Yksityiskohtainen kuvaus
Lähes 50 % perusterveydenhuollon potilaista raportoi unettomuuden oireita (esim. ongelmia nukahtamisessa ja/tai unen ylläpitämisessä). Tällaiset unen laatuun liittyvät vaikeudet voivat ennustaa uuden komorbidisen sairauden puhkeamista sekä pahentaa ja pahentua olemassa olevien komorbidisten sairauksien vuoksi. Näin ollen unettomuuden tehokas hoito perusterveydenhuollon potilaissa on hyödyntämätön keino paremman kansanterveyden edistämiseksi.
Tutkimuskysymys:
Vaikka unettomuuden kognitiivinen käyttäytymisterapia (CBT-I) pidetään ensisijaisena hoitona, useimmilla potilailla (erityisesti perusterveydenhuollossa) ei ole pääsyä tähän hoitomuotoon. Sen sijaan suurin osa hoidetuista potilaista käyttää reseptivapaita lääkkeitä (OTC), kuten melatoniinia ja difenhydramiinia (esim. Benadryl), tai heille on määrätty hypnoottisia lääkkeitä (yleisimmin trazodonia tai zolpidemia [esim. Ambien]). Yllättäen näiden yleisesti käytettyjen lääkkeiden absoluuttisesta tai suhteellisesta tehokkuudesta ja turvallisuudesta tiedetään vähän, ja usein käytetyistä lääkkeistä vain Ambien on FDA:n ja ammattimaisen lääketieteen tai unilääketieteen yhdistysten hyväksymä/suosittelema. On myös rajallista tietoa siitä, millä näistä lääkkeellisistä strategioista on paras riski/hyöty-profiili tai mikä on potilaille hyväksyttävin. Tutkijoiden äskettäinen arvio FDA:n hyväksymistä unettomuuslääkkeistä viittaa siihen, että harvemmin määrätty doxepiinilla on optimaalisin riski/hyöty-profiili. Useat viranomaiset, kuten Agency for Healthcare Research and Quality (AHRQ), National Institutes of Health (NIH) ja PCORI, ovat vaatineet tiukkoja vertailevia tehokkuustutkimuksia näiden tietoaukkojen täyttämiseksi. Lisäksi tutkijoiden palaute sidosryhmiltä osoittaa, että tällaisten tietojen tarve ei ole vain ammatillinen käytäntöhuoli, vaan sen jakavat perusterveydenhuollon potilaat ja kliinikot. Näin ollen tieteelliseen näyttöön perustuvaa ohjeistusta unettomuuden hoidosta OTC- ja reseptilääkkeillä tarvitaan kiireellisesti.
Protokolla-yhteenveto:
Tutkijat ehdottavat laajan kaksoissokkotutkimuksen, placebolla kontrolloidun sekvensiaalisen moninkertaisen määrityksen satunnaistetun kokeen (SMART) suorittamista, jossa verrataan potilaiden yleisesti käyttämien reseptivapaiden unettomuuslääkkeiden (eli difenhydramiinin ja melatoniinin) sekä kliinikkojen yleisesti määräämien reseptilääkkeiden (eli zolpidemin ja trazodonin) tai harvemmin käytettyjen, mutta mahdollisesti optimaalisemman riski/hyöty-profiilin omaavien lääkkeiden (eli doxepiinin) suhteellista tehokkuutta ja turvallisuutta. Kaikissa tutkimusryhmissä käytetään yökohtaista annostelustrategiaa ja sisällytetään unihygieniaopetus. Paremman yhteensovittamiseksi nykykäytäntöihin osallistujat, jotka sietävät alkuperäistä matalampaa lääkeannosta mutta eivät näytä hoitovastetta, siirtyvät korkeampaan annokseen 2 viikon jälkeen. Kuukauden kohdalla hoitovastetta saaneet seurataan jopa 6 kuukautta ymmärtääkseen hoidon hyötyjen pitkäaikaisempaa ylläpitämistä. SMART-suunnittelu uudelleensatunnoistaa kuukauden kohdalla hoitovastetta saamattomat osallistujat vaihtoehtoiseen ryhmään (jokaisen seuraavan hoitovasteen puuttuessa), tarjoten kaikille osallistujille mahdollisuuden saavuttaa hoitovaste millä tahansa tutkimuslääkkeistä. Sokotuksen ylläpitämiseksi osallistujia ohjeistetaan ottamaan jokainen lääke (mukaan lukien placebo) 30 minuuttia ennen nukkumaanmenoa, ja kaikki lääkkeet valmistetaan keskitetyssä apteekissa näyttämään identtisiltä. Kaikkia osallistujia ohjeistetaan edelleen olemaan sängyssä 7–9 tunnin ajan sekä turvallisuuden edistämiseksi että mahdollisesti lääkkeiden vaikutusten lisäämiseksi aamuvarhaisessa heräämisessä ja kokonaiseen unen määrään.

Sisällyttämiskriteerit
- Aikuiset 18–80-vuotiaat.
- DSM-5-kriteerit unettomuushäiriölle täyttyvät.
- Vähintään 15 pistettä Unettomuuden Vakavuusindeksissä (ISI).
- Unen saanti- ja/tai ylläpito-ongelmat: kestoltaan vähintään 30 minuuttia, esiintyvät vähintään 3 yötä/viikko, ja kesto vähintään 3 kuukautta.
- Halukkuus lopettaa kaikkien unilääkkeiden käyttö ennen osallistumista.
- Kahden viikon peseytymisjakson suorittaminen ennen minkään tutkimuslääkkeen aloittamista.
- Halukkuus antaa lääkärin suostumus osallistumiseen.
Poissulkemiskriteerit
Potilaat eivät ole kelpoisia, jos he täyttävät minkä tahansa seuraavista kriteereistä: omailmoitettu päiväunien ottaminen (vähintään 1 tunti päivässä vähintään 3 päivää viikossa); itsemurhayritysten historia tai ajatukset, akuutti tai krooninen psyykkinen tai lääketieteellinen sairaus, jota ei ole hoidettu (ensisijaisen terveydenhuollon lääkärin mukaan), tai nykyinen alkoholi- tai huumeiden väärinkäyttö; tai muiden unihäiriöiden, mukaan lukien vuorokausirytmihäiriöiden (vaiheen edistymis- tai viivästymisoireyhtymät), vuorotyöhön liittyvän unihäiriön ("päiväuniajajat", jotka työskentelevät noin klo 23–7) ja niiden, joilla on kiertävä vuorotyöaikataulu, diagnoosi (tai korkea riski). Yleiset näkökohdat
- Ikä < 18 tai > 80 vuotta

- Vähäinen taito älypuhelimien, tietokoneiden, iPadien tai internetin käytössä.
Naisten terveys Ottaen huomioon ainakin trazodonin (FDA-luokka C) mahdolliset teratogeeniset vaikutukset, naiset, jotka aikovat tulla raskaaksi, tai jotka ovat raskaana, tai jotka imettävät, eivät ole kelpoisia tutkimukseen. Naista pyydetään vahvistamaan ehkäisyn käyttö itseilmoituksella ja, mikäli sovellettavissa, antamalla todiste ehkäisylääkkeestä (esim. resepti tai pillipakkaus). Suoritamme virtsaraskautustestin perustasolla hedelmällisessä iässä oleville naisosallistujille kelpoisuuden vahvistamiseksi. Tutkimukseen osallistuessaan osallistujille kerrotaan, että heidän on ilmoitettava tutkimusryhmälle ja lopetettava tutkimuslääkkeiden käyttö, jos he tulevat raskaaksi. Osallistujia pyydetään testaamaan raskautta, jos kuukautiset myöhästyvät.
Lääketieteelliset ja psyykkiset näkökohdat

As measured by the Karolinska Sleepiness Scale (KSS), a single-item measure of state alertness versus sleepiness, administered twice daily.
The item is a Likert-scale with a range of 1 (extremely alert) to 9 (very sleepy), with lower scores representing higher levels of state alertness and higher scores representing higher levels of state sleepiness.

As measured by a Fitbit, a commercially-available wearable device that utilizes movement detection and heart rate measurements to make sleep/wake assessments every 60 seconds, as well as hourly measures of heart rate, diurnal activity levels, and inferential measures of caloric expenditure.
