상세 설명
뇌졸중 후 흔히 발생하는 장애인 경직은 환자의 활동과 참여에 지대한 영향을 미칩니다. 재활 훈련과 결합된 BoNT-A 주사는 경련성 편마비 뇌졸중 환자의 기능 회복을 향상시키는 데 권장됩니다.
경직이 있는 환자는 일반적으로 경직이 없는 환자보다 운동 기능이 낮고 감각 결손이 더 심합니다. 포스트 BoNT-A 주사 재활 프로그램을 설계할 때 위의 문제를 고려해야 합니다. RT와 MT는 운동 기능이 낮은 환자에게 감각 운동 입력을 제공하는 두 가지 개입입니다. 두 가지 접근 방식(거울 치료를 위해 로봇 손을 착용)을 결합하면 움직임을 제어하는 감각 운동 피질을 촉진하고 체성 감각 입력 및 추가 치료 효능을 증가시킬 수 있습니다. BoNT 주사를 양측 RT 대 MT 대 RMT와 결합하는 것이 경련성 편마비 뇌졸중 환자의 운동 및 관련 기능 수행에 차등 효과를 발생시키는지 여부와 그 방법은 알려져 있지 않습니다.
만성 경련성 편마비 뇌졸중이 있는 48명의 참가자가 모집되어 RT 및 RMT의 두 그룹 중 하나에 무작위로 할당됩니다. 모든 주사 후 개입은 8주 동안 60분/일, 3일/주로 시행될 것입니다. 각 교육 세션에서 모든 환자는 45분 RT 또는 RMT를 받은 후 15분 기능적 작업 교육을 받습니다.
결과 측정은 1) 신체 기능 및 구조: Fugl-Meyer Assessment, Modified Ashworth Scale, Medical Research Council scale, 2) 활동 및 참여: Box and block test, Motor Activity Log, Nottingham Extended Activities of Daily Living Scale, Action 연구 팔 테스트. 또한 환자의 고유한 요구 사항과 목표를 직접 반영하기 위해 목표 달성 척도를 평가합니다. 뇌파 검사(EEG) 및 기능적 근적외선 반사 분광법(NIRS) 평가는 3가지 개입의 신경생리학적 효과를 평가하기 위해 수행됩니다. 결과는 치료 전, BoNT-A 주입 1주 후, 치료 후 및 3개월 추적에서 측정됩니다.

포함 기준:
- 첫 번째 또는 재발성 일측성 뇌졸중 ≥ 3개월의 임상 및 상상 진단
- 손가락 굴근 근육 경직(수정된 Ashworth 척도 ≥ 1+)
- 중등도에서 중증의 운동 장애를 나타내는 10~56 범위의 FMA 점수 UE의 초기 운동 부분
- 심각한 인지 장애 없음(즉, Mini Mental State Exam 점수 > 20)

- 양측 반구 또는 소뇌 병변이 있는 경우
- 절단 실어증
- 상당한 시야 결손 또는 편측반사

이 연구에서는 웨어러블 로봇 손 시스템이 사용됩니다.
로봇 손 시스템은 웨어러블 외골격 손, 센서 장갑 및 컨트롤 박스로 구성됩니다.
외골격 손에는 개별 손가락을 움직일 수 있도록 외부 전원을 제공할 수 있는 각 손가락 구조에 5개의 액추에이터가 있습니다.
센서 장갑에는 이동 중 손가락의 자세를 감지할 수 있는 5개의 센서가 있으며 컨트롤 박스를 통해 외골격 손을 조작할 수 있습니다.
환자의 정상 손은 센서 장갑을 착용하고, 환부는 웨어러블 외골격 손을 착용하고, 정상 손은 미러 그룹으로서 특정 이행 및 자동 작업을 수행한 다음 외골격 로봇 손이 구동하는 것과 동일한 동작을 수행하게 합니다. .

마비 UE의 손재주를 평가하는 BBT는 두 개의 동일한 크기의 구획이 있는 나무 상자를 사용합니다.
큐브를 한 구획에 놓고 참가자에게 60초 이내에 가능한 한 빨리 큐브를 하나씩 다른 구획으로 옮기도록 지시했습니다.
점수는 파티션을 가로질러 운반되는 큐브의 수를 계산하여 결정되었습니다.
BBT는 뇌졸중 환자에서 검사-재검사 신뢰도가 높다(Thomas Platz et al., 2005).

NIRS는 시간 순차 실시간 모니터링 동안 뇌의 미세혈관에서 산소헤모글로빈(HbO2) 및 탈산소헤모글로빈(HbR) 농도의 변화를 측정할 수 있습니다.
