426 人の参加者が、セルフガイド型とサポート型の dCBT (SilverCloud) を比較する 2 つのアームによる試験に登録されます。
医療提供者レベルでは、研究者は医療提供者の紹介の有効性を診療所の患者登録簿の使用と比較して、デジタルメンタルヘルス介入の恩恵を受ける可能性のある候補者を特定します。

研究者は、うつ病や不安神経症のラテン系アメリカ人患者 (LEP) を対象としたセーフティネット プライマリ ケア クリニックでの、エビデンスに基づくスペイン語のデジタル認知行動療法 (dCBT) 介入 (SilverCloud) の実施を評価します。
プロバイダーレベルと患者レベルの無作為化の両方を備えた有効性と実装のハイブリッド試験(タイプ2)デザインが実施されます。
医療提供者レベルでは、研究者はアウトリーチ (診療所の患者登録を使用) とインリーチ (従来の医療提供者紹介) を比較し、患者レベルでは、dCBT プラットフォームの 2 つの提供モード (サポートされているものとサポートされていないもの) を比較します。

- PHQ-9 ≥ 10 または GAD-7 ≥ 8
- 自宅でスマートフォンおよび/またはブロードバンドを介してインターネットにアクセスでき、基本レベルの技術リテラシーまたは簡単な技術使用トレーニングを受ける意欲がある
- 18歳以上
- スペイン語で医療を受けることの希望
- 同時精神療法を受けていない
- 現在抗うつ薬を服用している場合、患者は少なくとも 6 週間は安定した用量を服用していなければならず、用量を変更する予定はありません。 一般化可能性が高まるため、抗うつ薬を服用している患者が研究に参加できるようにします。 抗うつ薬の状態は、必要に応じて、これらの効果を制御するために各評価で監視されます。
除外基準:
- 現在心理療法を受けていますが、この治療はうつ病と不安神経症の最前線の治療として提供されるため、患者はこの研究に参加している間治療を受けることが許可されており、治療の開始は監視され、分析で考慮されます
- -研究手順の完了を妨げる視覚、聴覚、音声、または運動障害または非識字
- 精神病性障害、双極性障害、解離性障害、物質使用障害の診断

実験的:処理
この条件にランダムに割り当てられた参加者には、SilverCloud プラットフォームのすべての機能が提供されるだけでなく、定期的なサポートを提供するピア サポーターが割り当てられます。

サポートされている dCBT 条件では、各患者は SilverCloud プラットフォームに完全にアクセスできます。
さらに、確立されたコーチングサポートプロトコルに基づいて定期的にサポートを提供するサポーターが患者に割り当てられます。
サポーターは簡単なエンゲージメント コール (30 ~ 40 分) を実施して、SilverCloud の概要を説明し、目標を特定し、期待を設定し、自己紹介を行い、参加者にピア サポーターの役割を説明します。
その後、サポーターは、電話またはメッセージを通じて毎週のチェックインを提供します。
