーションの間の統合リンクにより、情報を効率的に調整し、サービスの重複を削減します。 b) 回復に関する顕著な疑問に対処するために設計された患者ケアおよび教育資料。 c) 患者と医療提供者間のコミュニケーションを改善し、d) CR への合理化された系統的な紹介を確保します。 この革新的な戦略は、患者の満足度にプラスの影響を与え、患者の転帰を改善し、場合によっては医療システムに課せられる財政的制約を最小限に抑える可能性を秘めています。 プロトタイプ 1.0 のフェーズ 1 (予備的なユーザビリティ テスト) が完了しました。 フェーズ 2 では、パイロットランダム化対照試験デザインを使用して、心臓手術後の患者のサンプルで MyCaRe 2.0 をテストすることに焦点を当てます。

サンプルの包含基準には次のものが含まれます。
- 学習およびアプリ/Fitbit の使用に関する指示を理解する、
- 35歳以上、
- 従来の(胸骨切開アプローチ)冠動脈バイパス移植(CABG)手術を受けている、
- 術後の経過は複雑ではなく、
- 標準的な入院期間(4~8日)、
- 自宅で Wi-Fi インターネットにアクセスできる、
- 電話の会話を聞くことができ、
- トロント広域圏 (GTA) 内に居住している場合、または GTA の外にいる場合は、研究完了時に郵送でデバイスを返却する意思がある場合。
除外基準:
除外基準には、次のような人が含まれます。
- CABG以外の心臓手術を受けている、
- 老人ホームまたは長期介護施設に居住している、
- 自己反省やコミュニケーション能力を妨げる可能性のある神経障害または精神障害がある。
- 重大な重大な術後合併症(脳卒中、深部創傷感染、心嚢液貯留など)が院内で持続する、5)歩行不能(すなわち、 補助なしで時速2マイルで歩きます)。

介入なし:普段のお手入れ
通常のケアを受けている患者は、通常、急性期ケアセンターからの退院時に、紙または電子的な体系的な紹介状(医師または看護師が記入)を通じて CR に紹介されます。
さらに、医療提供者による CR に関する患者との会話が行われる場合もありますが、このコミュニケーションは常に一貫して行われるわけではありません。
紹介されると、患者はその地域の CR プログラムからの連絡を待ちます。実際のプログラム登録が行われる場合、通常は退院後 8 ~ 10 週間の間に行われます。

実験的:MyCare Android アプリケーション
MyCaRe モバイル アプリケーション (痛み、気分スケール、傷のモニタリングを含む教育および症状モニタリング) と Fitbit 加速度計 (歩数、距離) は、心臓手術後の最初の 6 ~ 8 週間の回復期間に介入グループの割り当てを受ける患者に提供されます。
患者には退院前に、MyCaRe と Fitbit Inspire 2 が搭載された一時的に貸し出されるモバイル デバイスが提供されます。
最初の 2 週間は可能であれば毎日、その後は 6 ~ 8 週間または心臓リハビリテーション プログラムに参加するまで週に 1 回、データ (痛み、気分、傷、活動、血糖、血圧) を入力するよう求められます。

登録はカナダ心臓血管学会のデータ定義 (http://bridge.ccs.ca/index.php/en/) に従って定義されます。
予定された心臓リハビリテーションの予約(つまり、危険因子の評価、目標設定)に出席した患者として。
登録は患者および患者が登録されている CR プログラムによって確認されます。
