臨床試験:
この臨床試験の目的は、統合失調症および関連疾患を持つ患者において、ダパグリフロジンが抗精神病薬による体重増加を軽減できるかどうかを検討することです。 また、ダパグリフロジンの安全性も評価します。

- 研究デザイン これは、オマーンのマスカットにあるスルタン・カブース大学病院で行われる26週間のランダム化比較試験です。

参加資格
16歳以上の方で:

- 開始時、および4週目、12週目、26週目にクリニックを訪問します
- 進捗状況と副作用を確認するために、2週目、8週目、18週目に電話連絡を受けます
- 血液検査と測定(体重、ウエスト、血糖、コレステロール)を受けます
- 身体活動、気分、生活の質に関する質問票に記入します

適格基準:
- 16歳以上。
- DSM-5に基づく統合失調症スペクトラム障害またはその他の精神病性障害の診断(物質/薬剤誘発性または医学的状態に関連する精神病、他の障害による緊張病、または特定不能の緊張病を除く)。
- 登録前少なくとも3ヶ月間、安定した抗精神病薬単剤療法を受けていること。
抗精神病薬誘発性体重増加の証拠(以下のいずれかを満たすもの):
- 治療前ベースラインからの体重が7%以上増加、または
- 抗精神病薬関連の体重増加が記録されている状態で、体格指数(BMI)が25 kg/m²を超えること。
- 安定した精神症状であり、臨床的にインフォームドコンセントを提供し、研究に参加できると判断されること。
除外基準:
- 糖尿病(1型または2型)。
- 腎機能障害(推定糸球体濾過量<45 mL/分/1.73 m²)、肝疾患、またはその他の重篤な医学的疾患。
- 妊娠中または授乳中。
- 過去3ヶ月以内の減量薬または減量プログラムの使用。
- メトホルミンまたはダパグリフロジンに対する既知の過敏症。

実験的:ダパグリフロジン
|
All participants will receive a standardized common lifestyle program.
This will include dietary counselling, physical activity counselling, and behavioural support delivered at baseline and reinforced during scheduled in-person visits and telephone follow-up.
Physical activity will be assessed using the International Physical Activity Questionnaire.
Dapagliflozin will be administered orally at a dose of 10 mg once daily.
Treatment will continue for 26 weeks.
Participants will be monitored for adherence, tolerability, and adverse events, including genitourinary symptoms, dehydration, sick-day events, and symptoms suggestive of ketoacidosis.

Metformin will be administered orally.
Participants will start with 500 mg twice daily, taken with meals, and will be titrated to 1000 mg twice daily by the second week as tolerated.
Slower titration may be used if gastrointestinal side effects occur.
Treatment will continue for the study period, with the primary endpoint assessed after 26 weeks at the maximum tolerated dose.
All participants will receive a standardized common lifestyle program.
This will include dietary counselling, physical activity counselling, and behavioural support delivered at baseline and reinforced during scheduled in-person visits and telephone follow-up.
Physical activity will be assessed using the International Physical Activity Questionnaire.

Self-reported physical activity will be assessed using the International Physical Activity Questionnaire-Short Form (IPAQ-SF).
Physical activity will be measured to describe participant activity levels and to support adjustment for physical activity as a potential confounding variable in analyses of body weight and metabolic outcomes.
