상세 설명
관상동맥우회술 후 초기 전환기는 환자가 지속 가능한 장기적 생활 방식 변화를 요구하는 강렬한 생리학적 및 심리사회적 적응과 관련이 있습니다. 심장재활(CR)은 관상동맥질환 환자의 전반적인 체력과 삶의 질을 개선하고 이환율과 사망률을 낮추는 것으로 나타난 증거 기반 위험 감소 프로그램입니다. 따라서 심장 수술 환자에 대한 조기 개입을 통해 급성 치료에서 CR로의 격차를 해소하기 위한 지원 전략의 개발이 필수적입니다. MyCardiacRecovery(MyCaRe)는 퇴원 후 처음 6~8주 동안 표준화된 교육 형식과 대화형 추적(사진 기능과 Fitbit Inspire 2 가속도계를 사용한 상처 치유 및 활동 진행) 지원을 포함하는 대화형 플랫폼입니다. 이 응용 프로그램은 다음을 제공하여 환자와 가족이 치료의 연속성을 탐색하는 데 도움이 됩니다. b) 현저한 회복 문제를 다루기 위해 고안된 환자 치료 및 교육 자료; c) 환자와 의료 제공자 사이의 개선된 의사소통 및 d) CR에 대한 간소화된 체계적 의뢰를 보장합니다. 이 혁신적인 전략은 환자 만족도에 긍정적인 영향을 미치고 환자 결과를 개선하며 의료 시스템에 가해지는 재정적 제약을 최소화할 수 있는 잠재력을 가지고 있습니다. 프로토타입 1.0의 1단계(예비 사용성 테스트)가 완료되었습니다. 2단계는 파일럿 무작위 통제 시험 설계를 사용하여 심장 수술 후 환자 샘플에서 MyCaRe 2.0을 테스트하는 데 중점을 둘 것입니다.

- > 35세,
- 전통적인 (sternotomy 접근 방식) 관상 동맥 우회술 (CABG) 수술을 받고,
- 복잡하지 않은 수술 후 과정,
- 표준 입원 기간(4~8일),
- 집에서 와이파이 인터넷 접속,
- 전화 통화를 들을 수 있고,
- 광역 토론토 지역(GTA) 내에 거주하거나 GTA 외부에 거주하는 경우 연구 완료 시 우편을 통해 기기를 기꺼이 반환합니다.
제외 기준:
제외 기준에는 다음과 같은 사람이 포함됩니다.
- CABG 이외의 심장 수술 절차가 있는 경우,
- 요양원 또는 장기 요양 시설에 거주
- 자기 반성 또는 의사 소통 능력을 방해할 수 있는 신경학적 또는 정신 장애가 있는 경우,
- 주요 의미의 지속적인 병원 내 수술 후 합병증(예: 뇌졸중, 심부 감염, 심낭 삼출액 등), 5) 보행 불능(즉, 도움 없이 2mph로 걷기).

간섭 없음: 평상시 관리
일반적인 치료를 받는 환자는 일반적으로 급성 치료 센터에서 퇴원할 때 서면 또는 전자 체계적 진료 의뢰(의사 또는 임상간호사가 완료)를 통해 CR에 진료를 의뢰합니다.
또한 의료 제공자가 CR에 대해 환자와 대화할 수 있지만 이 대화가 항상 일관된 것은 아닙니다.
추천을 받으면 환자는 해당 지역의 CR 프로그램에서 연락을 기다리고 실제 프로그램 등록이 발생하는 경우 일반적으로 퇴원 후 8~10주 사이에 발생합니다.

실험적: MyCaRe 안드로이드 애플리케이션
MyCaRe 모바일 애플리케이션(통증, 기분 척도, 상처 모니터링을 포함하는 교육 및 증상 모니터링) 및 Fitbit 가속도계(걸린 걸음 수, 거리)는 심장 수술 후 초기 6~8주 회복 동안 중재 그룹 할당을 받는 환자에게 제공됩니다.
환자들은 병원을 떠나기 전에 MyCaRe 및 Fitbit Inspire 2가 탑재된 임시 모바일 기기를 대여받을 것입니다.
처음 2주 동안은 가능하면 매일 데이터(통증, 기분, 상처, 활동, 혈당, 혈압)를 입력하고 그 이후에는 6주에서 8주까지 또는 심장 재활 프로그램에 들어갈 때까지 매주 한 번 데이터를 입력해야 합니다.
