이 임상시험의 목적은 성인에서 위식도역류질환(GERD) 증상 완화를 위한 펙수프라잔 40 mg의 유효성과 안전성을 평가하는 것이었습니다. 본 연구는 또한 GERD에 일반적으로 사용되는 치료제인 에소메프라졸 40 mg과 펙수프라잔을 비교했습니다.

연구 참가자들은:

상세 설명
이 다기관, 무작위, 개방형, 활성 대조군 임상 연구는 위식도 역류병(GERD) 성인 환자에서 프로톤 펌프 억제제(PPI)인 에소메프라졸 40 mg과 비교하여 칼륨 경쟁적 산 억제제(P-CAB)인 펙수프라잔 40 mg의 유효성과 안전성을 평가하기 위해 수행되었습니다.
GERD는 속쓰림과 산 역류와 같은 역류 관련 증상으로 특징지어지는 만성 질환으로, 삶의 질을 크게 저하시킬 수 있습니다. PPI가 일차 치료제로 널리 사용되고 있지만, 작용 시작 지연과 야간 증상 조절 부족 등의 한계가 보고된 바 있습니다. 새로운 P-CAB인 펙수프라잔은 H⁺/K⁺-ATPase에 대한 가역적이고 칼륨 경쟁적 억제를 통해 위산 분비를 억제하며, 이전 임상 연구에서 빠른 작용 시작과 지속적인 산 억제 효과를 입증하였습니다.
적격 참가자는 1:1 비율로 무작위 배정되어 펙수프라잔 40 mg 또는 에소메프라졸 40 mg을 하루 한 번 경구 투여받았습니다. 초기 치료 기간은 4주였습니다. 초기 치료 기간 후 충분한 증상 완화를 달성하지 못한 참가자는 추가 4주 동안 배정된 치료를 계속할 수 있었으며, 총 치료 기간은 최대 8주였습니다.

- 남성 또는 여성 참가자
- 속쓰림 및/또는 산 역류를 포함한 위식도 역류병(GERD) 증상 병력
다음 중 하나 이상으로 확인된 GERD 증상:
- GERD-Q 점수가 7점 초과, 및
- 지난 7일 동안 3일 이상 발생한 속쓰림 및/또는 산 역류
- 연구 절차를 이해하고 설문지 및 일일 증상 일지를 작성할 수 있음
- 서면 동의서를 작성할 의사와 능력이 있음
제외 기준:
위장관 질환:
- 염증성 장질환(크론병, 궤양성 대장염), 원발성 식도 운동 장애 또는 췌장염 진단
- 산 분비에 영향을 미치는 위 또는 식도 수술 병력(충수절제술, 담낭절제술 또는 양성 용종의 내시경적 용종절제술 제외)
- 위장관 악성종양을 시사하는 경고 증상(예: 심한 연하곤란, 통증성 연하장애, 위장관 출혈, 빈혈, 설명되지 않는 체중 감소)이 있는 경우, 악성종양이 배제되지 않는 한
병력
- 임상적으로 유의한 간, 신장, 내분비, 혈액, 종양 또는 비뇨기계 질환
- 과거 5년 이내 악성종양 병력(완전히 치료된 후 ≥5년 동안 재발 없이 유지된 비소화기계 악성종양 제외)
- 정신병, 약물 남용 또는 알코올 남용 병력
- 알려진 HIV 감염, 활동성 B형 간염 또는 C형 간염 감염(HCV RNA 양성)
약물 및 치료
등록 2주 이내에 금지된 병용 약물 사용 또는 연구 기간 동안 지속적인 금지 약물 필요, 포함:
- 양성자 펌프 억제제, 칼륨 경쟁적 산 억제제, H2 수용체 길항제 또는 기타 산 억제제
- 특정 정신약물, 항콜린제, 항경련제, 전신 스테로이드 또는 점막보호제
- 연구 약물 투여 4주 이내에 다른 연구용 의약품 사용

Symptom relief was assessed using the Gastroesophageal Reflux Disease Questionnaire (GERD-Q).
The GERD-Q total score ranges from a minimum of 0 to a maximum of 18, where higher scores indicate a worse outcome (greater severity and higher likelihood of GERD).
The primary outcome was defined as the proportion of participants with a GERD-Q total score <8 at Week 4, indicating adequate relief of GERD symptoms.
