Bevételi kritériumok:
- A klinikai interjú során diagnosztizált Major Depressive Disorder (MDD) az Affective Disorders and Schizophrenia Agenda iskolásoknak jelenlegi változata és az élethosszig tartó K-SADS-PL angol rövidítése alapján, egyetlen vagy visszatérő epizóddal, amely 4 hétnél hosszabb ideig tart.
- A jelenlegi depressziós epizódnak 5 évnél rövidebbnek kell lennie, és legalább 2 hónapnál hosszabbnak kell lennie, amelyben a beteg megfelel az MDD működési meghatározásának a DSM-V-ből.
- CID skála 17 vagy magasabb értékekkel a klinikai értékelésben.
- A beavatkozási napokon más gyógyszert tilos használni.
- A vizsgálat kezdetén (az utolsó 4 hétben) nem szedhetnek antidepresszánsokat, vagy az antidepresszáns gyógyszerek „lemosásának”/kiürülésének megfelelő, 5 vagy több biológiai felezési időként megállapított, illetve a randomizálás előtti 5 napon túli időszakon kell túlmenniük.
- Legyen képes és hajlandó tájékozott beleegyezését megadni, mind a szülő, mind az oktató, mind a 14-17 év közötti résztvevő. A 10-13 éves korosztályban a beteg beleegyezése és a szülő, oktató beleegyezése.
- 20 mg diazepammal egyenértékű benzodiazepinek (pl. lorazepám) alkalmazása, amely stabil marad a vizsgálat során.
- Azok a betegek, akiknél az elmúlt 4 hétben nem változott az MDD gyógyszeres kezelése, és akik beleegyeztek a fluoxetin váltásba a randomizáció időpontjában, megfelelve a korábban leírt antidepresszáns kritériumoknak.
Kizárási kritériumok:
- Korábbi koponyaműtét.
- Legutóbbi koponyasérülés (6 hónap).
- Központi idegrendszeri stimulánsok alkalmazása.
- A légzőrendszer fertőzései.
- NEM írja alá, vagy nem tudja aláírni a tájékozott beleegyezést, illetve a jogi oktatók/szülők nem járulnak hozzá.
- Klinikai állapot vagy anomália, amely a kutató véleménye szerint veszélyezteti a beteg biztonságát vagy az adatok minőségét.
- Ellenőrizetlen egyidejűleg fennálló egészségügyi állapot, progresszív agyi rendellenesség, súlyos szisztémás betegségek, tünetekkel járó betegség, szívbetegség, krónikus bőrbetegségek vagy sérült fejbőr, amelyek megzavarhatják a tDCS-stimulációt.
- Bármilyen koponya-fém implantátum (kivéve a fogtömést vagy titánlemezeket) vagy orvosi eszközöket (szívritmus-szabályozó, mélyagyi stimulátor, gyógyszerinfúziós pumpa, cochleáris implantátum, vagus idegstimulátor).
- Közvetlen kutatói rokonok vagy a vizsgálatban közvetlenül részt vevő személyzet.
- Az akut öngyilkossági kockázat a Montgomery Asberg depressziós besorolási skála (MADRS) alapján mérve, >4 pont, vagy a Columbia Suicide Severity Rating Scale (C-SSRS) 4. vagy 5. tételének pozitivitása.
- A jelenlegi epizód alatt több mint 4 jól jelzett gyógyszerrel szembeni kezelésre rezisztens depresszió.
- Bipoláris zavar kórtörténet vagy jelenlegi mánia tünetei.
- Elektrosokk terápia egyidejűleg és az elmúlt 12 hónapban.
- Egyéb releváns pszichiátriai rendellenességek a DSM 5 szerint (kivéve a generalizált szorongásos zavart, a specifikus fóbiát vagy a szeparációs szorongásos zavart).
- Korábbi tDCS-kezelés a major depressziós rendellenességtől eltérő betegség miatt.
- Korábban dokumentált értelmi fejlődési rendellenesség vagy klinikai értékelés, amely megfelel a CIE-F79 kritériumoknak.
- Tekintettel arra, hogy a tDCS-kezelésnek ellenjavallatai vannak, kizárásra kerülnek: azok, akiknek anamnézisében nem provokált görcsrohamok, görcsös rendellenességek, lázas rohamok szerepeltek, a családban előfordult epilepszia vagy agydaganat, terhesség.
- Ellenjavallatok vagy a kórtörténetben előforduló mellékhatások az enyhétől a súlyosig a fluoxetinig.
Kizárási kritériumok:
- Betegek, akik nem töltik ki a neuropszichológiai értékelő teszteket.
- A beavatkozás megszakítása a tervezettnek kevesebb mint 50%-ával.
- Aktív öngyilkossági gondolatok megjelenése.
- Aktív pszichózis.
- A kialakuló mánia kezelésének kezdete.
- Terhesség.

A résztvevők 10 egymást követő ülést kapnak, amelyet heti 1 alkalom követ, 10 héten át tDCS (30 perc és 2 mA) + fluoxetin (napi 10 mg 2 héten keresztül, majd napi 20 mg).

Az F3-on anódos stimulációt alkalmazunk a szándékos 10/20-as rendszer szerint, a katódot pedig az ellenoldali pozícióban (F4).

Placebo Comparator: Irányító kar
A résztvevő 10 egymást követő kezelésben részesül, majd heti 1 alkalommal 10 héten át placebót (30 perc placebo-szimulációs tDCS) + fluoxetint (napi 10 mg 2 héten át, majd napi 20 mg).

Minden beteg napi 10 mg fluoxetint kap az első két héten, majd napi 20 mg-ot.

Ez az egyik legszélesebb körben használt, önállóan alkalmazott skála gyermekek és tinédzserek depressziós tüneteinek felmérésére.
A skála 27 tételből áll, mindegyik 3 válaszlehetőséggel, ahol 0 = a tünetek hiánya, 1 = közepes tünetek és 2 = súlyos tünetek.
Az összpontszám 0 és 54 között mozog.
A skála kézikönyv tartalmazza a szerző által közölt pszichometriai tulajdonságokat.
Használata 7-17 éves gyermekek számára javasolt.

A C-SSRS egy félig strukturált interjú, amely összegyűjti az öngyilkossággal kapcsolatos viselkedés és gondolatok kezdetét, súlyosságát és gyakoriságát az értékelési időszakban.

Ez egy eszköz a mánia tüneteinek számszerűsítésére, amely 11 elemből áll, mindegyik 5 válaszlehetőséget tartalmaz, amelyek nagyobb intenzitás esetén magasabb pontszámot tükröznek. A klinikus alkalmazza, és az értékelést megelőző 48 órában a páciens szubjektív beszámolóján, valamint a páciens interjú során tanúsított viselkedésén alapul. A páciens minden elemhez kiválasztja a tünet intenzitási szintjét egy 0 és 4 közötti opcióval.
