一次医療を受ける患者のほぼ50%が不眠症の症状(例:睡眠開始困難および/または睡眠維持困難)を報告しています。 このような睡眠関連の困難は、新たな併存疾患の発症を予兆するだけでなく、既存の併存疾患を悪化させ、また既存の併存疾患によって悪化することもあります。 したがって、一次医療患者の不眠症を効果的に治療することは、より良い公衆衛生を促進するための未開拓の手段です。
研究課題:
不眠症に対する認知行動療法(CBT-I)は第一選択治療と考えられていますが、ほとんどの患者(特に一次医療の患者)はこの形態の治療にアクセスできません。

プロトコル概要:

適格基準
- 18〜80歳の成人。
- DSM-5の不眠症の診断基準を満たす。
- 不眠重症度指数(ISI)で15点以上。
- 睡眠開始および/または睡眠維持に関する訴え:30分以上持続し、週3夜以上発生し、3か月以上持続している。
- 登録前にすべての睡眠関連薬の使用を中止する意思がある。
- 研究薬を開始する前に2週間のウォッシュアウト期間を完了する。
- 参加について臨床医の同意を提供する意思がある。
除外基準
以下の基準のいずれかを満たす患者は不適格となります:自己申告による日中仮眠(週3日以上、1日1時間以上); 自殺企図の既往または現在の自殺念慮、治療で制御されていない急性または慢性の精神疾患または身体的疾患(かかりつけ医による)、または現在のアルコールまたは薬物乱用; または、概日リズム障害(位相前進または位相遅延症候群)、交代勤務関連睡眠障害(午後11時頃から午前7時まで勤務する「昼寝」者)、およびローテーション勤務スケジュールの者を含む、他の睡眠障害の診断(またはその高いリスク)。 適格性を判断するために、すべての被験者は、オンラインスクリーナー、インテークインタビュー、被験者のEMRのレビュー、および最終的には患者のかかりつけ医の同意の受領を含む多層プロセスを使用してスクリーニングされます。 以下は、除外基準の追加リストと詳細(AD)を示します:
一般的な考慮事項
- 年齢が18歳未満または80歳超
