実験的:実践ベースの介入
介入には、医師の教育、電子的意思決定支援、電子紹介システムの実装、事前診療提供者による患者教育/共同管理が含まれます。
|
介入医師は、メモ テンプレート、オーダー セット、割り込みアラート、および UI 陽性の患者の通知で構成される電子的な臨床意思決定支援にアクセスできます。
PCP の追加のケアの負担を軽減するために、研究者は高度な実践プロバイダーを患者教育とケアの自己管理部分に組み込みます。 APP は介入医師と一緒にトレーニングを受けますが、患者教育、UI 知識、ケーゲル体操の指導、意思決定の共有、および自己管理に関する追加の標準化されたトレーニングも受けます。 その後、患者は、最初の訪問から 1 か月以内に、遠隔医療 (ビデオ訪問または電話訪問) による APP を使用した UI 教育および自己管理セッションをスケジュールされます。 年 1 回の内診が必要な患者で、患者の主治医が APP による内診を希望する場合は、APP で別の診察が予定されます。 その後、非外科的治療の結果を評価し、専門家の紹介が必要かどうかを判断するために、最初の APP 訪問から 3 か月以内にフォローアップのテレビ訪問が予定されます。
民間部門に電子相談システムを導入するにあたり、調査員は、ロサンゼルス郡専門プライマリケア作業グループによって開発された期待される実践をモデル化します。
この eConsult システムは「キックバック」メカニズムを利用しており、紹介をレビューする専門家は、特定の基準基準を満たしていない場合にそれを返すことができます (例えば、OAB/尿意切迫感のある女性の場合: UA 陰性、予定された排尿、水分の滴定を文書化する)。のどの渇き、ケーゲル、抗ムスカリン薬、利尿コントロールの最適化、利尿薬の調整など)。

このグループのオフィスは、4 つの方面からの介入を受けません。
ただし、研究試験の後、最初の対照群が介入を受ける「検証段階」があります。
このクロスオーバー設計により、60 のオフィスすべてが介入を受けることができます。
クロスオーバー介入グループからのデータは、別のコホートとして分析され、患者ケアの改善を確認し、研究試験の下で介入グループで測定された結果を再現することができます。

|
介入期間中の専門医 (泌尿器科医または婦人科医) への紹介数を定量化し、介入群と対照群の間で比較します。 特定の目的 1 から、かかりつけ医の診療所で UI の新規または悪化が陽性であるとスクリーニングされた患者について、治験責任医師は、患者が治療のために専門医に紹介されたかどうか、またいつ紹介されたかを記録します。 提供されるケアの各レベルの日付は、タイミングを計算できるように、各チャートの抽象化に組み込まれます。 調査員は、ベースラインチャートのレビュー中に、各オフィスの専門紹介のベースライン率も記録します。 |
