- 18세 미만
- 요실금 검사 도구에 "아니오"라고 대답하고/하거나 참여에 동의하지 않음
- 1차 진료 제공자(양성 선별 검사를 한 환자 목록을 검토함)는 환자가 임신, 심각한 기억 장애 또는 참여를 방해하는 정신과적 병력으로 인해 부적격하다고 간주합니다.

실험적: 실습 기반 개입
개입에는 의사 교육, 전자 의사 결정 지원, 전자 의뢰 시스템의 구현, 고급 진료 제공자에 의한 환자 교육/공동 관리가 포함됩니다.

개입 의사는 메모 템플릿, 주문 세트, 중단 경고 및 UI 양성 선별 환자 알림으로 구성된 전자 임상 의사 결정 지원에 액세스할 수 있습니다.
PCP의 추가 진료 부담을 줄이기 위해 조사관은 환자 교육 및 진료의 자기 관리 부분에 Advanced Practices Providers를 통합할 것입니다. APP는 개입 의사와 함께 교육을 받을 것이지만 환자 교육, UI 지식, 케겔 운동에 대한 지침 제공, 공동 의사 결정 및 자기 관리에 대한 표준화된 추가 교육도 받게 됩니다. 그런 다음 환자는 최초 방문 후 1개월 이내에 원격 진료(화상 방문 또는 전화 방문)를 통해 UI 교육 및 APP를 통한 자가 관리 세션 일정을 잡게 됩니다. 연례 골반 검사가 필요한 환자의 경우 환자의 PCP가 APP에서 골반 검사를 수행하는 것을 선호하는 경우 APP를 통해 별도의 방문 일정이 잡힙니다. 비수술적 치료의 결과를 평가하고 전문의 의뢰가 필요한지 결정하기 위해 최초 APP 방문 후 3개월 이내에 후속 원격 방문이 예정됩니다.
민간 부문에서 전자 상담 시스템을 구현함에 있어 조사관은 로스앤젤레스 카운티 전문 일차 진료 작업 그룹이 개발한 예상 사례를 모델로 삼을 것입니다.
이 eConsult 시스템은 추천을 검토하는 전문가가 특정 기본 기준(예: OAB/요절박이 있는 여성의 경우: 음성 UA 기록, 예정된 배뇨, 적정 체액 갈증, 케겔, 항무스카린제, 이뇨제 조절 최적화, 이뇨제 조정).

그러나 연구 시험 후 초기 통제 그룹이 개입을 받는 "검증 단계"가 있을 것입니다.
이 교차 설계를 통해 60개 사무실 모두가 개입을 받을 수 있습니다.
교차 개입 그룹의 데이터는 연구 시험에서 개입 그룹에서 측정된 환자 치료 및 결과의 개선을 확인하기 위해 별도의 코호트로 분석될 것입니다.

개입 기간 동안 전문의(비뇨기과 의사 또는 산부인과 의사)에게 추천된 횟수는 정량화되고 개입 그룹과 통제 그룹 간에 비교됩니다. 특정 목표 1에서 주치의 사무실에서 새로운 UI 또는 악화 UI에 대해 양성으로 선별된 환자의 경우 조사관은 환자가 치료를 위해 전문의에게 의뢰되었는지 여부와 시기를 기록합니다. 제공되는 각 수준의 치료 날짜는 타이밍을 계산할 수 있도록 각 차트 추상화에 통합됩니다. 조사관은 또한 기본 차트 검토 중에 각 사무실에 대한 전문 추천의 기준 비율을 기록합니다. |
