Therasphere® voor de behandeling van inoperabele primaire leverkanker en gemetastaseerde leverkanker

Een humanitair hulpmiddelvrijstellingsprotocol van Therasphere® voor de behandeling van inoperabele hepatocellulaire en gemetastaseerde levertumoren - HDE #980006
Het doel van dit protocol is om onder toezicht en beperkte toegang tot Therasphere®-behandeling te bieden aan patiënten met primaire leverkanker en chemotherapie-refractaire levermetastasen die niet kunnen worden behandeld door chirurgische verwijdering van het aangetaste deel van de lever.
De respons van de patiënt op de behandeling en eventuele bijwerkingen van de Therasphere®-behandeling zullen worden onderzocht.

Deelname Criteria
Onderzoekers zoeken naar mensen die aan een bepaalde beschrijving voldoen, de zogenaamde geschiktheidscriteria. Enkele voorbeelden van deze criteria zijn iemands algemene gezondheidstoestand of eerdere behandelingen.

Beschrijving
Inclusiecriteria:
- Bevestigde diagnose van primair of gemetastaseerd intrahepatisch carcinoom
- De kanker moet inoperabel zijn met beperkte gevestigde behandelingsopties
- ECOG prestatiestatusscore 0-2
- Leeftijd 19 jaar of ouder
- In staat om schriftelijke geïnformeerde toestemming te begrijpen en te geven
Uitsluitingscriteria:
- Alle laboratoriumbevindingen vóór de behandeling binnen 15 dagen na de behandeling waaruit blijkt:
- Absoluut aantal granulocyten ≤ 1.500/ul
- Aantal bloedplaatjes ≤ 75.000/ul
- Serumcreatinine > 2,0 mg/dl
- Serumbilirubine ≥ 2,0 mg/dl
- Een van de volgende contra-indicaties voor angiografie en selectieve viscerale katheterisatie:
- Geschiedenis van ernstige allergie of intolerantie voor contrastmiddelen, narcotica, sedativa of atropine
- Bloedingen, diathese, niet corrigeerbaar door gebruikelijke vormen van therapie
- Ernstige perifere vasculaire ziekte die katheterisatie zou verhinderen
- Aanzienlijke veneuze shunt weg van de lever
- Bewijs van mogelijke afgifte van meer dan 11 mCi (20 Gy geabsorbeerde dosis) straling aan de longen bij ofwel 1) eerste toediening van Therasphere®; of 2) cumulatieve afgifte van straling aan de longen gedurende meerdere behandelingen
- Bewijs van een detecteerbare Tc-99 MAA-stroom naar de maag of twaalfvingerige darm, toepassing van gevestigde angiografische technieken om een dergelijke stroom te stoppen
- Significante extrahepatische aandoening die een dreigende levensbedreigende situatie vertegenwoordigt
- Ernstige leverdisfunctie of longinsufficiëntie
- Actieve ongecontroleerde infectie
- Aanzienlijke onderliggende medische of psychiatrische ziekte
- Zwangere vrouwen mogen niet deelnemen
