この臨床試験の目的は、脳卒中の亜急性期および慢性期における上肢運動リハビリテーションの強化における個別オンライン反復経頭蓋磁気刺激 (rTMS) の有効性を調査することです。 また、個別化されたオンライン rTMS 介入法の安全性についても学びます。 回答することを目的とした主な質問は次のとおりです。
- 運動トレーニングと同期したrTMSは、運動誘発電位(MEP)を誘発する能力を持つ患者の運動リハビリテーションを改善しますか?
- 運動トレーニングと同期した用量の個別のrTMSは、MEPを誘発する能力のない患者の運動リハビリテーションを改善しますか?
研究者は、予備試験でオンライン rTMS が運動機能障害を軽減するのに効果があるかどうかを確認するために、MEP 患者におけるオンライン rTMS とオフラインおよび偽刺激を比較します。 また、個別化オンライン rTMS を個別化されていないオンライン rTMS および MEP のない患者を対象とした偽のオンライン rTMS と比較し、ランダム化二重盲検対照試験で個別化オンライン rTMS が運動機能障害を軽減する効果があるかどうかを確認します。
参加者は次のことを行います:
- 各人のMEP状態に応じて1つのグループにランダムに割り当てられます。
- rTMS 治療を 10 日間、週に 5 日間勤務し、合計 2 週間受けます。
- 治療全体の前後に磁気共鳴画像法 (MRI) と脳波 (EEG) の評価を受けます。
- 治療の前日、治療の 1 日、1 か月、および 3 か月後に体重計と MEP 評価を実施します。

予備実験では、患者はオンライン、オフライン、偽のグループの 3 つのグループにランダムに割り当てられました。 患者が MEP を誘発できた場合、TMS の強度は 80% RMT になります。そうでない場合、TMS の強度は TMS 出力の 70% ~ 80% になります。
RCTでは、MEPのない患者は、個別化グループ、非個別化グループ、および偽グループの3つのグループにランダムに割り当てられました。 患者は 2 週間にわたって刺激を受け、毎日 4 セッションのタスクが実行されました。 患者は 2 週間 (10 日間) の刺激プロトコルを受け、毎日 40 のタスクが実行されます。 これには、10 個のベースライン タスク測定の 1 セッションと、10 個の運動タスクと同期した 5 Hz rTMS の 3 セッションが含まれます。 rTMSは同運動皮質に適用され、TMS出力の70〜80%の強度で1日あたり約750パルス、合計7,500パルスを送達します。

説明
包含基準:
- 患者は、神経学的検査、CT または MRI スキャンによって最初に脳卒中と診断されます。
- バイタルサインは安定していますが、ある程度の上肢の運動機能障害があります。
- 年齢は18歳から80歳くらいです。
- 認知能力には大きな影響はなく、MMSE スコアが 20 点以上であれば、患者はさまざまな検査や評価に協力できます。
- 重篤な合併症(肺炎、心不全、尿路感染症、栄養失調など)はありません。
- 病歴上、TMS の禁忌となる病理学的状態はありません (たとえば、脳内に動脈瘤クリップなどの金属が挿入されている患者、心臓ペースメーカーを装着している患者、妊娠中の女性、またはてんかん発作の病歴のある患者)。
- 患者または保護者は、インフォームドコンセントフォームに署名することに同意します。
除外基準:
- 重度の心臓、肺、肝臓、腎臓の疾患および悪性腫瘍の患者。
- 失語症、重度の認知障害、または精神疾患の病歴のある方。
- 先月にてんかん発作の病歴がある患者、または最近抗てんかん薬を服用している患者。
- 重度の視覚障害または聴覚障害があり、正常にコミュニケーションができない方。

The Fugl-Meyer Assessment (FMA) is a stroke-specific, performance-based impairment index. It is designed to assess motor functioning, balance, sensation and joint functioning in patients with post-stroke hemiplegia. It is applied clinically and in research to determine disease severity, describe motor recovery, and to plan and assess treatment. The Fugl-Meyer Assessment - Upper Extremity (FMA-UE) is the upper limb motor domain includes items assessing movement, coordination, and reflex action of the shoulder, elbow, forearm, wrist, hand. The Action Research Arm Test (ARAT) is a 19 item observational measure used by physical therapists and other health care professionals to assess upper extremity performance (coordination, dexterity and functioning) in stroke recovery, brain injury and multiple sclerosis populations. The ARAT was originally described by Lyle in 1981 as a modified version of the Upper Extremity Function Test and was used to examine upper limb functional recovery post damage to the cortex. The WMFT is used to assess the upper limb motor function of patients and serves as an effective supplement to the FMA-UE. By timing single-joint movements, multi-joint movements, and functional activities, as well as evaluating movement quality and speed, it can quantitatively assess the upper limb motor ability of stroke patients. It is more sensitive to patients with mild to moderate stroke, and its grading is relatively detailed, which can sensitively reflect the subtle changes in patients' motor function during evaluation. The WMFT consists of 15 items, among which items 1-6 are simple joint movements, and items 7-15 are complex functional actions. This scale can not only evaluate impairments but also assess the effect of training on disabilities. The WMFT has good validity and reliability and can be used to evaluate the upper limb function of stroke patients. WMFT score was assessed by site raters who were masked to the intervention. |
