SUPERA: Ondersteuning van interacties tussen leeftijdsgenoten om de toegang tot digitale cognitieve gedragstherapie uit te breiden voor Spaanstalige vangnetpatiënten in de eerstelijnszorg
Onderzoekers zullen de implementatie evalueren van een evidence-based, Spaanstalige, digitale interventie voor cognitieve gedragstherapie (SilverCloud) in de eerstelijnszorg voor Latino-patiënten met depressie en/of angst.
426 deelnemers zullen worden ingeschreven in een tweearmige proef waarin zelfgestuurde vs. ondersteunde dCBT (SilverCloud) wordt vergeleken.
Op het niveau van de aanbieder zullen onderzoekers de effectiviteit van doorverwijzingen van aanbieders vergelijken met het gebruik van een patiëntenregister in de kliniek om kandidaten te identificeren die baat zouden kunnen hebben bij een digitale interventie op het gebied van geestelijke gezondheid.

De onderzoekers zullen de implementatie evalueren van een evidence-based, Spaanstalige, digitale cognitieve gedragstherapie (dCBT)-interventie (SilverCloud) in vangnetklinieken voor eerstelijnszorg voor LEP-Latinx-patiënten met beperkte Engelse taalvaardigheid met depressie en/of angst.
Er zal een effectiviteit-implementatie hybride studie (Type 2) worden uitgevoerd met randomisatie op zowel provider- als patiëntniveau.
Op providerniveau zullen de onderzoekers outreach (met behulp van het patiëntenregister van de kliniek) vergelijken met inreach (traditionele providerverwijzing), op patiëntniveau zullen twee leveringswijzen van het DCBT-platform - ondersteund en niet-ondersteund - worden vergeleken.

Deelname Criteria
Onderzoekers zoeken naar mensen die aan een bepaalde beschrijving voldoen, de zogenaamde geschiktheidscriteria. Enkele voorbeelden van deze criteria zijn iemands algemene gezondheidstoestand of eerdere behandelingen.

Inclusiecriteria:
- PHQ-9 ≥ 10 of GAD-7 ≥ 8
- Heeft thuis toegang tot internet via smartphone en/of breedband, en een basisniveau van technologische kennis of bereidheid om een korte training in technologiegebruik te volgen
- ≥18 jaar
- Voorkeur voor het ontvangen van medische zorg in het Spaans
- Niet in gelijktijdige psychotherapie
- Als de patiënt momenteel een antidepressivum gebruikt, moet hij gedurende ten minste 6 weken een stabiele dosis hebben gehad en geen plannen hebben om de dosis te veranderen. We zullen patiënten die antidepressiva gebruiken toestaan om deel te nemen aan de studie, omdat dit de generaliseerbaarheid vergroot. De status van het antidepressivum zal bij elke beoordeling worden gecontroleerd om, indien nodig, op die effecten te controleren.
Uitsluitingscriteria:
- Momenteel ontvangen ze psychotherapie, aangezien deze behandeling zal worden aangeboden als een eerstelijnsbehandeling voor depressie en angst. Patiënten mogen echter worden doorverwezen naar behandeling terwijl ze deelnemen aan deze studie en de start van de behandeling zal worden gecontroleerd en overwogen in analyses
- Visuele, gehoor-, stem- of motorische stoornissen of analfabetisme die de voltooiing van studieprocedures zouden verhinderen
- Diagnose van een psychotische stoornis, bipolaire stoornis, dissociatieve stoornis of stoornis in het gebruik van middelen
- Ernstige suïcidaliteit (zoals beoordeeld door suïcidale gedachten, plannen en intenties te uiten). Hoewel er procedures met back-upplannen zijn voor patiënten die suïcidaliteit ontwikkelen (zie sectie Mensen), zullen patiënten die zijn beoordeeld met ernstige suïcidaliteit worden doorverwezen naar intensievere behandelingsbronnen.

Experimenteel: Behandeling
Deelnemers die willekeurig aan deze voorwaarde worden toegewezen, krijgen alle functies van het SilverCloud-platform aangeboden en krijgen ook een peer-supporter toegewezen die regelmatig ondersteuning biedt.

In de ondersteunde dCBT-conditie krijgt elke patiënt volledige toegang tot het SilverCloud-platform.
Daarnaast krijgen patiënten een begeleider toegewezen die op basis van een vastgesteld coachingsondersteuningsprotocol reguliere ondersteuning biedt.
Supporters houden een kort gesprek (30-40 minuten) om een overzicht te geven van SilverCloud, doelen te identificeren en verwachtingen te stellen, zichzelf voor te stellen en de deelnemer te oriënteren op de rol van de peer-supporter.
Supporters zorgen dan voor wekelijkse check-ins via telefoontjes of berichten.
Deelnemers kunnen via het platform met de supporters communiceren door middel van berichten of deelactiviteiten voor aanvullende discussie.
Het belangrijkste doel van de ondersteuning is het bevorderen van het gebruik van het platform.

Deelnemers die willekeurig aan deze voorwaarde worden toegewezen, krijgen alle functies van het SilverCloud-platform aangeboden, met uitzondering van de peer-ondersteuner.

Niet-ondersteunde dCBT biedt alle functies van het SilverCloud-platform, met uitzondering van ondersteuningsfuncties (gestructureerde interacties met supporter en deelfuncties).
Patiënten krijgen volledige toegang tot SilverCloud en worden geïnstrueerd om het gedurende 8 weken te gebruiken.
Patiënten ontvangen wekelijks geautomatiseerde berichten om betrokkenheid te stimuleren.

(d.w.z. huishoudelijke, werk- en/of schoolactiviteiten) en participatie in de samenleving.
