Descrição
Critério de inclusão:
- Transtorno Depressivo Maior (TDM) diagnosticado por meio de entrevista clínica com base na Agenda de Transtornos Afetivos e Esquizofrenia para escolares-versão atual e para a vida K-SADS-PL por sua sigla em inglês, com um episódio único ou recorrente com duração ≥4 semanas.
- O episódio depressivo atual deve ter duração inferior a 5 anos e pelo menos superior a 2 meses em que o paciente atende à definição operacional de TDM do DSM-V.
- Escala CID com valores iguais ou superiores a 17 na avaliação clínica.
- Sem uso de outra medicação durante os dias de intervenção.
- Eles não deveriam estar tomando antidepressivos no início do estudo (últimas 4 semanas) ou deve ter passado o tempo adequado de "lavagem"/depuração de drogas antidepressivas estabelecido como 5 ou mais meias-vidas biológicas ou 5 dias antes da randomização.
- Para poder e querer dar o seu consentimento informado, ambos, o pai ou tutor e o participante com idades compreendidas entre os 14 e os 17 anos. Na faixa etária de 10 a 13 anos, o consentimento do paciente e o consentimento do pai ou responsável.
- Uso de benzodiazepínicos (por exemplo, lorazepam) equivalente a 20 mg de diazepam e que se mantém estável durante o estudo.
- Pacientes com tratamento medicamentoso inalterado para MDD nas últimas 4 semanas e que concordam em trocar a fluoxetina no momento da randomização, atendendo aos critérios antidepressivos descritos anteriormente.
Critério de exclusão:
- Cirurgia de crânio anterior.
- Lesão craniana recente (6 meses).
- Uso de estimulantes do sistema nervoso central.
- Infecções do Sistema Respiratório.
- NÃO assina ou é incapaz de assinar o consentimento informado ou tutores/pais legais não consentem.
- Condição clínica ou anomalia que, na opinião do pesquisador, comprometa a segurança do paciente ou a qualidade dos dados.
- Condição médica coexistente descontrolada, distúrbio cerebral progressivo, doenças sistêmicas graves, doença sintomática, doença cardíaca, doenças crônicas da pele ou couro cabeludo danificado que podem interferir na estimulação do tDCS.
- Qualquer implante de metal craniano (excluindo obturação dentária ou placas de titânio) ou dispositivos médicos (marca-passo cardíaco, estimulador cerebral profundo, bomba de infusão de drogas, implante coclear, estimulador do nervo vago).
- Parentes diretos do pesquisador ou pessoal diretamente envolvido no estudo.
- Risco de suicídio agudo medido através da escala de classificação de depressão de Montgomery Asberg (MADRS) > 4 pontos ou positividade do 4º ou 5º item da Escala de Classificação de Gravidade do Suicídio de Columbia (C-SSRS).
- Depressão resistente ao tratamento com >4 medicamentos bem indicados durante o episódio atual.
- Histórico de transtorno bipolar ou sintomas de mania atuais.
- Tratamento com terapia de eletrochoque concomitantemente e durante os últimos 12 meses.
- Outros transtornos psiquiátricos relevantes de acordo com o DSM 5 (exceto transtorno de ansiedade generalizada, fobia específica ou transtorno de ansiedade de separação).
- Tratamento anterior com tDCS para uma doença diferente do transtorno depressivo maior.
- Transtorno do desenvolvimento intelectual previamente documentado ou avaliação clínica preenchendo os critérios CIE-F79.
- Pelo fato de o tratamento com ETCC ter contra-indicações, serão excluídos: aqueles com histórico de convulsões não provocadas, distúrbios convulsivos, histórico de convulsão febril, histórico familiar de epilepsia ou tumor cerebral, gravidez.
- Contra-indicação ou histórico de reações adversas de leve a grave à fluoxetina.
Critérios de eliminação:
- Pacientes que não concluem os testes de avaliação neuropsicológica.
- Interrupção da intervenção em menos de 50% do previsto.
- Início da ideação suicida ativa.
- Psicose ativa.
- Início do tratamento da mania emergente.
- Gravidez.

Experimental: braço tDCS
Os participantes recebem 10 sessões consecutivas seguidas de 1 sessão por semana 10 semanas de tDCS (30 minutos e 2 mA) + fluoxetina (10 mg diariamente 2 semanas seguido de 20 mg diariamente).

Os participantes recebem 10 sessões consecutivas seguidas de 1 sessão por semana, 10 semanas de tDCS (30 minutos e 2 mA).
A estimulação anódica será aplicada em F3 de acordo com o sistema intencional 10/20, e o cátodo será aplicado na posição contralateral (F4).

Cada paciente receberá 10 mg de fluoxetina diariamente nas primeiras 2 duas semanas, seguido de 20 mg diariamente.

Comparador de Placebo: Braço de controle
O participante recebe 10 sessões consecutivas, seguidas de 1 sessão por semana, 10 semanas de placebo (30 minutos de simulação de placebo tDCS) + fluoxetina (10 mg por dia 2 semanas, seguido de 20 mg por dia).

Cada paciente receberá 10 mg de fluoxetina diariamente nas primeiras 2 duas semanas, seguido de 20 mg diariamente.

O placebo será administrado usando os mesmos parâmetros de estimulação e a posição de tratamento ativa dos eletrodos, mas a corrente será interrompida 30 segundos após o início da inclinação ascendente.

É uma das escalas autoaplicadas mais utilizadas para avaliar sintomas depressivos em crianças e adolescentes.
A escala é composta por 27 itens, cada item com 3 opções de resposta, onde 0= ausência de sintomas, 1= sintomas moderados e 2= sintomas graves.
A pontuação total varia de 0-54.
O manual da escala contém as propriedades psicométricas relatadas pelo autor.
É recomendado para uso em crianças de 7 a 17 anos.

O C-SSRS é uma entrevista semiestruturada que coleta o início, a gravidade e a frequência de comportamentos e pensamentos relacionados ao suicídio durante o período de avaliação.

Alteração da pontuação da Young Mania Rating Scale do início ao fim do estudo

É um instrumento para quantificar os sintomas de mania que consiste em 11 itens, cada um com 5 opções de resposta, que refletem maior pontuação se for de grande intensidade. É aplicado pelo clínico e baseia-se no relato subjetivo do paciente durante 48 horas antes da avaliação, bem como na observação do comportamento do paciente durante a entrevista. O paciente seleciona o nível de intensidade do sintoma para cada item com uma opção de 0 a 4. O intervalo total da escala de 0 a 60 pontos, onde pontuações mais altas indicam um maior grau de características maníacas; a pontuação mínima para definir mania ou hipomania de início recente é de 8 pontos no final das semanas 2, 4, 8 e 12 para avaliar mania ou hipomania durante o ensaio clínico. |
