상태
상세 설명
일차 진료 환자의 거의 50%가 불면증 증상(예: 수면 시작 및/또는 유지 문제)을 보고합니다. 이러한 수면 관련 어려움은 새로운 동반 질환의 발병을 예고할 수 있을 뿐만 아니라 기존 동반 질환을 악화시키거나 기존 동반 질환에 의해 악화될 수 있습니다. 따라서 일차 진료 환자의 불면증을 효과적으로 치료하는 것은 더 나은 공중보건 증진을 위한 미개척 수단입니다.
연구 질문:
불면증에 대한 인지 행동 치료(CBT-I)가 일차 치료로 간주되지만 대부분의 환자(특히 일차 진료 환자)는 이러한 형태의 치료에 접근할 수 없습니다.

연구 계획서 개요:

- DSM-5 불면증 기준 충족.
- 불면증 심각도 지수(ISI) 점수 ≥15점.
- 수면 시작 및/또는 유지 문제: 지속 시간 ≥30분, 발생 빈도 ≥3일/주, 지속 기간 ≥3개월.
- 등록 전 모든 수면 관련 약물 중단 의지.
- 연구 약물 시작 전 2주간 워시아웃 기간 완료.
- 참여에 대한 임상의 동의 제공 의지.
제외 기준
다음 기준 중 하나라도 충족하는 환자는 참여 불가능합니다: 자가 보고 주간 낮잠(≥1시간/일, ≥3일/주); 자살 시도력 또는 현재 자살 사고, 치료로 조절되지 않는 급성 또는 만성 정신과적 또는 의학적 상태(주치의 판단 기준), 또는 현재 알코올 또는 약물 남용; 또는 기타 수면 장애(일주기 리듬 장애(위상 전진 또는 위상 지연 증후군), 교대 근무 관련 수면 장애(오후 11시~오전 7시 근무 "주간 수면자") 및 교대 근무 스케줄을 가진 자) 진단 또는 고위험군. 적격성 판단을 위해 모든 대상자는 다단계 과정을 통해 선별됩니다: 온라인 선별 설문; 등록 인터뷰; 대상자 EMR 검토; 그리고 최종적으로 환자의 주치의 동의서 수취. 다음은 제외 기준에 대한 추가 목록 및 세부사항(AD)입니다:
일반 고려사항
