調査の概要
詳細な説明
中等度から重度の大うつ病エピソードを持つ患者 100 人が、この前向き多施設研究に登録されます。 1 年間に 6 回の訪問が予定されています。
- V0 (包含訪問): 包含基準と除外基準、情報、および同意の検証。
V1 (ランダム化前 - ベースライン状態):
- うつ病の重症度、生活の質、快感消失、無関心、認知機能障害について、検証済みのアンケートを使用した臨床評価。
- 一連のテストを使用して、モチベーション、感情処理、信念構築、およびそれらの更新を調査する神経認知評価。 検査の一部は機能的 MRI セッション中に実行されます。
- 構造 (解剖学的) および機能 MRI、ASL。
- 血液サンプル。
- 新しい抗うつ薬のランダム化と導入は、V1 の直後に行われます。 精神科医への紹介による受容性を最大限に高めるため、投与量と併用処方は担当精神科医の裁量に委ねられますが、割り当てられた治療法は 4 週間 (V3 まで) 変更されません。
V2 (新しい抗うつ薬の投与開始から 7 日後 - 「早期対応訪問」):
o V1 と同様。
V3 (新しい抗うつ薬の投与開始から 28 日後 - 「従来の対応訪問」):
- うつ病の重症度、生活の質、快感消失、無関心、認知機能障害について、検証済みのアンケートを使用した臨床評価。
- 血液サンプル
V4 (新しい抗うつ薬の投与開始から 6 か月後 - 「寛解訪問」):
- うつ病の重症度、生活の質、快感消失、無関心、認知機能障害について、検証済みのアンケートを使用した臨床評価。
- モチベーション、感情処理、信念の構築、およびそれらの更新を調査するための一連のテストを使用した認知評価。
- 構造(解剖学的)MRI、ASL
- 血液サンプル
V5 (新しい抗うつ薬の投与開始から 1 年後 - 「機能的寛解訪問」):
うつ病の重症度、生活の質、快感消失、無関心、認知機能障害について、検証済みのアンケートを使用した臨床評価。
神経疾患や精神疾患の病歴がなく、年齢、性別、教育が一致した健康なボランティア 36 名が含まれます。 V0 ~ V2 を実行します (V2 では MRI と血液サンプルは使用しません)。健康なボランティアは研究の一環としていかなる治療も受けません。

大うつ病性障害の患者
包含基準:
- 大うつ病性障害(単発または再発エピソード)の DSM-5 基準を満たす
- MADRS スコアが 24 以上の場合
- 新しい治療法が必要な場合
- 今回のエピソードでは以前の抗うつ薬の投与は行っていないか、影響の持ち越しを避けるために十分な期間の洗い流しは行っていません
- 有効な医療保険
除外基準:
- 治療抵抗性うつ病(適切な用量および期間で処方された抗うつ薬の少なくとも2回の試験にもかかわらず、不十分な反応として定義される)
- 現在のエピソードに対してエスシタロプラムおよび/またはボルチオキセチンの治験を受けている被験者、またはこれら2つの薬剤のいずれかに対して禁忌のある被験者
- 持続性うつ病性障害、双極性障害または統合失調症、神経発達障害、タバコ以外の容赦のない薬物乱用障害、追跡調査を困難にするほど重篤な人格障害(研究者の評価に基づく)の診断を受けた被験者。
- 神経疾患の既往歴のある者:パーキンソン病、認知症
- MRIスキャンの禁忌: 妊娠、閉所恐怖症、金属インプラント
- 妊娠中または授乳中の女性
- 非自発的入院と法的保護措置

治療反応は、モンゴメリー・アスバーグうつ病評価スケール (MADRS) で測定されます。 MADRS は、うつ病の重症度を評価するためにうつ病研究で広く使用されている 10 項目の尺度です。 反応は、ベースラインMADRSスコアと比較して2で割ったスコアによって定義され、寛解は、抗うつ薬戦略の開始から28日後のスコア<7(症状が消失)によって定義されます。 「計算表現型」は、行動の計算分析の結果です。 抽象的な単位で表現します。 V1 と V2 の間の計算上の表現型の変化は、MADRS スコアで測定される 28 日後の臨床反応を予測することを目的としたロジスティック回帰に入力されます。 |
