대장암 환자의 신경인지 기능 및 증상의 화학요법 관련 변화: 파일럿 연구

암 치료는 성공적으로 암 결과를 개선했지만 암 생존자의 삶의 질에 부정적인 영향을 미치는 경우가 많습니다.
특히 화학요법(CTX)은 집중력, 기억력 등 인지 능력 장애와 관련이 있습니다.
목표는 학제간 번역 접근법을 사용하여 화학요법 관련 인지 장애(CRCI)의 신경 메커니즘을 조사하는 것입니다.
이 분야의 이전 연구에서는 연구된 암 집단과 주로 유방암에 초점을 맞춘 치료법의 다양성이 부족하고 CRCI가 신경 구조, 기능 및 연결성의 변화에서 어떻게 나타나는지에 대한 제한된 이해를 제공합니다.
이러한 한계를 극복하기 위해 이 타당성/파일럿 연구는 대장암(CRC) 환자의 CRCI를 조사하는 것을 목표로 합니다.

공중 보건 및 종양학 간호에 대한 우려가 커지고 있는 것은 대장암(CRC) 생존자가 암 진단 및 치료 후 장기간 신체적, 정신적 기능 상태가 저하될 가능성이 있다는 점입니다.
치료를 받기 전에 처리 속도, 공간 작업 기억 및 언어 기억의 인지 장애는 건강한 대조군(HC)의 15%에 비해 CRC 환자의 45%에서 나타났습니다.
보조 화학요법 후에는 화학요법을 받지 않은 CRC 환자(Ctx- 그룹) 및 HC 참가자에 비해 화학요법을 받은 CRC 환자(Ctx+ 그룹)에서 인지 기능이 더 손상됩니다.
이 연구는 다음을 보여줍니다: (1) CRC 환자는 인지 장애의 위험이 높고 (2) Ctx+ 환자는 치료 중 인지 기능이 저하될 가능성이 더 높습니다.
이러한 암 및 화학요법과 관련된 인지 기능의 변화는 생리적 및 동반 증상, 신체적, 정신적 기능 상태를 비롯한 여러 삶의 질 요인과 관련되어 있습니다.
대조적으로, CRC 환자의 인지 장애의 신경 메커니즘은 집행 기능 네트워크(EFN)의 변화와 관련이 있습니다.
EFN은 전두엽 피질 영역과 두정엽 피질 영역 사이의 장거리 통신을 촉진하며 주의력 제어 프로세스와 관련이 있습니다.
CRC 환자를 연구하는 경험적 목표는 두 가지입니다. (1) 인지 기능, 뇌전도(EEG)의 사건 관련 전위(ERP) 측정, 휴식 상태 기능적 자기 공명 영상(rsfMRI) 측정의 핵심 세트를 개발하여 주의력 조절 장애와 EFN 기능 장애 사이의 관계 및 (2) 신경인지 장애와 동시 증상 심각도 및 기능에 미치는 영향 사이의 연관성에 대한 이해를 높입니다.
연구자들은 보조 화학요법을 시작할 예정인 수술 후 CRC 환자(Ctx+ 그룹)를 연구하기 위한 종단적, 전향적 코호트 파일럿 설계를 제안합니다.
비교 그룹에는 화학요법을 받지 않는 수술 후 CRC 환자(Ctx- 그룹) 및 인구통계학적으로 Ctx+ 참가자(HC 그룹)와 일치하는 건강한 대조군이 포함됩니다.
모든 참가자(N=60, 그룹당 20명)는 기준선에서 추가 1시간 연구 방문을 완료하고 rsfMRI 측정값을 수집하기 위해 24주를 완료합니다. Ctx+ 환자는 수술 후 기준선 평가를 완료하지만 화학요법을 시작하기 전에 CTx- 환자는 기준선 평가를 완료합니다. 수술 후 4-6주 후 HC와 일치 및 동의를 받았습니다.

CRC 진단:
더 나쁜 예후에는 완전한 결장 폐쇄, 천공, 검사된 12개 미만의 림프절 T4 종양, 수술 전후 CEA 수준 상승, 양성 마진, 신경 주위, 림프 또는 정맥 침범이 포함됩니다. 분자 표지는 또한 대장암의 공격성과도 상관관계가 있습니다.

- 보조 CTX를 투여받는 II기/IV기 환자(Ctx+ 그룹)
- CTX를 투여받지 않는 I/III기 환자(Ctx-군)
- 정상 또는 정상 시력으로 교정됨(교정 원거리 시력 20/50 이상)
인구학적으로 일치하는 건강한 대조군(HC 그룹)
- 연령(±5세, 성별, 인종, 폐경기 상태 및 교육(±2세))에 따라 CTX를 투여받는 환자와 일치
- 정상 또는 정상으로 교정된 시력(교정 원거리 시력 20/50 이상)
