이 프로젝트는 제1형 당뇨병을 앓고 있는 젊은 성인의 불안 및 우울 증상과 혈당 조절을 목표로 하기 위해 지속적인 포도당 모니터(CGM) 검토로 강화된 인지 행동 치료를 위한 원격 의료 제공 통합 프로토콜(UP-CBT)을 평가할 것입니다.

상세 설명
상업적 FDA 승인 연속 포도당 모니터링(CGM)과 결합된 인지 행동 치료를 위한 통합 프로토콜(UP-CBT)의 효능은 무작위 대조 임상 시험에서만 CGM과 비교하여 테스트됩니다.
중심 가설은 UP-CBT 중재를 추가하면 CGM 단독에 비해 불안 및 우울 증상의 중증도와 혈당 조절이 임상적으로 유의미하게 개선될 것이라는 것입니다.
우리는 인종/민족 소수자를 대상으로 모집하여 5년에 걸쳐 완전히 가상의 12개월 연구를 위해 전국 인구에서 차선으로 통제된 제1형 당뇨병과 불안 또는 우울 장애가 있는 150명의 청년(18-35세)을 모집할 것입니다.

- 혈당 조절에 영향을 미칠 약물 또는 최근 의료 절차의 사용 또는 연구에 대한 지속적인 포도당 모니터링(CGM) 사용
- 작년에 인지 행동 치료(CBT)를 받았거나 CBT를 시작할 계획입니다. (6) 최근 향정신성 약물 치료 시작을 위한 일시적 제외 - 등록 전 6주 동안 안정적인 용량을 유지해야 합니다.

이 부문에 무작위로 배정된 참가자는 지속적인 포도당 모니터링(CGM) 데이터 검토를 통해 강화된 인지 행동 치료를 위한 통합 프로토콜(UP-CBT)을 받게 됩니다.
참가자는 실험 참여 첫 6개월 동안 연구에서 제공한 CGM을 착용하게 됩니다.

UP-CBT는 약 20주 동안 진행되는 약 16개의 개별 CBT 세션으로 구성됩니다.
UP-CBT는 부정적인 감정과 감정적 경험에 대한 혐오 반응을 대상으로 하는 5개의 핵심 모듈로 구성됩니다.
이러한 모듈은 환자의 현재 증상을 검토하고 치료적 근거를 제공하는 소개 세션과 동기 부여 향상에 대한 모듈이 선행됩니다.
마지막 모듈은 재발 방지로 구성됩니다.
UP-CBT 세션은 지속적인 포도당 모니터링(CGM) 데이터 검토를 통합하고 치료사가 피드백을 제공합니다.
