この観察研究は、TriNetXグローバル共同ネットワーク(2010-01-01から2025-07-01)からの非識別化された電子健康記録データを使用して、周術期のデクスメデトミジン(DEX)が成人心臓手術後の30日間の転帰と関連するかどうかを調べるものです。
18〜100歳の成人で、初回の冠動脈バイパス移植術または心臓弁手術を受ける患者が対象となります。
曝露は、手術前24時間から手術後48時間までのいずれかのDEX投与であり、対照群はこの期間内にDEXを投与されません。
主要転帰は30日以内のせん妄です。
副次転帰は、30日以内の全死因死亡率、急性腎障害、肺炎、敗血症、赤血球輸血/主要出血、心筋梗塞、虚血性脳卒中/一過性脳虚血発作、心房細動、96時間超(4〜30日目)の人工呼吸管理、および30日以内(1〜30日目)の再入院です。

初回の冠動脈バイパス移植術または心臓弁手術を受ける18歳から100歳の成人を特定します。
周術期曝露は、索引手術の24時間前から48時間後までのデクスメデトミジン投与と定義され、比較コホートはこの期間にデクスメデトミジンを受けなかった患者で構成されます。
術後30日以内の臨床転帰には、主要転帰としてせん妄、および主要な術後合併症と死亡率の範囲が含まれます。

Dexmedetomidine medication record present
Eligible patients with a qualifying CABG-coded procedure and any RxNorm 48937 dexmedetomidine medication record on the preceding calendar day or the procedure day (days -1 through 0).
The same medication-record definition is used in the separately constructed exploratory valve-coded population relative to its own qualifying procedure.
Exposure is observed in routine records and is not assigned by investigators.
Administration, dose and intraday timing are not confirmed.
The CABG-coded and valve-coded populations may overlap and are reported separately.

No drug, dose, route or regimen is assigned by the study.
