Het hoofddoel van deze lokale, multicenter studie is het onderzoeken van genetische en genexpressieveranderingen in de tumor voorafgaand aan en na progressie op ribociclib, tijdens de kernfase en vervolgens voorafgaand aan en volgend op progressie op alpelisib en zo patronen van mutaties te identificeren, hoe ze evolueren, en hun associatie met CDK4/6-remming en uitkomsten zoals aanhoudende respons of vroege progressie. De studie heeft ook tot doel de farmacogenomica en de associatie ervan met bijwerkingen (frequentie en ernst), interacties tussen geneesmiddelen en klinische resultaten te evalueren.
Ten slotte zal de studie ook aanvullende veiligheids- en werkzaamheidsgegevens op lange termijn genereren in deze specifieke Italiaanse populatie.

KERNFASE Opnamecriteria:
- Patiënt heeft een gevorderde (locoregionaal recidiverende of gemetastaseerde) borstkanker in eerstelijnsbehandeling (behandelingsnaïef voor de gevorderde setting).
- De patiënt bevindt zich in de postmenopauze, gedefinieerd door een van de volgende:
- Eerdere bilaterale ovariëctomie
- Leeftijd ≥60
- Leeftijd <60 en amenorroe gedurende 12 of meer maanden (bij afwezigheid van chemotherapie, tamoxifen, toremifen of onderdrukking van de eierstokken) en FSH en oestradiol in het postmenopauzale bereik volgens lokaal normaal bereik
- Patiënt heeft een histologisch en/of cytologisch bevestigde diagnose van oestrogeenreceptorpositieve en/of progesteronreceptorpositieve borstkanker door een plaatselijk laboratorium.
- Patiënt heeft een HER2-negatieve borstkanker gedefinieerd als een negatieve in situ hybridisatietest of een IHC-status van 0, 1+ of 2+. Als IHC 2+ is, is een negatieve in situ hybridisatietest (FISH, CISH of SISH) vereist door lokale laboratoriumtests.
- Patiënt is bereid bloed- en tumormonsters te nemen voor de biologische beoordelingen/doelstellingen zoals gepland in het protocol.
KERNFASE Uitsluitingscriteria:
- Patiënt die eerder is behandeld met een CDK4/6-remmer.
- Patiënt die eerder systemische hormonale therapie of chemotherapie heeft gekregen voor borstkanker in een gevorderd stadium.
Opmerking:
Patiënten die neo/adjuvante therapie voor borstkanker hebben gekregen, komen in aanmerking. Als de eerdere neo-/adjuvante therapie letrozol of anastrozol omvatte, moet het ziektevrije interval langer zijn dan 12 maanden vanaf de voltooiing van de behandeling tot aan het begin van de studie.
• Patiënten die ≤ 28 dagen letrozol of anastrozol kregen voor gevorderde ziekte voorafgaand aan opname in deze studie komen in aanmerking.
- Patiënt gebruikt momenteel een andere antikankertherapie. Andere in het protocol gedefinieerde opname-/uitsluitingscriteria kunnen van toepassing zijn.
VERLENGINGSFASE Inclusiecriteria:
- De behandeling met ribociclib + letrozol in de kernfase van de patiënt is (om welke reden dan ook) stopgezet en wordt geschikt geacht voor behandeling met alpelisib + fulvestrant in de tweede lijn. Ribociclib + letrozol moet het laatste behandelingsregime zijn vóór alpelisib + fulvestrant.
- Patiënt heeft PIK3CA-mutatie zoals bepaald in tumorweefsel en/of plasma door een door Novartis aangewezen laboratorium. Resultaten van weefselmonsters verkregen tijdens de kernfase (screening of EOT) zijn aanvaardbaar
VERLENGINGSFASE Uitsluitingscriteria:
- Patiënt is eerder behandeld met PI3K-remmers.
- Patiënt gebruikt gelijktijdig andere antikankertherapieën. Ribociclib en letrozol die in de kernfase worden gebruikt, moeten ten minste 7 dagen vóór dag één van de verlengingsstudiebehandeling worden stopgezet.
Alle geneesmiddelen met overlappende toxiciteiten moeten binnen 7 dagen worden gestaakt en AE moet worden opgelost tot NCI CTCAE v4.03 Graad ≤1 voorafgaand aan de studiebehandeling. Uitzondering op dit criterium: patiënten met elke graad van alopecia mogen deelnemen aan het onderzoek.
