My Cardiac Recovery (MyCaRe): een proef-RCT om het effect van MyCaRe Android-applicatie op herstelresultaten en deelname aan hartrevalidatie na hartchirurgie te onderzoeken.

De vroege overgangsperiode na bypassoperatie van de kransslagader gaat gepaard met intense fysiologische en psychosociale aanpassing die van patiënten vereist dat ze duurzame levensstijlveranderingen op de lange termijn maken. Hartrevalidatie (CR) is een evidence-based risicoreductieprogramma waarvan is aangetoond dat het de algehele fitheid en kwaliteit van leven verbetert en de morbiditeit en mortaliteit verlaagt bij patiënten met coronaire hartziekte. Daarom is de ontwikkeling van ondersteunende strategieën om de kloof van acute zorg naar CR te overbruggen door middel van vroege interventie voor hartchirurgische patiënten essentieel. MyCardiacRecovery (MyCaRe) is een interactief platform met een gestandaardiseerd educatief formaat en interactieve tracking (wondgenezing en voortgang van activiteiten met behulp van fotomogelijkheden en Fitbit Inspire 2-versnellingsmeter) gedurende de eerste 6 tot 8 weken na ontslag uit het ziekenhuis. Deze applicatie zal patiënten en familie helpen hun weg te vinden door het continuüm van zorg door een: a) geïntegreerde koppeling te bieden tussen acute zorg en poliklinische hartrevalidatie voor efficiënte coördinatie van informatie en vermindering van duplicatie van diensten; b) patiëntenzorg en voorlichtingsmateriaal dat is ontworpen om in het oog springende herstelvragen aan te pakken; c) verbeterde communicatie tussen de patiënt en zorgverleners en, d) zorgen voor een gestroomlijnde systematische doorverwijzing naar CR. Deze innovatieve strategie heeft het potentieel om de patiënttevredenheid positief te beïnvloeden, de patiëntresultaten te verbeteren en mogelijk de financiële beperkingen voor het gezondheidszorgsysteem te minimaliseren. Fase 1 (voorlopige usability testing) van prototype 1.0 is afgerond. Fase 2 zal gericht zijn op het testen van MyCaRe 2.0 in een steekproef van patiënten na hartchirurgie met behulp van een gerandomiseerde, gecontroleerde proefopzet.

2) Om de impact van de MyCaRe-applicatie op het herstel en de tevredenheid van de patiënt te onderzoeken (d.w.z. kennisniveau, percepties van hersteluitkomsten zoals progressie van activiteiten, kwaliteit van leven, angst en depressie) 3) Het effect onderzoeken van MyCaRe-toepassing op het gebruik van gezondheidsdiensten. 4) Om de tevredenheid van patiënten en clinici te onderzoeken, met name gebruikersgemak, bruikbaarheid en comfort met de MyCaRe-applicatie.

Inclusiecriteria:
De opnamecriteria voor de steekproef omvatten:
- instructies voor studie en gebruik van app/Fitbit begrijpen/begrijpen,
- > 35 jaar,
- traditionele (sternotomiebenadering) coronaire bypassoperatie (CABG) ondergaan,
- een ongecompliceerd postoperatief beloop,
- standaard opnameduur (vier tot acht dagen),
- toegang tot wifi internet in hun huis,
- telefoongesprekken kunnen horen,
- woonachtig bent in de grotere regio Toronto (GTA) of buiten GTA bereid bent apparaten na voltooiing van de studie per post terug te sturen.
Uitsluitingscriteria:
De uitsluitingscriteria omvatten die personen die:
- andere hartchirurgische ingrepen hebben dan CABG,
- verblijf in een verpleeghuis of instelling voor langdurige zorg,
- een neurologische of psychiatrische stoornis heeft die hun vermogen tot zelfreflectie of communicatie kan belemmeren,
- aanhoudende postoperatieve complicaties in het ziekenhuis van grote betekenis (zoals beroerte, diepe wondinfecties, pericardiale effusie, enz.), 5) onvermogen om te lopen (d.w.z. loop zonder hulp met een snelheid van 2 mph).

Geen tussenkomst: Gebruikelijke zorg
Patiënten die de gebruikelijke zorg krijgen, worden op het moment van ontslag uit het acute zorgcentrum doorgaans doorverwezen naar CR via papieren of elektronische systematische verwijzing (ingevuld door een arts of verpleegkundig specialist).
Bovendien kan een gesprek met de patiënt over CR door een zorgverlener plaatsvinden, maar deze communicatie is niet altijd consistent.
Eenmaal doorverwezen, wachten patiënten op contact van een CR-programma in hun regio/gebied en als daadwerkelijke deelname aan het programma plaatsvindt, gebeurt dit meestal tussen 8 en 10 weken na ontslag.

Experimenteel: MyCaRe Android-applicatie
De MyCaRe mobiele applicatie (voorlichting en symptoommonitoring, waaronder pijn, stemmingsschalen, wondmonitoring) en Fitbit-versnellingsmeter (gelopen stappen, afstand) zullen worden verstrekt aan patiënten die interventiegroeptoewijzing krijgen voor de eerste 6 tot 8 weken herstel na een hartoperatie.
De patiënten krijgen een tijdelijk mobiel leenapparaat waarop MyCaRe en Fitbit Inspire 2 zijn geladen voordat ze het ziekenhuis verlaten.
Ze zullen worden gevraagd om dagelijks gegevens in te voeren (pijn, stemming, wonden, activiteit, bloedsuikerspiegel, bloeddruk) indien mogelijk in de eerste 2 weken en daarna eenmaal per week tot 6 tot 8 weken of tot deelname aan een hartrevalidatieprogramma.

Het aantal personen dat aanwezig is bij de initiële intake-afspraak voor hartrevalidatie zal de primaire uitkomstmaat van inschrijving zijn.
Inschrijving wordt gedefinieerd in overeenstemming met de gegevensdefinities van de Canadian Cardiovascular Society (http://bridge.ccs.ca/index.php/en/),
als patiënt die zijn geplande intake-afspraak voor hartrevalidatie heeft bijgewoond (d.w.z. risicofactorbeoordeling, doelen stellen).
Inschrijving wordt geverifieerd bij de patiënt en bij het CR-programma waarvoor ze zijn ingeschreven.
