Descripción
Criterios de inclusión:
- Trastorno Depresivo Mayor (TDM) diagnosticado a través de la entrevista clínica basada en la Agenda de Trastornos Afectivos y Esquizofrenia para escolares-versión actual y de por vida K-SADS-PL por sus siglas en inglés, con episodio único o recurrente de duración ≥4 semanas.
- El episodio depresivo actual debe tener una duración inferior a 5 años y ser al menos superior a 2 meses en los que el paciente cumpla con la definición operativa de TDM del DSM-V.
- Escala CID con valores iguales o superiores a 17 en la evaluación clínica.
- No uso de otro medicamento durante los días de intervención.
- No deben estar tomando antidepresivos al inicio del estudio (últimas 4 semanas) o haber pasado el tiempo adecuado de “lavado”/depuración de antidepresivos establecido como 5 o más semividas biológicas o 5 días antes de la aleatorización.
- Poder y estar dispuesto a dar su consentimiento informado, tanto el padre o tutor como el participante que tenga entre 14 y 17 años. En el grupo de edad de 10 a 13 años el asentimiento del paciente y el consentimiento del padre o tutor.
- Uso de benzodiazepinas (ej. lorazepam) equivalente a 20 mg de diazepam y que se mantenga estable durante el estudio.
- Pacientes con un tratamiento farmacológico sin cambios para MDD en las últimas 4 semanas y que aceptan cambiar la fluoxetina en el momento de la aleatorización, cumpliendo los criterios antidepresivos descritos anteriormente.
Criterio de exclusión:
- Cirugía de cráneo previa.
- Lesión craneal reciente (6 meses).
- Uso de estimulantes del sistema nervioso central.
- Infecciones del Sistema Respiratorio.
- NO firma o no puede firmar el consentimiento informado o los tutores/padres legales no dan su consentimiento.
- Condición clínica o anomalía, que a juicio del investigador comprometa la seguridad del paciente o la calidad de los datos.
- Condición médica coexistente no controlada, trastorno cerebral progresivo, enfermedades sistémicas graves, enfermedad sintomática, enfermedad cardíaca, enfermedades crónicas de la piel o cuero cabelludo dañado que podría interferir con la estimulación de tDCS.
- Cualquier implante de metal en el cráneo (excluyendo empastes dentales o placas de titanio) o dispositivos médicos (marcapasos cardíaco, estimulador cerebral profundo, bomba de infusión de medicamentos, implante coclear, estimulador del nervio vago).
- Familiares directos del investigador o personal directamente involucrado en el estudio.
- Riesgo de suicidio agudo medido a través de la escala de valoración de la depresión de Montgomery Asberg (MADRS) > 4 puntos o positividad del 4.º o 5.º ítem de la escala de valoración de la gravedad del suicidio de Columbia (C-SSRS).
- Depresión resistente al tratamiento con >4 fármacos bien indicados durante el episodio actual.
- Antecedentes de trastorno bipolar o síntomas de manía actuales.
- Tratamiento con terapia de electroshock concomitantemente y durante los últimos 12 meses.
- Otros trastornos psiquiátricos relevantes según el DSM 5 (excepto trastorno de ansiedad generalizada, fobia específica o trastorno de ansiedad por separación).
- Tratamiento previo con tDCS por una enfermedad diferente al trastorno depresivo mayor.
- Trastorno del desarrollo intelectual previamente documentado o evaluación clínica cumpliendo los criterios CIE-F79.
- Debido a que el tratamiento con tDCS tiene contraindicaciones, serán excluidos: aquellos con antecedentes de convulsiones no provocadas, trastornos convulsivos, antecedentes de convulsiones febriles, antecedentes familiares de epilepsia o tumor cerebral, embarazo.
- Contraindicación o antecedentes de reacciones adversas de leves a graves a la fluoxetina.
Criterios de eliminación:
- Pacientes que no completen las pruebas de evaluación neuropsicológica.
- Interrupción de la intervención de menos del 50% de lo programado.
- Inicio de la ideación suicida activa.
- Psicosis activa.
- Inicio del tratamiento de la manía emergente.
- El embarazo.

Los participantes reciben 10 sesiones consecutivas seguidas de 1 sesión por semana 10 semanas de tDCS (30 minutos y 2 mA) + fluoxetina (10 mg diarios 2 semanas seguidas de 20 mg diarios).

Los participantes reciben 10 sesiones consecutivas seguidas de 1 sesión por semana 10 semanas de tDCS (30 minutos y 2 mA).
La estimulación anódica se aplicará en F3 según el sistema intencionado 10/20, y el cátodo se aplicará en la posición contralateral (F4).

Comparador de placebos: Brazo de control
El participante recibe 10 sesiones consecutivas seguidas de 1 sesión por semana 10 semanas de placebo (30 minutos de tDCS de simulación de placebo) + fluoxetina (10 mg diarios 2 semanas seguidas de 20 mg diarios).

Cada paciente recibirá 10 mg de fluoxetina al día las 2 primeras dos semanas, seguido de 20 mg al día.

El placebo se administrará utilizando los mismos parámetros de estimulación y la posición de tratamiento activo de los electrodos, pero la corriente se cancelará 30 segundos después de que haya comenzado la pendiente ascendente.

Es una de las escalas autoaplicadas más utilizadas para evaluar síntomas depresivos en niños y adolescentes.
La escala consta de 27 ítems, cada ítem con 3 opciones de respuesta, donde 0= ausencia de síntomas, 1= síntomas moderados y 2= síntomas severos.
La puntuación total oscila entre 0 y 54.
El manual de la escala contiene las propiedades psicométricas reportadas por el autor.
Se recomienda su uso en niños de 7 a 17 años.
Se estableció una PUNTUACIÓN DE 20 como punto de corte para indicar problemas de depresión.

La C-SSRS es una entrevista semiestructurada que recoge el inicio, la gravedad y la frecuencia de las conductas y pensamientos relacionados con el suicidio durante el periodo de evaluación.
Por lo tanto, se miden 4 constructos.

Es un instrumento para cuantificar los síntomas de la manía que consta de 11 ítems, cada uno con 5 opciones de respuesta, que reflejan mayor puntuación si mayor intensidad. Es aplicado por el clínico y se basa en el reporte subjetivo del paciente durante las 48 hrs previas a la evaluación, así como en la observación del comportamiento del paciente durante la entrevista. El paciente selecciona el nivel de intensidad del síntoma para cada elemento con una opción de 0 a 4. El rango total de la escala de 0 a 60 puntos donde las puntuaciones más altas indican un mayor grado de características maníacas; la puntuación mínima para definir manía o hipomanía de nueva aparición es de 8 puntos al final de las semanas 2, 4, 8 y 12 para evaluar manía o hipomanía durante el ensayo clínico. |
