배경: 간 전이(CLM)는 대장암 환자의 약 절반에 영향을 미치며 환자의 예후를 좌우합니다. 예후 예측은 환자를 가장 적절한 치료에 배정하는 데 가장 중요하지만 이용 가능한 매개변수는 이 역할을 적절하게 수행하지 못합니다. 종양 병리학 및 분자 데이터 및 간-종양 경계면 특성은 주요 예후 영향을 나타냈지만 표준 예후 점수에는 포함되지 않았으며 표준 영상 기법은 이에 대한 정보가 부족합니다. 방사성 분석은 병리학 데이터와 여러 종양의 환자 결과에 대한 매우 좋은 예측을 보여 주었지만 CLM에 대한 적용은 아직 탐구해야 할 과제로 남아 있습니다.
가설 수술 전 CLM과 간-종양 경계면 특성을 확인하면 예후 예측과 환자의 치료 할당이 향상될 것입니다. 다른 종양과 마찬가지로 방사선 분석을 통해 병리학 데이터 예측을 크게 개선할 수 있습니다. 방사성 특징 자체는 예후와 주요 연관성을 가질 수 있습니다.
목표
이 연구에는 다음과 같은 종말점이 있습니다.
- 종양 및 간-종양 경계면의 방사성 특징이 표준 예후 점수와 비교하여 간 수술을 받는 CLM 환자의 예후 예측을 향상시키는지 여부를 평가합니다.
- 방사성 특징이 병리학 데이터와 연관되어 있는지 탐색합니다.
- 화학요법에 대한 종양 반응을 평가하기 위해 표준 방사선학적 기준과 비교하여 방사선학적 특징의 성능을 탐색합니다.
- 단일 예후 점수에 방사성 및 상세한 병리학 데이터를 병합합니다.
실험 설계 이 연구는 저자 기관에서 간 절제술을 받은 환자의 후향적(n=300 환자)과 전향적(n=400) 일련의 환자를 결합할 것입니다. 후향적으로 수집된 환자는 표준 예후 인자와 방사능 특징을 포함하는 예후 모델에 대한 훈련 데이터 세트를 대표하는 반면, 전향적 코호트(n=200)의 전반부는 검증 데이터 세트(최소 추적 기간 30개월)가 됩니다. 방사성 특징과 병리학 세부 사항 및 화학 요법에 대한 종양 반응의 연관성을 분석하기 위해 환자의 전향적 코호트(n=400, ≥800 CLM)가 훈련 및 검증 데이터 세트로 사용됩니다(간 종양 인터페이스에 대한 데이터는 안정적으로 평가할 수 없음) 회고 시리즈에서). 마지막으로, 적절한 추적 관찰을 통해 전향적으로 수집된 모든 환자는 방사성 특징과 상세한 병리학 데이터를 결합한 종합 예후 점수를 구축하는 데 기여할 것입니다. 환자별 평가는 예후 분석에서 수행됩니다. 방사선 데이터와 병리학 데이터 사이의 연관성을 평가하는 동안 병변별 평가가 수행됩니다.
