早期発症中等度から重度のアルツハイマー病患者とその家族には大きな負担がかかっており、現在病理学的プロセスを逆転させることができる真に効果的な治療法はありません。神経幹細胞由来エクソソーム(NSC-EVs)は、免疫原性が低く、血液脳関門を通過でき、細胞ベースの治療よりも標準化された製造経路を有する可能性があるため、有望な治療アプローチと考えられています。
本研究は、中等度から重度の早期発症アルツハイマー病患者における鼻腔内投与NSC-EVsの安全性、忍容性、および予備的有効性を探索するため、非盲検、単一施設、3群の第I相臨床試験としてデザインされています。この研究は、センチネル参加者モニタリングを伴う3+3頻度漸増デザインを用いて、最高忍容投与頻度を決定し、将来の大規模試験のための予備的臨床データを生成します。
合計9名の参加者が登録される予定です。参加者は逐次的に低頻度、中頻度、高頻度の3つの投与頻度群のいずれかに割り当てられます。全群が同一の治験薬を鼻腔内投与で28日間受けますが、投与頻度のみが異なります。低頻度群は1、4、7、10、13、16、19、22、25、28日に治療を受けます。中頻度群は1、3、5、7、9、11、13、15、17、19、21、23、25、27日に治療を受けます。高頻度群は1日目から28日目まで毎日治療を受けます。
4週間の治療期間後、参加者はフォローアップ期に入り、治療終了後4週、8週、24週に来院が予定されています。治療中およびフォローアップ中の評価には、安全性モニタリング、臨床検査、認知および精神神経学的評価、ならびに該当するプロトコルで定義されたバイオマーカーと画像評価が含まれます。主要目的は安全性と忍容性の評価であり、副次および探索的目的には、認知、行動、機能、およびバイオマーカーの経時変化の予備的評価が含まれます。

説明
- 包含基準:
- 男性または閉経後女性、年齢50〜75歳。
- 2011年NIA-AAの基準を満たすアルツハイマー病認知症の可能性が高い患者。
- 発症年齢≤65歳。
- CMMSスコア5〜20
- 認知機能改善薬または精神科薬物療法を受けている場合、登録前少なくとも2か月間投与量が安定している。
- 小学校卒業以上の学歴があり、研究で必要な認知機能評価を完了できる。
- ハチンスキー虚血スコア≤4。
- GDS合計スコア≤10。
- スクリーニング時の脳MRI+DWI+SWIでプロトコルで定義された脳血管除外基準を満たさず、アルツハイマー病と矛盾する主要な構造異常がない。
- Aβ-PET(スクリーニング時または登録前)でアミロイド病理が陽性であることが確認されている。
- 評価を完了するのに十分な視力と聴力。
- 信頼できる介護者がおり、研究来院に同伴し、評価のための情報を提供できる。
- 参加意思があり、インフォームドコンセントに署名する。
除外基準:
- アルツハイマー病以外の原因による認知症。
- 脳MRIで以下のいずれか:Fazekas白質高信号スコア>2;1.5cmを超えるラクナ梗塞が2つ以上;視床、海馬、嗅内皮質、海馬傍回などの重要な部位を含むラクナ梗塞;脳出血、硬膜下血腫、動脈瘤、動静脈奇形、頭蓋内占拠性病変、またはその他の臨床的に重要な構造異常。
- 幹細胞由来エクソソームまたはPET検査に対するアレルギー。
- 重度の精神障害または症状。
- 重大な活動性身体疾患(重度の心疾患、重度の全身感染症、重度の肝・腎機能障害など)。
- 腫瘍マーカー上昇または腫瘍の既往。
- 免疫関連疾患。
- 著しい鼻閉塞。
- 深刻な自殺リスク。
- 過去6か月以内に別の臨床試験または幹細胞療法に参加。
- 治験責任医師が不適切と判断するその他の状態。
- MRIに対する禁忌または脳MRIの実施が不可能(非MRI対応金属インプラント、特定のステント、プレート、ペースメーカー、重度の閉所恐怖症など)。

Participants receive neural stem cell-derived exosomes by intranasal administration every other day for 28 days.
The single dose is 6 × 10^9 particles administered into both nostrils.
In the medium-frequency group, dosing is scheduled on Days 1, 3, 5, 7, 9, 11, 13, 15, 17, 19, 21, 23, 25, and 27, for a total of 14 doses.
