Radiomics van colorectale levermetastasen: identificatie van nieuwe prognostische biomarkers.
Radiomische kenmerken van tumor en van lever-tumorinterface bij patiënten met colorectale levermetastasen. Identificatie van nieuwe prognostische biomarkers.
Achtergrond: Levermetastasen (CLM) treffen ongeveer de helft van de patiënten met colorectale kanker en bepalen de prognose van de patiënt. Het voorspellen van de prognose is van het allergrootste belang voor de toewijzing van patiënten aan de meest adequate behandeling, maar de beschikbare parameters vervullen deze rol niet adequaat. Tumorpathologie en moleculaire gegevens en kenmerken van het lever-tumor-interface vertoonden een grote prognostische impact, maar ze zijn niet opgenomen in de standaard prognostische scores en standaard beeldvormingsmodaliteiten geven er weinig informatie over. Radiomische analyses toonden een zeer goede voorspelling aan van pathologiegegevens en van de uitkomst van patiënten bij verschillende tumoren, maar de toepassing ervan op CLM moet nog worden onderzocht.
Hypothese De preoperatieve identificatie van CLM en kenmerken van het lever-tumor-interface zou de voorspelling van de prognose en de toewijzing van patiënten aan behandelingen verbeteren. Net als bij andere tumoren zouden radiomische analyses een grote verfijning in de voorspelling van pathologiegegevens mogelijk kunnen maken. Radiomische kenmerken op zich zouden een belangrijke associatie kunnen hebben met de prognose.
Doelstellingen
Het onderzoek heeft de volgende eindpunten:
- om te beoordelen of radiomische kenmerken van de tumor en van het grensvlak tussen lever en tumor de voorspelling van de prognose verbeteren bij CLM-patiënten die een leveroperatie ondergaan in vergelijking met standaard prognostische scores.
- om te onderzoeken of radiomische kenmerken verband houden met pathologiegegevens.
- om de prestaties van radiomische kenmerken te onderzoeken in vergelijking met standaard radiologische criteria om de tumorrespons op chemotherapie te beoordelen.
- om radiomische en gedetailleerde pathologiegegevens samen te voegen in één enkele prognostische score.
Experimenteel ontwerp De studie zal een retrospectieve (n=300 patiënten) en een prospectieve (n=400) reeks patiënten combineren die leverresectie ondergaan in de instelling van de auteur. Retrospectief verzamelde patiënten zullen de trainingsdataset voor het prognostische model vertegenwoordigen, inclusief standaard prognostische factoren plus radiomische kenmerken, terwijl de eerste helft van het prospectieve cohort (n=200) de validatiedataset zal zijn (minimale follow-up 30 maanden). Voor de analyse van de associatie van radiomische kenmerken met pathologische details en tumorrespons op chemotherapie zal het prospectieve cohort van patiënten (n=400, ≈800 CLM's) worden gebruikt als trainings- en validatiedataset (gegevens over lever-tumorinterface kunnen niet betrouwbaar worden beoordeeld in de retrospectievereeks). Ten slotte zullen alle prospectief verzamelde patiënten met adequate follow-up bijdragen aan het opbouwen van een samengestelde prognostische score die radiomische kenmerken en gedetailleerde pathologiegegevens combineert. Evaluatie per patiënt zal worden uitgevoerd in prognostische analyses; Er zal een evaluatie per laesie worden uitgevoerd, waarbij de associatie tussen radiomische en pathologische gegevens wordt geëvalueerd. De LifeX®-software zal worden gebruikt om radiomische analyses uit te voeren. Het interessevolume (VOI) van de tumor zal worden gevolgd. Er zal een automatische volume-expansie worden toegepast op de VOI van de tumor om het grensvlak tussen lever en tumor te volgen (expansie van 5 mm).
Verwachte resultaten De huidige studie heeft de solide verwachting om aan te tonen dat radiomische kenmerken van CLM en van het lever-tumor-interface een belangrijke prognostische rol spelen en een goede associatie hebben met pathologische gegevens. Wij zijn verder van mening dat een prognostische score die radiomische en pathologische gegevens combineert, de prognosevoorspelling verder kan optimaliseren.
Impact op kanker Onze analyse heeft tot doel de voorspelling van de CLM-prognose te verbeteren door radiomische kenmerken te identificeren die van invloed zijn op de prognose en pathologiegegevens te voorspellen, en een gecombineerd prognostisch model van radiomische en pathologische gegevens voor te stellen. Deze vormen de basis voor een precisiegeneeskunde gebaseerd op een preoperatieve, prognostisch gestuurde behandelingsallocatie.

Inclusiecriteria:
- Patiënten die een leveroperatie ondergaan vanwege CLM, bevestigd bij definitieve pathologie
- Minimaal 1 CLM met diameter >10 mm
- Preoperatieve CT-beeldvorming beschikbaar voor radiomische analyse.
- Leeftijd >18 jaar
- Geen andere maligniteiten in de afgelopen 5 jaar
- Interval CT-operatie ≤60 dagen
Uitsluitingscriteria:
- Locoregionale behandelingen van CRLM vóór leverresectie
- Ontoereikende portaalfase van de CT of materiële artefacten met hoge dichtheid die de analyse beïnvloeden
- Onvolledige klinische gegevens

Om te onderzoeken of radiomische kenmerken van de tumor en van het grensvlak tussen lever en tumor bij patiënten met CLM geassocieerd zijn met pathologische gegevens, waaronder TRG, percentage levensvatbare cellen, tumorgroeipatroon, tumordikte op het grensvlak, peritumorale micrometastasen en immuuninfiltraat in de tumor en het peritumorale gebied, en de moleculaire status.
Daarnaast zal elk verband tussen pathologiegegevens worden onderzocht
