Prikkelbare darmsyndroom en controlevrijwilligers: dieetuitdaging
Fecale galzuren, fecale vetzuren met korte keten en de darmmicrobiota bij patiënten met het prikkelbare darm syndroom (IBS) en controlevrijwilligers: dieetuitdaging
De studie zal de relatie onderzoeken tussen fecale galzuren, vetzuren met een korte keten (SCFA's) en de darmmicrobiota bij het prikkelbare darm syndroom (IBS). De centrale hypothese van deze studie is dat specifieke verschuivingen in de samenstelling van het GI-microbioom correleren met veranderde SCFA's en BA's van het colon en bijdragen aan PDS-symptomen. Primaire doelstellingen zijn onder meer: (a) het identificeren van GI-microbioomsignaturen in IBS-subtypes (IBS-C en IBS-D) en gematchte controles, en testen of microbioomsignaturen in deze groepen correleren met fecale SCFA's en bacteriële fermentatie van een onverteerbaar koolhydraat (inuline) na een dieetuitdaging (fecale inuline), en (b) bepalen of GI-microbioomsignaturen in IBS-subtypen en controles correleren met fecale BA's of markers van SCFA productie (fecale SCFA's of inuline) en test of BA's correleren met fecale SCFA's of inuline.
De doelpopulatie bestaat uit volwassenen in de leeftijd van 18-65 jaar die voldoen aan de Rome IV-criteria voor PDS (zowel diarree als constipatie overheersen, IBS-D en IBS-C) en asymptomatische controles. De primaire uitkomsten zijn de uitscheiding en het profiel van fecale galzuren, de uitscheiding en het profiel van vetzuren met een korte keten, de colontransit en de fecale microbiota. Secundaire uitkomsten zijn de kenmerken van de ontlasting, gebaseerd op reacties op gevalideerde darmdagboeken. Ontlastingsmonsters zullen worden verzameld van deelnemers tijdens de laatste 2 dagen van een 4-daags dieet met 100 g vet en opgesplitst in 3 monsters voor fecale microbiota, SCFA en galzuuranalyse.

Tijdens de studie:
Screening/bezoek 1
- Deelnemers wordt gevraagd deze geïnformeerde toestemming te lezen en te ondertekenen nadat alle vragen over het onderzoek naar tevredenheid zijn beantwoord.
- Dit is een screeningsbezoek en we kunnen vaststellen dat een deelnemer niet in aanmerking komt om verder deel te nemen aan het onderzoek.
- Het onderzoeksteam verzamelt medicatie en medische geschiedenis.
- Er zal een lichamelijk onderzoek worden uitgevoerd door de arts. Vitale functies en lichaamsafmetingen worden verzameld.
- Deelnemers krijgen een lasagnemaaltijd om mee naar huis te nemen (vegetarische en niet-vegetarische optie) en een dessert.
- Deelnemers ontvangen een ontlastingsverzamelpakket en instructies voor het verzamelen, opslaan en vervoeren van de ontlasting.
- Er worden instructies gegeven voor een vezelarm en vetrijk dieet voor dag 2-4.
- Deelnemers wordt gevraagd het dieetherinnering in te vullen met behulp van de ASA24 en een darmpatroondagboek om darmsymptomen in de loop van het onderzoek vast te leggen.
- PDS-deelnemers vullen de vragenlijsten voor het prikkelbaredarmsyndroom-ernstscoresysteem (IBS-SSS) in.
Dag 2-4 (thuis):
- Deelnemers registreren dagelijks ontlastingssymptomen en -patronen in een dagboek.
- Deelnemers krijgen de instructie om een vezelarm dieet te volgen (maximaal één stuk fruit en 100 g groenten per dag, en witte bloem in plaats van volkorenproducten), alcohol te vermijden en een vierdaags dieet met 100 g vet te starten.
- Op de avond vóór dag 5 krijgen de deelnemers de opdracht om thuis een ontlastingsmonster te verzamelen en terug te brengen tijdens bezoek 2.
Dag 5 (bezoek 2):
- Deelnemers keren op dag 5 terug naar de kliniek na een nacht vasten.
- Deelnemers in de reproductieve leeftijdscategorie krijgen vóór de inname van inuline een urinezwangerschapstest en het onderzoeksteam zal een negatief testresultaat bevestigen.
- De deelnemers krijgen ter plaatse een ontbijt. De niet-vegetarische optie bestaat uit pannenkoeken, boter, siroop, hardgekookte eieren en spek. Bij de vegetarische optie wordt het spek vervangen door een kaassnack (ongeveer 790 tot 840 kcal) samen met 10 g inuline Orafti®-poeder), grondig gemengd in 200 ml water.
- Er worden maaltijden voor thuis meegebracht voor de lunch en het diner, die 4 en 8 uur na het ontbijt moeten worden genuttigd.
- Deelnemers aan PDS vullen de vragenlijst van het prikkelbaredarmsyndroom-ernstscoresysteem (IBS-SSS) in.
- Op dag 5 wordt een ontlastingsmonster verzameld.
- Deelnemers zullen doorgaan met het voltooien van het dieetherinnering met behulp van de ASA24 en een darmpatroondagboek om darmsymptomen in de loop van het onderzoek vast te leggen.
Dag 6
- Deelnemers krijgen de opdracht om een laatste ontlastingsmonster te verzamelen (dag 6).
- Deelnemers zullen doorgaan met het voltooien van het dieetherinnering met behulp van de ASA24 en een darmpatroondagboek om darmsymptomen in de loop van het onderzoek vast te leggen.
Dag 7 (bezoek 3)
• Deelnemers keren terug naar de kliniek om de ontlastingscollecties (dag 5 en dag 6), de dagelijkse dagboeken en de IBS-SSS-formulieren (indien van toepassing) terug te brengen.
Dag 30-90 (Optionele follow-up over het dieet): Om het nut van de 24-uursdieetherinnering verder te beoordelen, worden bereidwillige deelnemers uitgenodigd om deel te nemen aan een optionele vervolgactiviteit waarin ze het geautomatiseerde, zelfbeheerde dieetprogramma voltooien. Assessment Tool met of zonder hulp van een getrainde interviewer. Geïnteresseerde vrijwilligers zullen telefonisch worden gecontacteerd en zullen door het onderzoeksteam instructies krijgen over hoe de tool online kan worden ingevuld.

Inclusiecriteria:
- Leeftijd 18-75
- Personen met het prikkelbare darm syndroom (IBS)
- Gezonde vrijwilligers (gezonde controles) zonder voorgeschiedenis van gastro-intestinale (GI) ziekten of symptomen
- Geen dieetbeperkingen anders dan vegetarisch
Uitsluitingscriteria:
- Inflammatoire darmaandoeningen, coeliakie of bepaalde soorten buikkanker, evenals mensen met schildklier- of leverproblemen
- Buikoperatie of buikbestraling binnen 6 maanden na deelname aan het onderzoek, met uitzondering van keizersnede of galblaasverwijdering
- Gebruik van receptgeneesmiddelen, zonder recept verkrijgbare kruidengeneesmiddelen waarvan bekend is dat ze de gastro-intestinale functie of de interpretatie van het onderzoek beïnvloeden, zoals opioïden, ontstekingsmedicijnen of bepaalde antidepressiva, binnen 6 maanden vóór deelname aan het onderzoek voor gezonde vrijwilligers, of binnen 2 dagen na deelname aan het onderzoek voor deelnemers met PDS.
- Er wordt een uitzondering toegestaan voor beperkt gebruik van stabiele lage doses antidepressiva door personen die deze langer dan een maand gebruiken.
- Reddingsmedicijnen zoals Bisacodyl (dulcolax) om ernstige constipatie te verlichten en het opvangen van ontlasting mogelijk te maken, zijn toegestaan wanneer dat nodig is.
- Gebruik van medicijnen van het Ozempic- en Ozempic-type
- Zwangere vrouwen of vrouwen die borstvoeding geven
- Antibioticagebruik binnen 3 maanden na deelname aan het onderzoek
- Gebruik van prebiotica of probiotica binnen de 2 weken vóór aanvang van het onderzoek
- Regelmatig tabaksgebruik in de afgelopen 6 maanden
