중증 정신 질환에서 긍정적인 심대사 건강 결과를 생성하기 위한 종단적 접근 방식
심장대사 질환은 정신병적 장애가 있는 개인들 사이에서 널리 퍼져 있으며, 수명 단축, 삶의 질 저하, 개인과 사회에 대한 경제적 영향에 크게 기여합니다. 심장대사 건강을 개선하려면 효과적이고 개별화된 개입이 중요합니다. 정신병 외래 진료소는 정기적인 환자 방문과 다양한 의료 전문가의 이용 가능성으로 인해 이러한 중재에 이상적입니다. 연구자들은 예테보리의 정신과 외래 진료소에서 심장 대사 건강을 개선하고, 삶의 질을 향상시키며, 특히 정신병 장애가 있는 사람들을 위한 건강한 생활 방식을 장려하기 위한 포괄적인 개입 프로그램을 개발했으며 이를 테스트하려고 합니다.
이 임상 시험은 예테보리에 있는 Sahlgrenska 대학 병원의 정신 장애 부서 내 6개 외래 진료소의 정신 장애 환자 644명을 포함하는 것을 목표로 합니다. 두 개의 외래 진료소는 LAGOM 개입을 제공하고, 다른 진료소는 "평상시와 같은 치료"를 제공하는 대조 역할을 할 것입니다. 개입 그룹은 일상적인 임상 절차에 통합된 종합적 지원을 받게 됩니다. 개입에는 정기적인 후속 조치와 체성분 분석기 및 심혈관 위험 예측 알고리즘(QRISK3)을 포함한 동기 부여 도구의 사용이 포함됩니다.

"일반 치료" 모델인 예테보리에서는 정신병적 장애가 있는 개인을 대상으로 연간 건강 검진을 포함합니다. 환자는 혈액 검사, 혈압, 체중 측정과 같은 평가를 위해 60분씩 두 차례 방문하며, 결과는 표준 벤치마크나 SCORE2와 같은 위험 알고리즘을 사용하여 평가됩니다. 의사는 1차 진료 의뢰를 제안하거나 식이, 운동, 물질 사용 조정을 포함한 생활 방식 변화를 권고할 수 있습니다. 확인된 문제는 간단한 조언을 유발하거나 금연, 식이 지도 또는 그룹 활동 지원을 위해 건강 증진자에게 의뢰를 촉진할 수 있습니다. 36 ± 6개월 임상 시험은 치료를 변경하지 않고 4개의 외래 진료소에서 데이터 수집을 표준화합니다. 적격 환자는 동의서에 서명하며, 환자가 한 연간 검진에서 제외 기준을 충족하지만 다음 검진에서는 충족하지 않을 경우 재선별이 허용됩니다. 비참여자는 정기 치료를 계속합니다. |

개입 그룹은 성별과 민족성을 고려하여 심장 대사 프로필을 평가하는 데 중점을 두고 연간 건강 검진을 위한 구조화된 흐름도를 따릅니다. 이 평가에는 SCORE2를 사용하여 전반적인 심대사 위험도와 함께 심대사 매개변수의 변화를 추적하고 대사 증후군 기준이 충족되는지 여부가 포함됩니다. 생활 습관은 건강 상태, 질병, 건강에 해로운 행동을 중단함으로써 얻을 수 있는 이점을 기준으로 평가됩니다. 교육 세션에서는 참가자와 가족에게 정신병 장애, 생활 방식 선택 및 심장 대사 건강 간의 연관성을 교육합니다. 점진적인 생활 방식 변화는 개인의 필요에 맞춰 스트레스와 인지적 문제를 해결하고 맞춤형 조언과 동기 부여 도구를 통해 국가 건강 지침을 따릅니다. 정기적인 후속 조치를 통해 진행 상황을 평가하고, 동기 부여 도구인 "체성분 분석기 및 QRISK3"을 통해 참여도를 높입니다. 내부 및 외부 리소스와의 접촉은 평가 및 동기 부여 작업을 기반으로 합니다. |
