- 주요 우울 장애(MDD)는 4주 이상 지속되는 단일 또는 재발성 에피소드와 함께 학생용 최신 버전 및 평생 K-SADS-PL 영어 약어에 대한 정동 장애 및 정신분열증 의제에 기반한 임상 인터뷰를 통해 진단되었습니다.
- 현재 우울 삽화는 지속 기간이 5년 미만이어야 하고 환자가 DSM-V에서 MDD의 작동적 정의를 충족하는 최소 2개월 이상이어야 합니다.
- 임상 평가에서 값이 17 이상인 CID 척도.
- 개입 일 동안 다른 약물을 사용하지 마십시오.
- 그들은 연구 시작 시(지난 4주) 항우울제를 복용하지 않아야 하거나, 5개 이상의 생물학적 반감기로 설정된 항우울제 약물의 "세척"/제거의 적절한 시간 또는 무작위화 5일 전에 경과해야 합니다.
- 부모 또는 튜터와 14-17세 사이의 참여자 모두 정보에 입각한 동의를 할 수 있고 기꺼이 할 수 있습니다. 10-13세 연령 그룹에서는 환자의 동의와 부모 또는 튜터의 동의가 필요합니다.
- 20mg의 디아제팜과 동등하고 연구 동안 안정적으로 유지되는 벤조디아제핀(예: 로라제팜)의 사용.
- 지난 4주 동안 MDD에 대한 변경되지 않은 약물 치료를 받았고 이전에 설명한 항우울제 기준을 충족하는 무작위 배정 시점에 플루옥세틴을 전환하는 데 동의한 환자.
제외 기준:
- 이전 두개골 수술.
- 최근 두개골 손상(6개월).
- 중추 신경계 흥분제 사용.
- 호흡기 감염.
- 정보에 입각한 동의서에 서명하지 않거나 서명할 수 없거나 법적 튜터/부모가 동의하지 않습니다.
- 연구자의 견해에 따라 환자의 안전 또는 데이터 품질을 손상시키는 임상 상태 또는 이상.
- tDCS 자극을 방해할 수 있는 통제되지 않은 공존하는 의학적 상태, 진행성 뇌 장애, 중증 전신 질환, 증후성 질환, 심장 질환, 피부 만성 질환 또는 손상된 두피.
- 모든 두개골 금속 임플란트(치과 충전재 또는 티타늄 플레이트 제외) 또는 의료 기기(심장 박동기, 심부 뇌 자극기, 약물 주입 펌프, 인공 와우, 미주 신경 자극기).
- 연구자의 직계 친척 또는 연구에 직접 참여한 인원.
- Montgomery Asberg 우울증 등급 척도(MADRS) >4점 또는 Columbia Suicide Severity Rating Scale(C-SSRS)의 4번째 또는 5번째 항목의 양성을 통해 측정된 급성 자살 위험.
- 현재 에피소드 동안 >4 잘 지시된 약물 치료에 내성이 있는 우울증.
- 양극성 장애 병력 또는 현재 조증 증상.
- 지난 12개월 동안 전기충격 요법을 동반한 치료.
- DSM 5에 따른 기타 관련 정신 장애(일반화된 불안 장애, 특정 공포증 또는 분리 불안 장애 제외).
- 주요 우울 장애와 다른 질병에 대한 tDCS 이전 치료.
- 이전에 문서화된 지적 발달 장애 또는 CIE-F79 기준을 충족하는 임상 평가.
- tDCS를 사용한 치료에는 반대 적응증이 있기 때문에 비유발성 발작, 경련 장애, 열성 발작의 병력, 간질 또는 뇌종양의 가족력, 임신의 병력이 있는 사람은 제외됩니다.
- 플루옥세틴에 대한 경증에서 중증에 이르는 이상 반응의 반대 징후 또는 이력.
제외 기준:
- 신경심리학적 평가 검사를 완료하지 않은 환자.
- 예정된 것의 50% 미만의 개입 중단.
- 적극적인 자살 생각의 시작.
- 활성 정신병.
- 신흥 조증 치료 시작.
- 임신.

양극 자극은 의도적인 10/20 시스템에 따라 F3에 적용되고 음극은 반대측 위치(F4)에 적용됩니다.

소아 및 청소년의 우울 증상을 평가하기 위해 가장 널리 사용되는 자가 적용 척도 중 하나입니다.
척도는 27문항으로 구성되어 있으며, 각 문항은 3개의 응답 옵션이 있으며, 여기서 0=증상 없음, 1=중등도 증상, 2=심각한 증상입니다.
총 점수 범위는 0-54입니다.
척도 매뉴얼에는 저자가 보고한 심리적 특성이 포함되어 있습니다.
7-17세 어린이에게 사용을 권장합니다.

11개 항목으로 구성된 조증 증상을 정량화하기 위한 도구이며, 각 항목에는 5개의 응답 옵션이 있으며 강도가 높을수록 더 높은 점수를 반영합니다. 임상의가 적용하며 평가 전 48시간 동안의 환자의 주관적 보고서와 면담 중 환자의 행동 관찰을 기반으로 합니다. 환자는 0에서 4까지의 옵션으로 각 항목에 대한 증상의 강도 수준을 선택합니다. 점수가 높을수록 조증 특성의 정도가 더 큰 것을 나타내는 0-60점의 척도의 전체 범위; 새로 발병한 조증 또는 경조증을 정의하기 위한 최소 점수는 임상 시험 동안 조증 또는 경조증을 평가하기 위해 2주, 4주, 8주 및 12주 말에 8점입니다. |
