公衆衛生と腫瘍学の看護における懸念が高まっているのは、結腸直腸がん(CRC)生存者ががんの診断と治療後に長期的に身体的および精神的機能状態の低下を経験する可能性があることです。
治療を受ける前に、処理速度、空間作業記憶、および言語記憶における認知障害が、健康な対照(HC)の15%と比較してCRC患者の45%で認められました。
補助化学療法後、化学療法を受けていないCRC患者(Ctx-グループ)およびHC参加者と比較して、化学療法を受けたCRC患者(Ctx+グループ)では認知機能がより損なわれています。
これらの研究は、(1) CRC 患者は認知障害のリスクが高いこと、(2) Ctx+ 患者は治療中に認知機能が低下する可能性が高いことを示しています。
これらのがんや化学療法に関連した認知機能の変化は、生理学的症状や併発症状、身体的および精神的機能状態など、いくつかの生活の質の要因と関連しています。
対照的に、CRC 患者における認知障害の神経メカニズムは、実行機能ネットワーク (EFN) の変化に関連しています。
EFN は、前頭葉皮質領域と頭頂葉皮質領域の間の長距離コミュニケーションを促進し、注意制御プロセスに関連しています。
CRC 患者を研究する経験的目標は 2 つあります。(1) 認知機能、脳波 (EEG) からの事象関連電位 (ERP) 測定、および安静状態機能的磁気共鳴画像法 (rsfMRI) 測定のコアセットを開発して、CRC 患者の状態を解明する。注意制御障害とEFN機能障害との関係、(2) 神経認知障害と同時症状の重症度と機能への影響との関連性についての理解を深めます。
研究者らは、術後補助化学療法を開始する予定の術後CRC患者(Ctx+グループ)を研究するための縦断的前向きコホートパイロットデザインを提案している。
比較グループには、化学療法を受けていない術後CRC患者(Ctx-グループ)と、Ctx+参加者と人口統計的に一致する健康な対照(HCグループ)が含まれます。
すべての参加者(N=60; グループあたり 20 人)は、ベースライン時に追加の 1 時間の研究来院を完了し、rsfMRI 測定値を収集するために 24 週間経過します。Ctx+ 患者は手術後、化学療法を開始する前にベースライン評価を完了します。CTx- 患者はベースライン評価を完了します。手術後 4 ~ 6 週間、HC が一致して同意した後。

予後不良には、完全な結腸閉塞、穿孔、検査されたリンパ節が 12 個未満の T4 腫瘍、術前または術後の CEA レベルの上昇、断端陽性、および神経周囲、リンパ管または静脈浸潤が含まれます。 分子マーカーも結腸癌の進行性と相関します。
