소아 OSA의 뇌 변화

소아 폐쇄성 수면 무호흡증의 뇌 변화
폐쇄성 수면 무호흡증(OSA)은 어린이에게 매우 흔하며 종종 어린이의 아데노이드 및/또는 편도선의 과성장으로 인해 발생합니다.
결과적으로 편도선 절제술(편도선 및 아데노이드 제거)은 소아 OSA의 가장 일반적인 치료법이지만 경우에 따라 편도선 또는 아데노이드만 제거할 수도 있습니다.
또한 어린이의 OSA는 종종 인지 기능 장애 및 기분 문제와 관련이 있으며, 이는 상태로 인한 뇌 변화를 시사합니다.
그러나 어린이의 뇌 변화, 인지 및 기분 문제, OSA 사이의 연관성은 철저히 조사되지 않았습니다.
따라서 본 연구는 아데노이드 및/또는 편도선 제거 전과 후뿐만 아니라 대조군과 비교하여 소아 OSA 대상자의 뇌 변화, 인지 및 기분을 조사하는 것을 목표로 합니다.
이 연구는 7-12세의 70명의 피험자, 35명의 건강한 대조군, OSA로 진단되고 아데노이드 절제술 및/또는 편도선 절제술이 예정된 35명의 피험자를 등록하기를 희망합니다.
대조군 피험자는 UCLA를 한 번 방문하도록 계획하고 OSA 피험자는 두 번 방문하도록 계획합니다.
첫 번째 방문 시 모든 피험자는 인지, 기분 및 수면 설문지와 MRI 스캔을 받게 됩니다.
그것은 통제군이 연구에 참여하는 기간입니다. 그러나 OSA 피험자는 6개월 후(아데노이드 절제술 및/또는 편도선 절제술 후) 다시 돌아와 동일한 절차를 반복합니다.

상세 설명
소아 폐쇄성 수면 무호흡증(OSA)은 간헐적인 저산소증 및 관류 변화에 의해 유발되는 손상된 신경 조직으로 인해 심각한 인지, 기분 및 주간 행동 문제뿐만 아니라 불량한 학교 성적을 동반하는 흔하고 진행성인 증후군입니다.
그러나 뇌 조직 손상이 급성 또는 만성 상태인지, 그리고 미엘린이 축삭보다 우선적으로 영향을 받는지 여부는 불분명합니다. 이는 신경 복구/회복을 위한 개입이 급성 대 만성 및 미엘린 대 축삭 손상에 따라 다르기 때문에 이해해야 할 필수 단계입니다.
또한 소아 OSA에서 수반되는 뇌 변화가 기능적 결과를 가져 인지 또는 기분 장애를 초래하는지 여부도 불분명합니다.
또한 간헐적 저산소증은 내피 세포에 영향을 미치는 일련의 유해한 과정을 유발하지만 소아 OSA에서 국소 대뇌 혈류(CBF)가 감소하는지 여부는 불분명합니다.
소아 OSA의 치료 방법에는 편도선 절제술 및/또는 아데노이드 절제술이 포함되며 뇌 조직 변화, 국소 CBF 및 인지 문제에 대한 신경 반응이 치료 후 개선되는지 여부는 불분명합니다.
확산 텐서 이미징(DTI) 및 확산 첨도 이미징(DKI) 기반 절차를 사용하여 급성 및 만성 조직 변화와 축삭 상태 및 미엘린 완전성을 평가할 수 있습니다.
지역 뇌 CBF는 검증된 ASL(동맥 회전 라벨링) 영상으로 평가할 수 있으며, 인지 문제에 대한 지역 신경 활동은 혈중 산소 수준 의존(BOLD) 기능적 자기 공명 영상(MRI)으로 검사할 수 있습니다.
따라서 치료 경험이 없는 소아 OSA 35명과 대조군 어린이 35명을 사용하여 구체적인 목표는 다음과 같습니다. 건강한 대조군에 비해 치료되지 않은 소아 OSA에서 DTI 및 DKI 측정을 사용하여 뇌 조직 손상의 특성 및 유형을 결정합니다. 건강한 어린이에 대한 소아 OSA에서 BOLD 기능적 MRI를 사용하여 ASL 이미징을 사용하여 지역 뇌 CBF 및인지 문제에 대한 신경 반응을 식별합니다. 통제 어린이와 비교하여 소아 OSA에서 인지(차등 능력 척도 II 및 NEPSY II에 의해) 및 감정 기능(아동 행동 체크리스트에 의해)을 평가하고, 소아 OSA에서 뇌 손상과 인지 및 감정 기능 장애 사이의 관계를 조사합니다. 소아 OSA에서 6개월에 아데노편도 절제술 후 뇌 조직 변화, CBF 감소, 인지 도전 반전에 대한 변경된 신경 반응, 인지 및 기분 징후 개선 여부를 조사합니다.

OSA
- 소아 OSA 피험자는 7-12세 범위에 속합니다(발달 관련 뇌 변화 및 뇌 MRI에 대한 잠재적인 마취 요구 사항을 피하기 위해 연령 상한 및 하한이 선택됨).
- 수면 검사실에서 야간 수면다원검사를 통해 최소 중등도 OSA(AHI>5 이벤트/시간) 진단을 받아야 합니다.
- 수술 전후 문제를 피하기 위해 비만이 없는 경우(연령 및 성별에 대한 ≥95번째 백분위수 BMI)
