閉塞性睡眠時無呼吸症候群 (OSA) は子供に多く見られ、多くの場合、子供のアデノイドおよび/または扁桃腺の過成長によって引き起こされます。
その結果、アデノイド扁桃摘出術 (扁桃腺とアデノイドの除去) は小児の OSA の最も一般的な治療法ですが、場合によっては扁桃腺またはアデノイドだけを除去することもあります。
同様に、子供の OSA はしばしば認知機能障害や気分の問題と関連しており、状態による脳の変化を示唆しています。
しかし、子供の脳の変化、認知および気分の問題、OSA との関連性は完全には解明されていません。
したがって、この研究は、アデノイドおよび/または扁桃腺の除去前後と同様に、対照と比較した小児 OSA 被験者の脳の変化、認知および気分を調べることを目的としています。
この研究では、年齢 7 ~ 12 歳の 70 人の被験者、35 人の健康な対照者、および OSA と診断され、アデノイド切除術および/または扁桃摘出術が予定されている 35 人の被験者を登録したいと考えています。
対照被験者は UCLA への 1 回の訪問をスケジュールし、OSA 被験者は 2 回の訪問をスケジュールします。
最初の訪問時に、すべての被験者は認知、気分、睡眠に関するアンケートとMRIスキャンを受けます。
それは、研究へのコントロールの参加期間になります。ただし、OSA 患者は 6 か月後 (アデノイド切除術および/または扁桃摘出術の後) に戻り、同じ手順を繰り返します。

小児の閉塞性睡眠時無呼吸症候群 (OSA) は、一般的で進行性の症候群であり、深刻な認知、気分、日中の行動の問題、および学業成績の低下を伴います。これはおそらく、断続的な低酸素症と灌流の変化によって引き起こされる神経組織の損傷に起因すると考えられます。
ただし、脳組織損傷が急性または慢性の状態にあるかどうか、およびミエリンが軸索よりも優先的に影響を受けるかどうかは不明です。これは、神経修復/回復のための介入が急性対慢性およびミエリン対軸索損傷で異なるため、理解するための重要なステップです。
また、小児 OSA に付随する脳の変化が機能的な影響を及ぼし、認知障害または気分障害をもたらすかどうかも不明です。
さらに、断続的な低酸素症は、内皮細胞に影響を与える有害なプロセスのカスケードを引き起こしますが、小児 OSA で局所脳血流 (CBF) が減少するかどうかは不明です。
小児 OSA の治療法には、扁桃摘出術および/またはアデノイド切除術が含まれますが、脳組織の変化、局所 CBF、および認知課題に対する神経反応が治療後に改善するかどうかは不明です。
拡散テンソル イメージング (DTI) および拡散尖度イメージング (DKI) ベースの手順を使用して、急性および慢性の組織変化、軸索の状態、ミエリンの完全性を評価できます。
局所脳 CBF は、検証済みの動脈スピン標識 (ASL) イメージングによって評価でき、認知課題に対する局所神経活動は、血液酸素レベル依存 (BOLD) 機能的磁気共鳴画像 (MRI) で調べることができます。
