취약 가정 아동 비만 예방을 위한 맞춤형 다중 구성요소 조기 중재 평가(밤부 프로젝트): 하이브리드 유형 1 효과성-이행 설계 무작위 대조 시험
소아 비만은 공중보건 문제입니다. 소아 비만을 예방하기 위해서는 초기 아동기를 대상으로 하며 가족의 사회경제적 맥락에 맞게 조정된, 근거에 기반한 다중 구성요소 양육 중재가 필요합니다.
본 연구는 어머니 및/또는 아버지가 과체중이거나 비만인 취약 가정을 대상으로 소아 비만 예방을 위한 1차 진료에서의 조기 중재 효과를 평가하는 것을 목표로 합니다.
보다 구체적으로, 본 연구의 주요 목표는 기존 치료와 비교하여 6개월부터 3세까지의 아동에게 건강한 체중을 유지하도록 하는 중재의 효과를 평가하는 것입니다. 이는 기준선, 6개월, 12개월, 18개월 시점의 연령별 체질량지수(BMI) z-점수를 통해 측정됩니다.
또한 본 연구는 기존 치료와 비교하여 중재의 효과를 다음과 같은 측면에서 평가하는 것을 목표로 합니다: 반응적 급여 촉진, 식이 섭취 개선, 부모와 아동의 신체 활동 수준 향상, 부모의 식사 스타일 개선, 아동의 수면 개선.
마지막으로, 본 연구는 중재 실행의 타당성을 확인하고, 실행 과정에서 변화에 영향을 미치는 요인과 맥락적 요인을 규명하는 것을 목표로 합니다.
실용적 군집 무작위 대조 시험이 수행될 예정입니다. 76개의 1차 진료 센터에 속한 약 526개 가정(부모-자녀 가족 쌍)이 모집될 것입니다. 1차 진료 센터는 무작위로 중재군 또는 대조군에 배정됩니다.
중재는 가족 및 의료 전문가와 함께 사전에 공동 설계되었으며, 각 2시간씩 진행되는 주간 세션 5회와 월간 세션 4회로 구성됩니다. 이는 사전에 훈련된 의료 전문가 또는 연구팀 구성원에 의해 수행될 것이며, 이는 1차 진료 센터 전문가들의 선호도와 가용성에 따라 달라질 것입니다. 중재군에 배정된 각 1차 진료 센터에서 약 38개의 대면 중재 그룹이 진행될 예정입니다.
본 연구는 스페인의 카탈루냐, 안달루시아 및 발레아레스 제도에서 수행될 것입니다.
효과성 결과는 아동 체질량지수(BMI) z-점수, 부모 급여 관행(급여 관행 및 구조 설문지를 통해 측정), 아동 식이 섭취(다중 패스 24시간 회상법을 통해 측정), 부모 식사 행동(네덜란드 식사 행동 설문지를 통해 측정), 아동 움직임(움직임 행동 설문지를 통해 측정), 부모 움직임 행동(간단한 신체 활동 평가 도구를 통해 측정), 아동 수면(간단한 영유아 수면 설문지의 선택 항목을 통해 측정)을 포함합니다.

- 취약 상황에 있는 가족으로, 다음 기준 중 ≥2개를 충족하는 경우로 정의됨: (1) 적어도 한 명의 부모의 낮은 교육 수준(중학교 졸업 이하 또는 이에 상응); (2) 지난 1년간 불안정한 고용 상황; (3) 지난 1년간 인지된 재정적 어려움; (4) 불안정하거나 불안한 주거 조건; (5) 한부모 가족 또는 주 양육자 중 한 명의 장기적 부재; (6) 안정적인 지원 네트워크 없이 또는 언어 장벽이 있는 최근 이민(≤10년).

실험적: Intervention group (Bambú program)
This arm will include all primary care centers assigned to the intervention group. All participants (parents) within these centers will receive the Bambú intervention. They will participate in a 9-session group-based face-to-face intervention. Sessions will last 2 hours. The first 5 sessions will be delivered weekly and the following 4 sessions will be delivered monthly. They will be delivered at primary care centers in Catalonia, Andalusia, the Balearic Islands and Castile and León. They will be led by two professionals, either healthcare professionals working at the primary care center (who will be previously trained) or by members of the research team. The following main topics will be covered in the program: (1) nutrition; (2) physical activity and interactive play; (3) emotion regulation; (4) positive parenting; (5) child sleep; and (6) community support networks. Approximately 38 groups will be conducted, and each of them will include around 7 families. |

The Feeding Practices and Structure Questionnaire is a self-administered questionnaire that measures parental feeding practices, specifically maternal responsiveness to children's hunger/satiety signals facilitated by routine and structure in feeding. Age- and feeding-stage-appropriate versions of the questionnaire will be administered: (a) the specific FPSQ-28 version for children aged ≥2 years; and (b) the FPSQ version for children aged <2 years, either the FPSQ milk-feeding version (for caregivers predominantly feeding their child with milk) or the FPSQ (semi-)solids version (for caregivers predominantly feeding their child with solid food. Each subscale is scored separately (range 1-5). No overall total score is calculated. Higher scores indicate greater endorsement of the feeding practice represented by the subscale. |

The Movement Behavior Questionnaire is a self-administered questionnaire that measures physical activity, screen time and sleep in infants, toddlers and preschoolers. The two age-specific versions of the questionnaire will be used: the MBQ-Baby (MBQ-B) for infants who have not yet reached the walking milestone, and the MBQ-Child (MBQ-C) for children who are able to walk. The closed-ended versions of the MBQ will be used. The questionnaire does not produce a total score. It assesses individual movement behaviours (e.g., tummy time, physical activity, screen time, restrained time, sleep duration). Interpretation depends on the behaviour measured and its alignment with age-specific movement guidelines. |

Parents' eating patterns will be measured with the self-administered Dutch Eating Behavior Questionnaire. The questionnaire comprises three subscales (Restrained Eating, Emotional Eating, and External Eating), each scored separately by averaging item responses (range 1-5). No overall total score is calculated. Higher scores indicate greater endorsement of the eating behaviour measured by each subscale; the interpretation of whether this represents a more or less desirable outcome depends on the subscale. |

Selected items (applicable to the whole children's age range included in the study) from the Brief Infant Sleep Questionnaire (BISQ) will be used to measure child sleep. The complete BISQ will not be administered because the original questionnaire is not appropriate for the full age range of the study population. Sleep parameters derived from selected BISQ items will be analyzed individually. No total sleep score will be calculated; therefore, there is no overall minimum or maximum score. The direction of interpretation depends on the specific sleep parameter assessed (e.g., longer sleep duration is generally considered favorable, whereas more frequent night awakenings or longer nocturnal wakefulness may indicate poorer sleep). |
