過敏性腸症候群 (IBS) 患者および対照ボランティアにおける糞便胆汁酸、糞便短鎖脂肪酸および腸内微生物叢: 食事の課題
この研究では、便中の胆汁酸、短鎖脂肪酸(SCFA)、および過敏性腸症候群(IBS)における腸内微生物叢の関係を調査する予定です。 この研究の中心的な仮説は、消化管マイクロバイオーム組成の特定の変化が結腸のSCFAおよびBAの変化と相関し、IBSの症状に寄与するというものである。 主な目的には以下が含まれます: (a) IBS サブタイプ (IBS-C および IBS-D) および対応する対照における消化管マイクロバイオームの特徴を同定し、これらのグループのマイクロバイオームの特徴が糞便 SCFA および感染後の難消化性炭水化物 (イヌリン) の細菌発酵と相関するかどうかをテストする。食事チャレンジ(糞便イヌリン)、および(b)IBSサブタイプおよび対照におけるGIマイクロバイオームサインが糞便BAまたはSCFA産生マーカー(糞便SCFAまたはイヌリン)と相関するかどうかを決定し、BAが糞便SCFAまたはイヌリンと相関するかどうかを試験する。
対象集団は、IBS (下痢と便秘の両方が優勢、IBS-D と IBS-C) および無症候性対照のローマ IV 基準を満たす 18 ~ 65 歳の成人です。 主な結果は、糞便胆汁酸排泄とプロファイル、短鎖脂肪酸排泄とプロファイル、結腸通過、および糞便微生物叢になります。 副次的結果は、検証された排便日誌に対する反応に基づく便の特徴となります。 4日間の100g脂肪食の最後の2日間に参加者から便サンプルを収集し、糞便微生物叢、SCFA、および胆汁酸分析のために3つのサンプルに分割します。

研究中:
スクリーニング/訪問 1
- 参加者は、研究に関するすべての質問に満足のいく回答が得られた後、このインフォームドコンセントを読み、署名するよう求められます。
- これはスクリーニングのための訪問であり、参加者が研究に参加し続ける資格がないと判断される場合があります。
- 研究チームは投薬内容と病歴を収集する。
- 医師による身体検査が行われます。 バイタルサインと身体測定値が収集されます。
- 参加者には、お持ち帰り用のラザニア料理 (ベジタリアンおよび非ベジタリアンのオプション) とデザートが提供されます。
- 参加者には、便採取キットと便の採取、保管、輸送に関する説明書が渡されます。
- 2~4日目の低繊維、高脂肪食の指示が提供されます。
- 参加者は、ASA24 と、研究期間中の腸の症状を記録するための排便パターン日記を使用して、食事の思い出しを完了するよう求められます。
- IBS 参加者は、過敏性腸症候群重症度スコアリング システム アンケート (IBS-SSS) に回答します。
2 ~ 4 日目 (自宅):
- 参加者は毎日、便の症状とパターンを日記に記録します。
- 参加者には、低繊維食(1日最大果物1個と野菜100g、全粒粉製品の代わりに小麦粉)を摂取し、アルコールを避け、4日間100gの脂肪食を開始するよう指示される。
- 5 日目の前夜、参加者は自宅で便サンプルを採取し、訪問 2 中に戻るように指示されます。
5 日目 (訪問 2):
- 参加者は一晩絶食した後、5日目にクリニックに戻ります。
- 生殖年齢範囲の参加者はイヌリン摂取前に尿妊娠検査を受け、研究チームは検査結果が陰性であることを確認する。
- 参加者は現地で朝食をお召し上がりいただけます。 非ベジタリアンのオプションは、パンケーキ、バター、シロップ、ゆで卵、ベーコンで構成されます。 ベジタリアン オプションでは、ベーコンの代わりに、200 mL の水でよく混ぜたチーズ スナック (約 790 ~ 840 kcal) とイヌリン Orafti® パウダー 10 g を混ぜます。
- 持ち帰り用の食事は昼食と夕食に提供され、朝食から4時間または8時間以内にお召し上がりください。
- IBS 参加者は、過敏性腸症候群重症度スコアリング システム (IBS-SSS) のアンケートに回答します。
- 便サンプルは5日目に収集されます。
- 参加者は、研究期間中、ASA24 と腸の症状を記録するための排便パターン日記を使用して、食事の思い出しを完了し続けます。
6日目
- 参加者は、最終的な便サンプルを収集するように指示されます (6 日目)。
- 参加者は、研究期間中、ASA24 と腸の症状を記録するための排便パターン日記を使用して、食事の思い出しを完了し続けます。
7 日目 (訪問 3)
• 参加者は診療所に戻り、採便 (5 日目と 6 日目)、毎日の日記、および IBS-SSS フォーム (該当する場合) を返却します。
30 ~ 90 日目 (オプションの食事フォローアップ): 24 時間の食事リコールの有用性をさらに評価するために、意欲のある参加者は、自動自己管理食事療法を完了するオプションのフォローアップ活動に参加するよう招待されます。訓練を受けた面接官の支援の有無にかかわらず、評価ツール。 興味のあるボランティアには電話で連絡があり、研究チームからオンラインでツールを完了する方法についての指示が提供されます。

除外基準:
- 炎症性腸疾患、セリアック病、または特定の種類の腹部がん、および甲状腺や肝臓に問題のある患者
- -治験参加後6か月以内の腹部手術または腹部放射線照射(帝王切開または胆嚢切除は除く)
- 胃腸機能や研究の解釈に影響を与えることが知られている処方薬、市販薬、または漢方薬(オピオイド、炎症薬、特定の抗うつ薬など)を、健康なボランティアの場合は研究参加前6か月以内に、健康なボランティアの場合は研究参加後2日以内に使用した場合。 IBSの参加者。
- 1か月を超えて抗うつ薬を服用している個人に対する、安定した低用量の抗うつ薬の限定的使用については例外が認められる。
- 重度の便秘を緩和し、便の採取を可能にするビサコジル(ダルコラックス)などの緊急治療薬の使用は、必要に応じて許可されます。
- オゼンピックおよびオゼンピックタイプの薬剤の使用
- 妊娠中または授乳中の女性
- 研究参加後3か月以内の抗生物質の使用
- -研究開始前2週間以内のプレバイオティクスまたはプロバイオティクスの使用
