喘息および慢性閉塞性肺疾患 (COPD) は、米国で 3,000 万人を超える成人が診断される一般的な慢性肺疾患です。 しかし、診断エラー (DE) は、医学研究所によって最も一般的で有害な患者安全問題の 1 つと見なされており、喘息および/または COPD で頻繁に発生し、マイノリティや十分なサービスを受けていない人々に不均衡に影響を与えます。 DE は、他の慢性疾患、回避可能な罹患率と死亡率、患者と医療システムへの不必要なコスト、およびケアの質の低下を特定する機会の損失につながります。 息切れや呼吸困難は、喘息や COPD でよくみられる症状ですが、心血管疾患や肥満など、他の多くの慢性疾患でもよくみられます。 喘息と COPD の診断精度を向上させるための DE と介入の影響をよりよく理解することは、呼吸困難につながる状態によって不均衡に影響を受けているマイノリティや十分なサービスを受けていない人々にとって特に重要です。
スパイロメトリーは、喘息および COPD の診断のためのすべての主要な国内および国際ガイドラインで推奨されている、簡単でモバイルで必須の検査です。 しかし、スパイロメトリーは外来のプライマリケア環境では日常的に使用されておらず、マイノリティや十分なサービスを受けていない集団はスパイロメトリーを受ける可能性が低く、DE の有病率が高くなることはよく知られています。 喘息および COPD と診断された人の 30 ~ 50% が誤診されていると推定されています。 喘息および/またはCOPDと誤診されたこれらの患者の多くは、肺炎、心血管イベント、および死亡を含む深刻なリスクをもたらす可能性のある不要な呼吸器薬物療法を受けています。 DEの設定では、これらは回避可能で不必要な呼吸器薬物療法の使用を少数派と見なし、すでに心血管疾患の影響を過度に受けている十分なサービスを受けていない人々は、より悪い結果のリスクを高めます。 喘息と COPD の診断にも DE があります。これらはどちらも臨床的に異なる呼吸器疾患であり、治療の推奨事項に微妙な違いがあるからです。 アフリカ系アメリカ人は COPD による死亡率が増加していると考えられており、喘息による死亡率の影響を不釣り合いに受けていると報告されています。 しかし、肺活量測定は定期的に実施されておらず、DE は喘息と COPD で蔓延しているため、これらの悪い転帰の構成要素は、他の慢性疾患の診断の見逃しまたは遅延、または喘息と COPD 内の誤診に起因する可能性があります。

- 2.過去1年間の呼吸器維持薬および次のいずれかの使用:
- 3.喘息および/またはCOPDの診断
- 4.過去3年間にスパイロメトリー検査を実施していない
- 5.過去または現在の喫煙者、またはタバコにさらされている
除外基準:
- 適切なスパイロメトリーを実施できない
- 非英語圏
- 妊娠
- 年内にシカゴエリアから移転予定
- 過去3年間に呼吸器専門医の診察を受けた
- 平均余命が6か月未満の末期疾患
- 生命を脅かす (例: 集中治療室への入院および/または人工呼吸器の使用) 過去 1 年間の呼吸不全イベント。

対象者には、クリニックに行き、その後はPCPによる管理を受けるようアドバイスされます。 1年間の追跡調査の終わりに、患者はBD前後のスパイロメトリーを予定され、介入グループと同じスパイロメトリープロトコルを受けることになる。 |
