この臨床試験の目的は、成人における胃食道逆流症(GERD)の症状緩和に対するフェクスプラザン40mgの有効性と安全性を評価することでした。 この研究では、GERDの一般的な治療薬であるエソメプラゾール40mgとフェクスプラザンも比較しました。

この多施設共同、無作為化、オープンラベル、活性薬対照臨床試験は、胃食道逆流症(GERD)の成人患者において、カリウム競合型酸分泌阻害薬(P-CAB)であるフェクスプラザン40mgと、プロトンポンプ阻害薬(PPI)であるエソメプラゾール40mgの有効性および安全性を比較評価するために実施されました。
GERDは、胸やけや酸の逆流などの逆流関連症状を特徴とする慢性疾患であり、生活の質を著しく損なう可能性があります。PPIは第一選択療法として広く使用されていますが、作用発現の遅延や夜間症状の不十分な抑制を含む限界が報告されています。新しいP-CABであるフェクスプラザンは、H⁺/K⁺-ATPaseに対する可逆的かつカリウム競合的な阻害により胃酸分泌を抑制し、これまでの臨床試験で迅速な作用発現と持続的な酸抑制が実証されています。
適格な参加者は、フェクスプラザン40mgまたはエソメプラゾール40mgを1日1回経口投与する群に1:1の割合で無作為に割り付けられました。初期治療期間は4週間でした。初期治療期間後に十分な症状緩和が得られなかった参加者は、割り付けられた治療をさらに4週間継続することができ、最長8週間の治療期間となりました。
有効性は、主に、妥当性が確認された質問票と日々の症状日記を用いた患者報告による症状評価を通じて評価されました。安全性評価には、研究全体および追跡調査期間中の有害事象、バイタルサイン、身体検査、および臨床検査値のモニタリングが含まれました。

対象基準:
- 文書によるインフォームド・コンセントを提供する時点で18歳から60歳までの成人
- 男性または女性の参加者
- 胸やけおよび/または酸逆流を含む胃食道逆流症(GERD)の症状の既往歴
以下により確認されたGERD症状:
- GERD-Qスコアが7を超える、および
- 過去7日間のうち3日以上で発生した胸やけおよび/または酸逆流
- 研究手順を理解し、質問票および毎日の症状日誌を完了できること
- 文書によるインフォームド・コンセントを提供する意思と能力があること
除外基準:
消化器疾患:
- 炎症性腸疾患(クローン病、潰瘍性大腸炎)、原発性食道運動障害、または膵炎の診断
- 酸分泌に影響を与える胃または食道手術の既往歴(虫垂切除術、胆嚢摘出術、良性ポリープの内視鏡的ポリペクトミーを除く)
- 消化器悪性腫瘍を示唆する警告症状(例:重度の嚥下困難、嚥下痛、消化管出血、貧血、原因不明の体重減少)があり、悪性腫瘍が否定されていない場合
既往歴
- 臨床的に有意な肝臓、腎臓、内分泌、血液、腫瘍、または泌尿器系疾患
- 過去5年以内の悪性腫瘍の既往歴(完全に治療され、5年以上再発のない非消化器悪性腫瘍を除く)
- 精神病、薬物乱用、またはアルコール乱用の既往歴
- 既知のHIV感染、活動性B型肝炎、またはC型肝炎感染(HCV RNA陽性)
薬物および治療
登録前2週間以内の禁止併用薬の使用、または研究期間中の継続的な禁止薬の必要性、以下を含む:
- プロトンポンプ阻害薬、カリウム競合的酸ブロッカー、H2受容体拮抗薬、またはその他の酸分泌抑制薬
- 特定の向精神薬、抗コリン薬、鎮痙薬、全身性ステロイド、または粘膜保護薬

Symptom relief was assessed using the Gastroesophageal Reflux Disease Questionnaire (GERD-Q).
The GERD-Q total score ranges from a minimum of 0 to a maximum of 18, where higher scores indicate a worse outcome (greater severity and higher likelihood of GERD).
The primary outcome was defined as the proportion of participants with a GERD-Q total score <8 at Week 4, indicating adequate relief of GERD symptoms.

This assessment applied only to participants who continued treatment beyond the initial 4-week period.
