상세 설명
This is a Phase Ib, open-label, dose-escalation study designed to evaluate the safety and tolerability of Pexa-Vec (JX-594), a vaccinia GM-CSF/thymidine kinase-deactivated virus, administered intravenously in patients with advanced/metastatic colorectal carcinoma (CRC) that is refractory to standard therapy. Vaccinia virus, from which Pexa-Vec is derived, shows a natural selectivity toward cancer relative to normal tissues after intravenous (IV) administration.
The study utilizes a sequential dose-escalating design to determine the maximum tolerated dose (MTD) and/or maximum feasible dose (MFD). Patients will receive 4 biweekly treatments of Pexa-Vec administered by IV infusion over 60 minutes on Days 1, 15, 29, and 43. Patients will be sequentially enrolled into one of three dose cohorts:
- Cohort 1: 1 × 10^6 pfu/kg
- Cohort 2: 1 × 10^7 pfu/kg
- Cohort 3: 3 × 10^7 pfu/kg Three patients will be treated at each dose level unless a dose-limiting toxicity (DLT) is observed, at which point the cohort may be expanded. All patients will receive hydration and will be observed in the clinic and/or hospital for a minimum of 24 hours after each infusion.
In addition to safety endpoints, secondary objectives include the evaluation of Pexa-Vec pharmacokinetics (PK), pharmacodynamics, immune response, and preliminary anti-tumoral activity. Tumor response assessments will be conducted using CT or MRI on Days 29 and 57 (±2 days) based on Response Evaluation Criteria in Solid Tumors (RECIST).

설명
포함 기준:
- 조직학적으로 확인된 진행성/전이성 대장암
- 진행성/전이성 질환에 대한 옥살리플라틴 및 이리노테칸 기반 요법 모두 실패(종양이 치료 종료 후 즉시 또는 3개월 이내에 진행된 경우)
- 얼비툭스 내성: Ras 변이가 있거나 얼비툭스 투여에 실패한 환자(종양이 즉시 또는 치료 종료 후 3개월 이내에 진행되었거나 EGFR(표피 성장 요인))
- Karnofsky 성능 점수(KPS) ≥ 70
- 연령 ≥18세
- 검사실 안전: WBC ≥ 3,500 cells/mm3 및 ≤ 50,000 cells/mm3, ANC ≥ 1,500 cells/mm3, 헤모글로빈 ≥ 10 g/dL(수혈 허용), 혈소판 수 ≥ 100,000 plts/mm3, 총 빌리루빈 ≤ 1.5 X ULN, INR ≤ 1.5, AST, ALT ≤ 2.5x ULN (간 전이의 경우: AST,ALT ≤5.0 x ULN)
- 정상 범위(WNL) 또는 1등급(알칼리성 포스파타제 제외) 내의 혈청 화학 - 환자가 당뇨병인 경우 공복 혈당을 실시해야 하며 환자는 > 160 mg/dL이어야 합니다.
- 성적으로 활발한 경우 JX-594 투여 후 3주 동안 성행위를 자제할 의지와 능력이 있는 환자. JX-594의 최종 투여 후 3개월 동안 허용된 피임약을 사용할 의향과 능력이 있는 환자.
제외 기준:
- 기저 질환으로 인한 심각한 면역결핍(예: HIV/AIDS) 및/또는 약물(예: 전신 코르티코 스테로이드)
- 전신 요법을 필요로 하는 알려진 골수증식성 장애
- 벗겨지는 피부 상태의 병력(예: 전신 치료가 필요한 습진 또는 이소성 피부염)
- 기회 감염의 병력.
- 주요 혈관 구조를 침범하는 종양(예: 경동맥)
- 치료 후 종양 종창이 발생할 경우 잠재적으로 심각한 임상적 부작용을 초래할 수 있는 위치의 종양(들)
- 임상적으로 조절되지 않거나 빠르게 축적되는 복수, 심낭 및/또는 흉막삼출
- 심각하거나 불안정한 심장 질환의 병력
- 현재 알려진 CNS 악성 종양(WBRT 또는 정위 방사선 수술에 의해 완전히 절제되거나 조사된 뇌 전이의 병력이 허용됨)
- 첫 치료 전 4주 이내에 항암 요법을 시행한 자(미토마이신 C 또는 니트로소우레아의 경우 6주)
- 항바이러스제, 항혈소판제 또는 항응고제 사용 [첫 치료 전 7일 이내에 이러한 약물을 중단한 환자는 이 연구에 적합할 수 있습니다.] 저용량 아스피린(약 81mg) 허용.
- 맥박 산소 측정기 O2 포화도 <90% 휴식 시 맥박 산소 측정기 O2 포화도 <90%
- 이전 천연두 예방접종으로 인해 심각한 전신 반응 또는 부작용을 경험한 경우

- 피부병이 있는 사람(예: 습진, 아토피성 피부염 및 관련 질환

Anti-tumoral response was evaluated using Response Evaluation Criteria in Solid Tumors (RECIST) on CT or MRI.
Categories include: Complete Response (CR) = Disappearance of all target lesions; Partial Response (PR) = >=30% decrease in the sum of the longest diameters of target lesions; Progressive Disease (PD) = >=20% increase in the sum of the longest diameters of target lesions or appearance of new lesions; Stable Disease (SD) = Neither sufficient shrinkage to qualify for PR nor sufficient increase to qualify for PD.
